Provide the above information to the Project Sponsor. The Project Sponsor should either accept or reject
the project and include any comments. If the Project Sponsor is rejecting the project, the reason for
rejection must be provided. If the project is being approved, the Project Sponsor must sign the form and
enter the Date approved.
