SAFETY PLAN

1. Introduction

This Health Safety & Environmental (HSE) Plan is to cover the work of
CHITALAND TOWER PROJECT which is to be carried out by Lotte Engineering &
Construction - Totalindo Eka Persada (hereinafter referred as LTJO).

The Site HSE Plan is prepared based on the LTJO HSE management system and the
related standards to pursue LTJO HSE Policy and fulfil the Commitment.

All personal, including subcontractor’s employees, angaged in the project


construction work shall comply with following laws, regulation, rules and procedures
in the given order of priority.

 Government Laws and Regulations.

 Local Municipal Laws and Regulations.

 Employer’s rules, regulations and requirements.

 Contractor’s HSE program, procedures, standards and practices

2. HSE Policy, Objective & Commitment

Project HSE Policy, Objective and Commitment are established in accordance with
the LTJO HSE management systems and strategies which are compatible with the
relevant HSE codes and standards internationally recognized.

2.1. HSE Policy

The Contractor’s policy consists of the following principles


 Each of us has a right and duty to intervene with unsafe acts and
conditions or when activities are not in compliance with this HSE
Policy and Commitment”

 Has a systematic approach to HSE management designed to ensure


compliance with the law & to achieve continuous performance
improvement;

 Personnel safety and protection of environment at and around the site


for work are paramount.
 All accidents and injuries can be prevented.
 All tasks must be planned and performed with concern for the safety

and security of the staff and the protection of the environment.


 All employees, including suppliers and subcontractors, are responsible
for their own and their colleagues’ health and safety at work.
 Management must train employees to work safely.
 Full cooperation and liaison must be offered to the Employer, statutory
authorities and local communities.

2.2. Objectives

The following are the health and safety objectives of CHITALAND


TOWER PROJECT which all the participants of the Project shall subscribe
and strive to achieve:

 No Fatalities.
 Zero Lost Time Incidents (injuries and work-related
illnesses).
 Minimum medical treatments and First Aids cases.
 Continuing reduction of incidents which could lead to any of
the above.
 No spills or environmental releases/impacts under the Contractor’s
control.
 All wastes disposed of in full compliance with the related laws of
Indonesia, the Employer, Sponsors, and Contractor’s rules and
procedures.

good engineering and sound environmental practices.

3. HSE Responsibilities

3.2. HSE Manager


 Establishes & manages HSE program complying with the
acknowledged international HSE standards

 Ensures that Contractor ‘s execution is to fulfil the Project

 Ensures that HSE law of Indonesia is known and adhered to in their
area of work
 Provides necessary HSE resources required for the Project
 Ensures that sites conditions, equipments & activities allow safe work
execution
 Ensures that Hazards Identification and Risk Assesment and Risk
Control (HIRARC) is practiced for site safety management and
supervision
 Ensures that only competent workers are assigned for tasks so as to
avoid accident, incident and/or near miss due to lack in required skill
for a task.
 Ensures that adequate safe work inductions, talks and initiatives are in
force
 Ensures that engagement of total workforce is in accordance with safe

3.3. HSE Supervisor (s)

Implement HSE training in accordance with the HSE Training
Procedure.
Conduct , Safety Indoctrination, monitoring & auditing activities in
accordance with the relevant procedures.
 Practice Hazards Identification and Risk Assesment and Risk Control
(HIRARC) in site safety supervision
 Define & implements HSE improvement activities
 Supervise Daily Activities in accordance with the HSE Plan
and the associated procedures to ensure that Environmental
Commitment is fulfilled

4. HSE CONTROL AND MANAGEMENT

4.1. Monitoring Safety Status on Construction Site

Monthly safety status routinely reported from construction sites. The HSE team, based on
an analysis of these status reports, prepares a monthly comprehensive safety report giving
necessary advice for improving HSE control. This report is reviewed by the Site Manager
and distributed to each discipline and other related parties.

In case of serious personal injury/fatality or property damage, the accident will be


immediately reported to the Site Manager who will take prompt action regarding corrective
measures and give instructions to site in case of an emergency

4.2. Coordinating for Effective HSE Control

Upon request from the site, or when deemed necessary, the HSE Team provides

information in the form of news, letters, various data, HSE slogans, targets and warnings
derived from accidents/near misses. This is to encourage the safety consciousness of
assigned personnel at site.

4.3. Providing Procedures, Formats, Material/Tools for HSE Control

The HSE Team not only continuously updates procedures and formats, but also keeps track
of the material and tools needed for HSE control at site.

