AFFIDAVIT OF ACCIDENT

I, JOHN RABBI C. UMAGUING, Filipino citizen, of legal age,

single, with postal and residence addresses at No. B 46 L 2 Amaia
Scapes Cavite, Brgy. Santiago, General Trias, Cavite, Philippines,
having duly sworn in accordance with law, do hereby depose and
state, that:

1. I am the duly licensed driver with Professional Driver’s

License No. N03-13-021693;

2. Likewise, I am the authorized driver of a motor vehicle

registered under MANG INASAL PHILIPPINES, INC.,
which is more particularly described as follows:

3. On May 24, 2022, at about 8:48 AM, I parked the

abovementioned motor vehicle in a vacant space near
7ELEVEN Convenient Store adjacent to crossing along
Aguinaldo Highway and Daang-hari, Imus City, Cavite, to
buy food for dinner;

4. When I came back, I noticed a big dent and scratch on

the side of the passenger seat door left side portion of the
aforesaid motor vehicle, (A copy of photo image of
damaged motor vehicle, hereinafter referred to as
Annex “A”;

5. I am executing this affidavit to attest to the truth of the

foregoing facts and for whatever legal purpose it may
serve.
