1. Introduction
This Health Safety & Environmental (HSE) Plan is to cover the work of
CHITALAND TOWER PROJECT which is to be carried out by Lotte Engineering &
Construction - Totalindo Eka Persada (hereinafter referred as LTJO).
The Site HSE Plan is prepared based on the LTJO HSE management system and the
related standards to pursue LTJO HSE Policy and fulfil the Commitment.

Project HSE Policy, Objective and Commitment are established in accordance with
the LTJO HSE management systems and strategies which are compatible with the
relevant HSE codes and standards internationally recognized.
2.2. Objectives
No Fatalities.
Zero Lost Time Incidents (injuries and work-related
illnesses).
Minimum medical treatments and First Aids cases.
Continuing reduction of incidents which could lead to any of
the above.
No spills or environmental releases/impacts under the Contractor’s
control.
All wastes disposed of in full compliance with the related laws of
Indonesia, the Employer, Sponsors, and Contractor’s rules and
procedures.

3. HSE Responsibilities
Indonesia
Monthly safety status routinely reported from construction sites. The HSE team, based on
an analysis of these status reports, prepares a monthly comprehensive safety report giving
necessary advice for improving HSE control. This report is reviewed by the Site Manager
and distributed to each discipline and other related parties.
Upon request from the site, or when deemed necessary, the HSE Team provides

information in the form of news, letters, various data, HSE slogans, targets and warnings
derived from accidents/near misses. This is to encourage the safety consciousness of
assigned personnel at site.
The HSE Team not only continuously updates procedures and formats, but also keeps track
of the material and tools needed for HSE control at site.
Assessment of HSE performance for construction site is done once a year. The sites which
have recorded outstanding performance receive commendations from the chairman of the
HSE committee as a motivation towards better HSE performance.
In order to realize the safety targets, Site Manager shall establish an independent Site HSE
Committee comprising of Contractor and subcontractors’ site representatives. They shall
jointly act for the site HSE control with a common understanding of Contractor’s HSE
program.

The committee is lead by Contractor’s Project Manager. However, a chairman for the
committee operation will be selected from among the members.
Highlight high risk works and coordinate necessary measures among parties involved
in the works.
Summarize the results of patrol and discuss necessary measures for improvement.
6. HSE MEETING
6.1. Pre-Construction Meeting
The pre-construction meeting addresses health, safety and environmental requirements and
necessary preparations for each work party. High risk work, potential hazards and specific
rules/restrictions are also highlighted and discussed.
The pre-construction meeting will confirm the following between Contractor and
subcontractors:

Contractor plans several types of HSE meetings considering the nature of the site, work
formation in the project, classification of attendees, project progress and current aspects,
etc.
Contractor plans an orientation and training program by which all persons at site will be fully
aware of site rules, restrictions and necessary safe work methods.
The orientation and training programs will be conducted by the following parties:

Each subcontractor with contractor’s support/attendance

Emergency and Rescue
As general requirements, the following protective equipment will be considered for each
type of work:
Contractor’s site supervision will ensure that these facilities are installed or provided,
taking into account working site condition and work progress, to prevent any personal
injuries.
Each Subcontractor must include in his submitted safety program, a procedure relating to
construction equipment and vehicle inspection, operation, and maintenance plan. It must be
consistent with the manufacturer's recommendations and Employer’s and Contractor's
requirements as contained in the program.
All cranes and lifting equipment shall be thoroughly examined by an approved qualified
person as applicable by local law and regulations. A report on the result of examination
shall be submitted (in a prescribed form) before equipment is brought into site.
Operators of all cranes and other heavy equipment must be physically and mentally fit to
operate the assigned equipment. They should be well trained to operate the particular type
of equipment. Operators’ qualification for cranes and heavy equipment are subject to
review by the competent equipment supervisor and safety manager.
Construction equipment and vehicles found to be unsafe for operational use shall be tagged
“Out of Service, Do not use” at the operator’s position. Their use shall be strictly
prohibited until they are repaired and certified as fit for use.
(1) Possession of valid driving a license from the government and job site authorization
card signed by Contractor’s HSE Manager.
(2) Successful completion of a practical operating examination administered by
competent and authorized personnel.
(3) Training - Operator responsibilities, familiarity and comprehension concerning all
crane safety and O&M requirements, load capacity charts, rigging methods and
practices, hand signals and other requirements will be examined by competent and
authorized personnel.
All construction equipment and vehicles will be clearly identified with the appropriate
names and equipment serial numbers.
9.6.2. Insulation
During routine inspections and tests, the insulation of electrical equipment and the
associated items will be checked. Where it is found to be damaged or incomplete the item
will be removed from the service.
All wiring will be insulated thoroughly and where there is the possibility of physical
damage, the wiring must be further protected either by metal conduits or armoring.
Insulation shall be provided at all locations where it is possible for workers to come into
contact with live equipment.
9.6.3. Earthing
Earth cables will be tested for continuity and resistance to ensure they are effective for
that particular application. Remedial action will be taken where they are found inadequate
or ineffective. Until such remedial action is taken the item of equipment will be taken out
of service.
Work shall officially be carried out on electrical systems until a competent or authorized
person has indicated, in writing, on a work permit that he has isolated the system by
means of locks using the lock out/tag out procedure. The person issuing the work permit
must witness the isolation.
All electrical systems shall be signed with appropriate warning signs. Work adjacent to
electrical systems shall be subject to work permit procedures even where surrounding
areas do not come under such procedures.
Contractor’s site management and supervision will ensure that all drivers are competent
and hold a valid license for the class and type of vehicle or equipment driven or operated.
Each driver or operator must have completed the Site Induction Course before driving or
operating vehicles and equipment on site.
10.2. Traffic Safety Regulations
Contractor’s site management and supervision will monitor and strictly enforce the
following traffic safety rules.
Each driver or operator will be given a copy of the site traffic safety regulations and
regular compulsory Tool Box talks.
Drivers and operators will fully comply with all traffic safety regulations, traffic
controls and signs.
The maximum speed limit on the construction site is 25 km/h. The maximum speed
limit in work areas is 10 km/h.
Drivers or operators who blatantly violate the site traffic safety regulations will be
permanently removed from the site.
Personnel must not be transported while standing on the bed of trucks or trailers.
Personnel will not ride on the body of material handling or earth moving equipment.
Vehicles must be frequently inspected and properly maintained. Each vehicle must be
equipped with the required safety equipment in good working order, e.g., reversing arms,
flashing lights, etc.
Contractor and subcontractors will plan the prevention and measures for the worst possible
conditions from the initial stages of construction planning. These plans will include an
emergency evacuation plan. Fire drills and emergency evacuation exercises will be
conducted regularly.
Contractor’s site management will ensure that workmen are trained in the use of fire
extinguishers, and the basic skills of safe firefighting.