4.4. Monitoring and Audit at Construction Site

To accentuate and improve HSE consciousness, Construction Department periodically


dispatches a safety auditor to site. It is his duty to check site performance and, whenever
necessary, directly implement measures for improving HSE control.
4.5. HSE Performance Evaluation of Construction Site

Assessment of HSE performance for construction site is done once a year. The sites which
have recorded outstanding performance receive commendations from the chairman of the
HSE committee as a motivation towards better HSE performance.

4.6. Motivating the Safety Performance of Collaborating Subcontractors

Safety performance of all collaborating subcontractors is evaluated once a year according


to Contractor’s award criteria scheme. Subcontractors who have attained outstanding safety
performance will be presented a Contractor safety award.

5. SITE HSE COMMITTEE

In order to realize the safety targets, Site Manager shall establish an independent Site HSE
Committee comprising of Contractor and subcontractors’ site representatives. They shall
jointly act for the site HSE control with a common understanding of Contractor’s HSE
program.

Employer’s participation in the committee at an advisory level is appreciated so that their


requirements are commonly understood. It will also enable all participants to closely
recognize their restrictions and site conditions.

The committee is lead by Contractor’s Project Manager. However, a chairman for the
committee operation will be selected from among the members.

The members consist of:


 Organizer : Contractor’s Project Manager
 Advisor : Contractor’s HSE Manager & Employer’s HSE Representative
 Members : Subcontractors' Construction Manager and Safety Officers

Major roles of the Site HSE Committee are as follows:

5.1. Weekly HSE Meeting


Typical activities are to ;
 Establish detailed common safety targets and necessary actions at site for all
participants.
 Decide and conduct site HSE events and activities for the motivation of HSE
consciousness among all participants.
 Review safety performance and statistics of all participants. Jointly evaluate their
causes and consequences.
 Study necessary measures for improvement.

 Highlight high risk works and coordinate necessary measures among parties involved
in the works.

5.2. Daily HSE Patrol / Inspection


Typical activities are to ;
 Decide the patrol/inspection schedule and items to be monitored.
 Conduct the patrol, inspect site condition and activities.

 Summarize the results of patrol and discuss necessary measures for improvement.

5.3. Safety Orientation and Training


Monitor training activities to ensure compliance with requirements which are detailed in
this document.

5.4. Safety Award


Beside Contractor’s corporate safety award program, the Site HSE Committee plans a
safety award program.

6. HSE MEETING
6.1. Pre-Construction Meeting

A pre-construction meeting is conducted by Contractor with each subcontractor before the


actual start of work.

The pre-construction meeting addresses health, safety and environmental requirements and
necessary preparations for each work party. High risk work, potential hazards and specific
rules/restrictions are also highlighted and discussed.

The pre-construction meeting will be held with the following participants:

 Contractor’s Project Manager


 Contractor’s HSE Manager
 Contractor’s Superintendents / Supervisors

6.2. TYPE HSE MEETING

Contractor plans several types of HSE meetings considering the nature of the site, work
formation in the project, classification of attendees, project progress and current aspects,
etc.

The following typical meetings summarize standard modes of operation, objectives,


discussion subjects, etc. These shall further be developed in taking project features and
execution formation into account.

7. SAFETY INDUCTION AND SUPERVISORY AWARENESS TRAINING

Contractor plans an orientation and training program by which all persons at site will be fully
aware of site rules, restrictions and necessary safe work methods.

Contractor will recognize and reward subcontractors for their outstanding performance
and contribution towards HSE program.
Their performance is evaluated annually by the Corporate HSE Committee and awards are
granted by Contractor in accordance with "Contractor Recognition Awards Scheme for
Outstanding Safety Performance".

Contractor’s Site Manager and members of the Site HSE Committee.

8. PERSONAL PROTECTIVE EQUIPMENT AND FACILITIES

8.1. Personal Protective Equipment (PPE)


Contractor considers it a responsibility to provide suitable personal protective equipment
and to enforce its use by workmen.
Contractor site supervision will ensure that subcontractors and their supervisors provide
workmen with suitable protective equipment as required by the work conditions.

As general requirements, the following protective equipment will be considered for each
type of work:

 Helmet (hard hat)


 Goggles, face mask (if required)
 Dark eye glasses (during the day only, if required)
 Clear eye glasses (during the night, if required)
 Face visor (if required)
 Ear plugs and/or ear muffs (if required)
 Hand gloves (if required)
 Safety shoes and boots
 Equipment and clothing for body protection (if required)
 Safety belts at grade and full body harnesses for heights (if required)
 Respiratory (breathing) protection (if required)

Contractor’s site supervision will ensure that these facilities are installed or provided,
taking into account working site condition and work progress, to prevent any personal
injuries.

