REPORT ACCIDENT IN THE WORKSHOP


Introduction

My Name Is Nicholas Smith Currently Attending Meadow Brook Highschool. I Am in the 11th
Grade at The Age Of 16 And One of Meadow Brook’s Mechanical Engineering Students Which
Utilize Scientific and Mathematic Principles to Devise Technical Solutions for Problems in
Construction, Engine Design, and Manufacturing. I am an Aspiring mechanical engineer who
wants to earn a bachelor’s degree in mechanical engineering at a minimum, and may someday
pursue graduate degrees in this field, it may be hard but with serious hard work and
determination, I think I would pull through.
Nunez Concrete & Construction Co. Ltd.

Nunez Concrete & Construction Co. Ltd. Is a family owned and operated business. It was
established in 1995 on the foundation of over 50 years of experience laid down by Jesus Nunez,
a Cuban immigrant who came to Jamaica in the 1930’s. Jesus along with his son Keith Nunez
formed a formidable team and they built a reputation for craftsmanship and quality in the precast
concrete and tiling business.
Today the management team have developed the company into one of the leading and most
respected concrete casting companies in Jamaica.
Nunez Concrete & Construction Co. Ltd. Has been supplying quality precast concrete products
to the Jamaican community for more than 20 years.
They boast a vast inventory of architectural columns, moldings, balusters and steppingstones.
They have the capability to handle the demands of major construction projects as well as the
needs of landscapers and do-it-yourselfers looking for a birdbath or fountain to complement that
special little spot in the garden.

PRODUCTS
HALESON PCB (S) PTE LTD
Shunde Hou Xin Electronic Parts Company Limited is a wholly
owned subsidiary of Haleson PCB Co. Limited. Hou Xin
Electronic Parts commenced production in the year 1992, with a
mere 60 people to start with. Haleson Singapore was then
established in 2006 to serve the customers in Southeast Asia,
Japan, America and the European region. It was an intensive
labor manufacturing with an annual production volume of just
400,000 square feet. Throughout the years of improvement, we
had consistently converted the dedication of our staff into
creating high quality products and have managed to gather
customer satisfaction, expanding our capacity and upgrading our
production technology as our customer’s demand increased.
Together, we grow along with our customers. Our staff is now
composed of around 700 employees, and our annual production
has reached 5 million square feet, eleven times of the initial
production volume. Our staff are proficient in the latest
technology, and our production process is highly automated in
order to achieve better quality.

Not only did we obtain ISO9000 certification in 1999 and


certifications such as the UL and ISO14000; in year 2004, we
passed the RoHS environment requirement in certified SGS in
PCB products series, and we have also obtained TS16949 in
2007 and will be obtaining IATF16949 in Oct 2017. During the
last few years, we have upgraded and introduced a new set of
equipment for advanced manufacturing processes. Through
continuous improvement in all areas, we have provided PCB –
Professional, Cost Effective and Best Service to our customers,
and we have achieved intangible (total customer satisfaction) in
every tangible aspect.

Safety Rules In At Workshop

There are many safety rules that apply to different types of workshops, such as
woodworking, metalworking, or electrical. However, some general workshop
safety rules and tips that are common to most workshops are:

 Wear safety gear such as goggles, gloves, ear plugs, and helmets when
appropriate.

 Follow the dress code and avoid loose clothing, jewelry, or long hair that
could get caught in machinery or tools.

 Maintain personal hygiene and wash your hands before and after handling
materials or equipment.

 Take responsibility for your personal safety and report any hazards,
accidents, or injuries to your supervisor or manager.

 Maintain a clean workspace and keep your tools and materials organized and
stored properly.

 Use the right tools for the job and do not improvise or use damaged or faulty
equipment.

 Follow the instructions and procedures for operating, maintaining, and
cleaning the machinery and tools.
 Do not work under the influence of alcohol, drugs, or fatigue.

 Be aware of your surroundings and watch out for moving parts, sharp
objects, hot surfaces, or electrical wires.

 Seek help or advice when in doubt or when facing a problem that you cannot
solve on your own.

REPORT ACCIDENT IN THE WORKSHOP

On November 22, 2008, Employee #1 was an equipment operator


at Ceradyne Inc., a manufacturer of advanced technical ceramic
products. A coworker had worked the day shift and set up a CNC
ID/OD Grinder, Type 1632 Gold, Serial No. 837, manufactured by
Weldon Machine Tools, in order to grind ceramic rings. The set up
entailed installing a 7 in. aluminum grinding wheel and
downloading a program from a laptop computer to the CNC
Grinder. Employee #1 worked the night shift and made
adjustments to the machine to grind ceramic parts. He changed
the wheel speed in the CNC program from 6,500 rpm to 13,000
rpm and turned on the machine. Other employees indicated that
they heard a loud and unusual sound for several seconds followed
by a loud popping sound. Employee #1 was injured when the
grinding wheel broke apart at the high rpm and pieces of it struck
him. He was hospitalized and died when he was taken off of life
support three days later.
