SNAP/TANF INSTRUCTIONS

Complete the Judicial Notice template.


If you have an active or open benefits case, mail the Judicial Notice form via registered mail with
confirmation and tracking to your local Social Service office.
If you are applying for the first time:
upload the judicial notice with your application when you apply online.
OR
apply in person, take a notarized copy of the judicial notice with you to submit with the application.
OR
mail it in registered mail with confirmation and tracking to your local Social Services office.

If you have recently applied and been denied benefits, file an appeal (if you are still within the allotted
time frame to file an appeal), and mail the judicial notice to the office you applied with in reference to
your denied application.
The Judicial Notice SHOULD BE NOTARIZED
The Judicial Notice SHOULD BE SUBMITTED TO THE HHS COMMISSIONER OR DIRECTOR
FOR YOUR COUNTY
MAIL DOCUMENT VIA REGISTERED MAIL WITH TRACKING AND CONFIRMATION
RECEIPT. DO NOT ALLOW THE POST OFFICE CLERK TO TALK YOU OUT OF SENDING
REGISTERED MAIL, IF THEY GIVE YOU ANY ISSUES, REQUEST THE MANAGER.
EVEN IF YOU HAVE UPLOADED A COPY OF THIS FORM ONLINE, STILL MAIL A COPY VIA
REGISTERED MAIL TO ENSURE THEY HAVE A COPY IN MORE THAN ONE WAY.
You can google the HHS commissioner or director; you do not need anyone to assist you with that.
When you fill out your application for benefits, be sure to do the following if it applies to your
circumstances:
- Never report your income, your income has nothing to do with you being eligible to get food
stamps or cash assistance. On the entire application report $0, even for your bills, assets, and any
other income info that they request in the application.
- Apply for food stamps, cash assistance, and Medicaid. If you are a female and do not wish to put
your child’s father on child support, when you apply, handwrite a letter, and upload it to the
portal with your application saying that you and your child’s dad have an open agreement for you
not to put him on child support because he helps with the child when he can. Tell them in the
letter that you have a verbal agreement with the father that he will not be placed on child support
in any way possible.
- If you currently work and are a W-2 employee, you can add an extra layer of security to further
benefit your odds at first-time approval by doing the following:
Freeze your work number with Equifax. Equifax is a credit reporting agency that stores and
furnishes info and data on its consumers. Equifax is not federally owned or organized; they are a
private 3rd party company that violates the rights of the consumers every day. You can freeze your
income and employment history, which will prevent other companies from seeing your personal
info without your consent which is a direct violation of the FCRA (Fair Credit Reporting Act).
This is your rights that you are unaware of, none of this is illegal or against the law. It’s your
consumer rights that were written a long time ago.

First Step- Fill out the FREEZE PLACEMENT FORM form that I provided.
You can mail, fax, or email the form back. All the instructions and where to send it are located on the
form. If you haven’t already applied for benefits, DO NOT APPLY UNTIL YOU GET
CONFIRMATION THAT THE FREEZE IS ACTIVE
