HSE Super Sack Hoist

ALERT Chain Parted

What happened?
Operation was mixing barite using super sack hoist. The big bag was placed on the mixing frame and after
it was lifted approx. 1 foot, the super sack hoist chain parted and the big bag landed inside the mixing
frame.
Consequences:
● Potential injury
● Property damage

Why did it happen?

Incident is still under investigation and the preliminary findings are:

● Chain parted due to material fatigue.

● OEM Preventive Maintenance (PM) was not followed.

What should be done to prevent reoccurrence?

● Visually inspect the hoist condition before use.
● Use certified chains for super sack hoist system.
● Follow the required OEM inspections and tests for the hoist chain.
