AFFIDAVIT OF ACCIDENT

I, KENNETH GALANG, Filipino citizen, of legal age, single

with postal and residence address at No. B 88 L 2 P 3 Langka
St., Marycris Complex Subd., Pasong Camachile II, General
Trias, Cavite, Philippines, after having been duly sworn to in
accordance with law hereby depose and state that:

1. That, I am the authorized driver of the following described-

motor vehicle, to wit:

abovementioned motor vehicle is duly registered to
JAN ERICK ABAY ALIM;

3. That, on December 9, 2022, at around 6:00 P.M., the above-

stated motor vehicle was travelling along General Yengco St.,
Imus City, from Nueño Avenue bound to General Trias,
Cavite. In the front a motor vehicle hazard signal light
turned on blinking so I stopped and could not move forward.
A motorcycle at the back was travelling similar lane
accidentally and inadvertently hit the rear bumper of the
abovementioned motor vehicle;

4. That, the above-said motor vehicle incurred of an estimated

cost of vehicular damages;

5. That, I am executing this Affidavit to attest to the

truthfulness of the foregoing facts and for
whatever legal purpose it may serve.
