accept and agree that by allowing my/our child to participate in LOCAL WORLD YOUTH DAY
PREPARATIONS & CELEBRATION, I/we, the parents, or guardians named below, undertake, and agree to
indemnify and hold blameless the Parish Youth Ministry (PYM), Our Lady of Mount Carmel Parish
(OLMCP), and the organizers, Project 6, Quezon City for any accidents due to force majeure, untoward
incidents, the nature of the activities, misconducts, or negative behaviors of my child. This consent is
effective only when participating on or traveling to events of the OLMCPYM. I have read, understood,
