Client: Kate Smith #318

Case Manager: Aislynn Garvin

Date: 3/13/20204

D: Today’s appointment took place in the CM office in State College, Pennsylvania. Client

arrived on time to the session. Client was observed to be nervous at first as she was fidgeting and

seen breaking eye contact multiple times. Client seemed indifferent when talking about her

desires to work on her substance use issues and her loss of motivation in school. Client identified

feeling alone with little supervision as she is away from her family. She expressed not trying to

solve these presenting problems before. The client expressed interest in joining organizations on

campus to help her find friends. She also expressed interest in attending counseling so she “can

talk through her problems and seek advice from professionals”. Client appears to lack knowledge

on where to begin with those interests and doesn’t feel like her presenting problems are related.

At the end of the session, the client disclosed “wanting to get help but not feeling confident in

her abilities to follow through with it”.

A: Client appeared to be nervous as evidenced by the fidgeting and lack of eye contact while

sitting down. Client lacks enthusiasm and a clear motivation to work on her presenting problems

when discussing them. Client appeared overwhelmed by being alone and having to work on her

presenting problems with a lack of direct support from her family. Client seems more excited

about joining an organization on campus and hopefully finding a social fit there evidenced by a

more enthusiastic tone. Client seems to lack insight into how her substance use and lack of

motivation may be connected. During the session, the client seemed overwhelmed by the idea of
participating in classes and bringing her grades up which was evidenced by short answers and

avoiding eye contact.

P: CM will obtain lists of organizations of campus, counseling services, and AA meetings for

next week’s session. CM will provide a copy of the treatment plan to the client at next week’s

session. Next session is scheduled for 3/20/2024 at 2:30 pm.
