I, Shilpi D/O Kishan Sharma Aged About 19 Years R/O House No. RZ-A-
134, Rohtash Nagar, Mahavir Enclave Part-2, Uttam Nagar, West Delhi, Delhi-
110059 do hereby solemnly affirm and declare as under:

1. That I am a Citizen of India

2. That I belong to Hindu Religion.

3. That I got married to Mr. Ashok Kumar S/O Akhil Dev Aged about 22
Years R/O Bhagwanpur Desua, Samastipur, Bihar-848134 on 24.03.2023
at Arya Samaj Mandir, Ground Floor, Khirki Village, Press Enclave
Road, New Delhi-110017 according to Hindu Rites and Ceremonies.

4. That my date of birth is 13.02.2004 and at the time of marriage I was 19

Years old.

5. That I was unmarried till the time of marriage on 24.03.2023 and/or I did
not have any other living spouse at the time of marriage.

6. That at the time of marriage I was not related to Ashok Kumar S/O Akhil
Dev within prohibited Degree of relationship and not sapindas as per
Hindu Marriage Act, 1955.

7. That no pressure or undue influence was practiced upon me for my said

marriage.

8. That all supporting documents are genuine and nothing has been
concealed.

9. That at the time of marriage I was capable of giving valid consent and of
sound mind and not suffering from any mental disorder of insanity.