11.2. Housekeeping
Contractor’s management and supervision will ensure that sites and work are maintained to
a high standard of housekeeping at all times, including

In addition to these inspections, periodic management inspections and joint inspection by
the site safety committee will be organized and conducted.
In order to provide adequate sanitary conditions for all personnel at site, Contractor
considers the following provisions as a part of the temporary facilities. If they shall be
provided by subcontractors, Contractor will indicate guidelines for planning, installation
and operation.
Provision for an adequate supply of potable water
Provision for toilets and hand wash basins

11.3.4. Illumination
Adequate lighting is essential to ensure safety in work areas, quality work and
productivity. Contractor’s site management and supervisors will make sure that all work
areas and places are properly illuminated.
11.3.5. Ventilation
During construction activities, airborne contaminants may be created which have harmful
effects on the respiratory and circulatory system. Workmen must be protected against
these harmful effects through effective controls, i.e. elimination, isolation or the use of
personal protective equipment (PPE).
Contractor’s site management will take the necessary action as practicable and possible to
control and minimize the harmful effects of airborne contaminants such as dust, fumes,

The areas of pollution can be classified into the following four categories:

Noise, Vibration and Odor Nuisance
Water Pollution
Air Pollution
It is most important that solid waste materials be segregated according to the nature of the

materials and the method of disposal. Mixing all solid materials will cause difficulties in
disposal and hamper the smooth operation.
Recyclable steel material for disposal shall be handed over to enterprises which can
change these materials into usable form if such enterprises are available in the country of
the construction site.
Material which do not cause any contamination to the surrounding soil through the
process of leaching shall be buried in an excavated pit. Such pits will be approved and
designated by Employer or local authorities.
If materials are combustible and do not produce an adverse influence on air pollution,
they shall be burnt in a furnace. If the materials are combustible but produce a harmful
emission of gases when burnt, it is prohibited to do so.
The disposal of solid chemicals shall be determined carefully by examining the nature and
characteristics of the chemicals.
Solid toxic chemicals shall never be disposed of in any unauthorized place. The method of
disposing of these materials shall be studied and determined at the beginning of the
project, if the production of such chemicals is expected.
The best way to reduce these nuisances is to eliminate sources from the working site.
If the degree of nuisance may exceed an acceptable level, the following measures shall be
considered in the work planning:
For noise and vibration nuisance, workers who are directly involved in the noise

producing work shall wear appropriate noise protection equipment such as ear plugs and
ear mufflers. This will cover people working in the proximity as well. Duration of such
noise producing work shall also be considered. For example, short working hours are
preferable, so work shall be arranged not continuously but intermittently.
For odor nuisance, the work shall be limited to confined or enclosed buildings. The
worker shall wear an appropriate filter type face mask. Odoriferous materials shall be
stored in an enclosed container when not in use. The duration of exposure to the
atmosphere shall be reduced as much as possible.
(3) Water Pollution
Due to the concentration of a large workforce in a limited place, the site office, temporary
site facilities and camp are sources of sewage that must be properly managed.
In process construction, the biggest source of air pollution is the exhaust gas from
construction equipment.
Reduction of polluted air emissions from construction equipment can be attained by good
maintenance of internal combustion engines and by avoiding use of equipment that has
old, deteriorated engines.
Contractor’s construction management and supervision will check type and year of
manufacture of subcontractor's construction equipment during the construction planning.