9. CONSTRUCTION EQUIPMENT AND VEHICLES


9.1. Pre-Construction Examination and Inspection
Contractor will ensure that, in the early stage of construction work planning, subcontractors
survey the need for construction equipment and vehicles. They should provide a list of
equipment and vehicles needed to safely perform the work. Need for cranes, hoists, slings,
etc., are analyzed considering the load limits as per manufacturer's recommendations.

Each Subcontractor must include in his submitted safety program, a procedure relating to
construction equipment and vehicle inspection, operation, and maintenance plan. It must be
consistent with the manufacturer's recommendations and Employer’s and Contractor's
requirements as contained in the program.

All cranes and lifting equipment shall be thoroughly examined by an approved qualified
person as applicable by local law and regulations. A report on the result of examination
shall be submitted (in a prescribed form) before equipment is brought into site.

Operators of all cranes and other heavy equipment must be physically and mentally fit to
operate the assigned equipment. They should be well trained to operate the particular type

of equipment. Operators’ qualification for cranes and heavy equipment are subject to
review by the competent equipment supervisor and safety manager.

9.2. Inspection Schedule and Maintenance


All Subcontractors' construction equipment and vehicles are to be periodically inspected
and maintained according to pre-determined schedule. Complete record of all inspection
and maintenance are to be maintained for Contractor's review and approval.

Construction equipment and vehicles found to be unsafe for operational use shall be tagged
“Out of Service, Do not use” at the operator’s position. Their use shall be strictly
prohibited until they are repaired and certified as fit for use.

9.3. Equipment Operator Qualification and Training


Contractor and subcontractors shall review the qualifications of all crane operators, crane
maintenance personnel and other mobile equipment operators in accordance with the
following procedure:

(1) Possession of valid driving a license from the government and job site authorization
card signed by Contractor’s HSE Manager.
(2) Successful completion of a practical operating examination administered by
competent and authorized personnel.
(3) Training - Operator responsibilities, familiarity and comprehension concerning all
crane safety and O&M requirements, load capacity charts, rigging methods and
practices, hand signals and other requirements will be examined by competent and
authorized personnel.

9.4. Guarding and Safety Devices


Contractor’s Site management and supervision shall ensure that all equipment and vehicles
are equipped with guards and that all safety devices required by law and site regulations are
fitted and effective.

9.5. Construction Equipment and Vehicles Identification

All construction equipment and vehicles will be clearly identified with the appropriate
names and equipment serial numbers.

9.6. Electrical Appliances Safety


9.6.1. Electrical appliances and equipment
Electrical appliances and equipment and the associated boards & cables will be inspected
and tested by a competent electrical charge-man according to Contractor and

subcontractors’ preventative maintenance procedures. During operation of these


equipment, if items are found to be faulty, they will be repaired immediately. If they
cannot be repaired they will be removed from the site. Records will be kept to show such
action.

9.6.2. Insulation
During routine inspections and tests, the insulation of electrical equipment and the
associated items will be checked. Where it is found to be damaged or incomplete the item
will be removed from the service.

All wiring will be insulated thoroughly and where there is the possibility of physical
damage, the wiring must be further protected either by metal conduits or armoring.

Insulation shall be provided at all locations where it is possible for workers to come into
contact with live equipment.

9.6.3. Earthing
Earth cables will be tested for continuity and resistance to ensure they are effective for
that particular application. Remedial action will be taken where they are found inadequate
or ineffective. Until such remedial action is taken the item of equipment will be taken out
of service.

9.6.4. Isolation Procedure


Detailed as part of the Permit-To-Work system, isolations are intended to prevent
accidental contact with live electrical equipment.

Work shall officially be carried out on electrical systems until a competent or authorized
person has indicated, in writing, on a work permit that he has isolated the system by
means of locks using the lock out/tag out procedure. The person issuing the work permit
must witness the isolation.

9.6.5. Work in the Vicinity of Live Equipment

All electrical systems shall be signed with appropriate warning signs. Work adjacent to
electrical systems shall be subject to work permit procedures even where surrounding
areas do not come under such procedures.

10. VEHICULAR AND TRAFFIC SAFETY


Contractor considers the following approaches to eliminate traffic accidents from the
construction site:

10.1. Driver Qualifications and Training

Contractor’s site management and supervision will ensure that all drivers are competent
and hold a valid license for the class and type of vehicle or equipment driven or operated.

