duly sworn to in accordance with law, do hereby say, that:

1. I was employed as a Seafarer at ANSCOR SWIRE SHIP MANAGEMENT

CORPORATION from March 24, 2019 up to present;

2. Currently, I am on the process for a Saudi Visa Application with the

Department of Foreign Affairs;

3. I hereby attest to the authenticity and genuineness of the certificates and other
relevant documents attached made as integral part of my records;

4. I am executing this Affidavit to attest to the truth of the above statements and
for whatever legal purpose this may serve.
