GUIDELINES ON THE PREPARATION AND SUBMISSION OF THE SPECIAL ORDERS (SO)

FOR THE APPROVAL OF THE ELIGIBILITY FOR GRADUATION FROM SENIOR
HIGH SCHOOL OF GRADE 12 LEARNERS ENROLLED IN PRIVATE SCHOOLS,
COLLEGES AND TECHNICAL VOCATIONAL INSTITUTIONS

1 Pursuant to Regional Memorandum 110 s.2024 and Deped Order 10.s.2018, titled
“Guidelines on the Issuance of Special Orders (SO) for the Approval of the Eligibility
from SHS of Grade 12 Learners Enrolled in Private Schools/Colleges and Technical-
Vocational Institutions”, this Office informs the Private Schools of the following documents
to submit on or before March 31,2024.

2. The following documents (in BLUE colored folder) shall be submitted to the SDO
Private School Coordinator for evaluation and endorsement to Regional Office:

a. application letter by the School Head;

b. three (3) original copies of SO applications in long size bond paper, font is
Bookman Old Style and font size is 11;
c. one (1) original copy of List of graduating students signed by the School Head or
Registrar (per track if various tracks are offered);
d. one (1) Certified True Copy of the Permit to Operate SHS;

e. students’ Permanent Records duly signed by the School Head/Registrar.

3. Attached is the Special Order template (RO-QAD-FO87) for reference and guidance.

4. Immediate dissemination and strict compliance with this Memorandum are

directed.
