SILVESTRE C. BARRERA, JR. AND MARITES BARRERA who was born
on 27 April 2006 in Morong, Rizal;

2. That I have been under the care and custody of petitioners LUGOD
SILVESTRE C. BARRERA, JR. AND MARITES BARRERA since birth;

3. That the petitioners desire to adopt CARL JAMES M. VISMONTE as

their son;
4. That his adoption by the said petitioners is for his best interest;
5. That I hereby give my consent to this adoption by petitioners LUGOD
SILVESTRE C. BARRERA, JR. AND MARITES BARRERA; and

6. That this affidavit is executed in order to attest to the truthfulness of the

foregoing narration of facts and as basis by any offices concern to approve my
adoption and for whatever purpose or purposes it may serve.
