Parental Consent Form

The Outer Edge requests that all charity event participants are aged 18 or over unless they are
accompanied by a parent or a legal guardian (a minimum age of 14 applies). A teacher or youth leader
acting in loco parentis is not adequate. The parent or legal guardian must complete this Parental
Consent Form prior to the event and will need to remain within sight and earshot of the child – this
will normally mean that they need to be with the minor whilst they prepare and accompany them as is
reasonably practicable during their participation. If your child is taking part and is aged 14-17, please
complete this form and bring it with you on the day.