If these are very old or seem to have deteriorated engines, they will instruct the
subcontractors to take appropriate measures such as overhauling engines and improving
the frequency of maintenance service.
Contractor’s site supervisors will provide or instruct the subcontractors in its installation,
according to each worksite working conditions in various areas.

Site management and supervision will ensure that appropriate and adequate quantities of
signs, barricades and signaling supplies are available and are used, erected and maintained
as required to ensure the safety and health of site personnel.
Contractor will ensure that all management staff, supervisors and workmen on the site fully
understand the procedures, and the actions to be taken during emergency situations.

Designate first aid/medical supplies, and other material and equipment that may be
required for emergency use.
Set up emergency response teams.
Determine emergency evacuation routes for work areas and the construction site.
Establish a personnel staging area or assembly points for evacuees.
Make sure that all personnel clearly understand emergency warning siren signals or
other warning signals used by the Employer, Contractor and/or other authorities.
Provide summarized communication diagrams and necessary maps, and display them
at proper places in the site.
Designate a supervisor to conduct a head count and give instructions to evacuees as
necessary.
Post emergency information for each work area on the site.
Conduct emergency procedure drill periodically.
Contractor and subcontractors will provide or make arrangements to provide adequate first
aid facilities, equipment and qualified attendants for work site.
The following are key considerations in the preparation of first aid facilities at site:
Experienced and knowledgeable personnel who can lead the operation and give

12.2.3. First Aid Treatment
First aid facilities shall be readily available to all personnel and at all times at site.
Records of all types and causation of minor injuries and sickness will be maintained and
analyzed by Contractor.
(1) Contractor and Employer teams for periodic inspections will be formed consisting of
the following members:

Contractor’s Site HSE Manager
Employer’s Representative
Employer’s HSE Manager
(2) A standard HSE inspection checklist will be used to conduct each inspection.
(3) The Site HSE Manager will prepare a written report on the HSE problems/deficiencies
identified during inspection of each facility and work area.
(4) When potential deficiencies are found, immediate corrective actions will be taken for

incident/accident prevention.
(5) Reports on the HSE deficiencies will be kept in site office for statistical purposes and
analysis.
(6) Contractor will instruct each subcontractor to submit a weekly report of site inspection
using a standard form.
(7) Crane operator is required to check his crane every day using the prescribed form.
13.2.1. Investigations
All accidents, incidents and near miss occurrences will be investigated. The cause or the
chain of events that led to the accidents/incidents and near-miss must be determined.
When the cause is known, appropriate actions will be taken to prevent recurrence.
Accident investigations will be conducted without prejudice. The investigations will focus
on finding the facts, not fault or placing blame.
Any accidents will be reported to the Employer and the Managing Director in head office
by facsimile, telephone or e-mail.
Using information from the reports and records, management can determine weak areas in
the HSE plan. These weak areas can then be targeted for corrective action. Analysis of the
data will show the type of corrective action required. This may be training and education,
motivational and awareness schemes, new equipment and tools or new working methods.

Site management and supervisors will perform hazard analysis as required to identify
and eliminate the dangers of high risk and activities.
Monitor the site, work areas and jobs being performed on a routine and timely basis.
Site management and supervisors will take immediate action to ensure hazards are
made safe and unsafe acts corrected.
Supervisors will give clear instructions for the work to be performed. Workmen will
be made aware of the hazards of their work.
Adequate and appropriate personal protective equipment will be provided for the
work to be performed.
As required, workmen will be given training to ensure they have the knowledge to
perform their job safely.
Communications will be maintained between supervisors and workmen to ensure
effective hazard control and the elimination of unsafe acts.
Supervisors will respect and take into consideration the suggestions and
recommendations made by workmen concerning the work performed. Immediate
action will be taken to eliminate reported hazards and unsafe acts.

The consumption of any intoxicating substances during working hours or within such time
previous that a person would still be considered intoxicated is prohibited.
Any such substance such as alcoholic beverages, illegal drugs, inhalants and prescription
drugs are not to be consumed during working hours or within such time as the person would
still be mentally or physically impaired.
The use of prescription drugs during working hours is prohibited unless verification from the
prescription physician states that the medication would not impair the person’s ability to
perform their specific duties. This verification must be in writing from the physician and on
the office letterhead.
Contractor will implement education of the workforce at site safety orientation courses and
safety awareness campaigns of the dangers and consequences of substance abuse during
employment on the site to ensure compliance with the substance abuse policy.
As standard policy on all security access points and worksite locations, Contractor’s security
staffs are given the power to stop and search person on site, drivers of motor vehicles are
observed carefully.

Any person found to be intoxicated will be immediately refused access to the site or removed
from its normal duties.