Each driver or operator must have completed the Site Induction Course before driving or
operating vehicles and equipment on site.
10.2. Traffic Safety Regulations
Contractor’s site management and supervision will monitor and strictly enforce the
following traffic safety rules.

 Each driver or operator will be given a copy of the site traffic safety regulations and
regular compulsory Tool Box talks.
 Drivers and operators will fully comply with all traffic safety regulations, traffic
controls and signs.
 The maximum speed limit on the construction site is 25 km/h. The maximum speed
limit in work areas is 10 km/h.

 Drivers or operators who blatantly violate the site traffic safety regulations will be
permanently removed from the site.
 Personnel must not be transported while standing on the bed of trucks or trailers.
 Personnel will not ride on the body of material handling or earth moving equipment.

10.3. Vehicle Maintenance & Inspection


Contractor’s site management and supervision shall ensure that the subcontractors and their

employees will comply with the following requirements:

Vehicles must be frequently inspected and properly maintained. Each vehicle must be
equipped with the required safety equipment in good working order, e.g., reversing arms,
flashing lights, etc.

 Preventive maintenance will be performed on all vehicles on a monthly schedule. All


safety features will be checked and repaired as necessary to ensure proper operation.
 Drivers and operators will perform a pre-operational check of their assigned vehicles
and equipment at the start of each working day.
 Each driver shall keep a record of the daily and monthly pre-operational vehicle and
equipment checks.

10.4. Motor Vehicle Accident Investigations


Contractor’s site management and supervision shall give appropriate instructions and
ensure full compliance of the following requirements:
All motor vehicle and equipment accidents shall be reported and investigated. Investigation
and causation analysis is necessary to implement corrective actions to prevent recurrence.

10.5. Traffic Controls & Signs


Contractor’s site management and supervision will ensure that traffic control and signs are
displayed as required for the site.
The following traffic signs are mandatory as required by the conditions of site:
 Stop and yield signs
 Speed limit signs as follows:

b. Maximum speed limit in work areas is 10 km/h
 Pedestrian crossings

11. WORK ENVIRONMENT AND SITE CONDITIONS

11.1. Fire Prevention and Protection

Contractor and subcontractors will plan the prevention and measures for the worst possible
conditions from the initial stages of construction planning. These plans will include an
emergency evacuation plan. Fire drills and emergency evacuation exercises will be
conducted regularly.

Contractor and subcontractors will implement fire prevention by housekeeping, training,


meetings, safety patrol, site inspection, daily work instructions, etc. Contractor and
subcontractor supervision will make workers fully aware of fire prevention policy and
strategy through these activities.

11.1.1. Fire Fighting Equipment


Contractor’s site management will ensure that adequate and well-maintained fire
extinguishers are provided for each construction site. Contractor’s site management will
also ensure that subcontractors provide an adequate quantity of well-maintained
extinguisher for their premises, equipment and vehicles.

11.1.2. Fire and Emergency Simulations


Contractor will arrange simulation opportunities for fire extinguisher training and site
evacuations every six months.

11.1.3. Reporting a Fire


Fires will be reported to Contractor’s Site Manager and the Site HSE Manager. Then,
corrective actions can be determined and taken to prevent recurrence.

11.2. Housekeeping
Contractor’s management and supervision will ensure that sites and work are maintained to
a high standard of housekeeping at all times, including

 Securing of clear access


 Elimination of potential fire causes
 Maintaining a safe work environment
 Maintaining a healthy work environment

All supervisors at site including Subcontractors will give daily inspections and instructions.
In addition to these inspections, periodic management inspections and joint inspection by
the site safety committee will be organized and conducted.

The Site HSE Committee establishes a scheme to promote a housekeeping program by


which all subcontractors will suspend work for the duration of time once a week and jointly
conduct housekeeping and site cleaning. During this exercise, all workers including
subcontractors and Contractor’s supervisors will jointly conduct housekeeping around their
work areas, including warehouses, fabrication shops and offices.

11.3. Occupational Health and Environmental Control

In order to provide adequate sanitary conditions for all personnel at site, Contractor
considers the following provisions as a part of the temporary facilities. If they shall be
provided by subcontractors, Contractor will indicate guidelines for planning, installation

11.3.2. Workers’ Rest Space


Contractor considers the following provisions as a part of temporary facilities. If they are
provided by subcontractors, Contractor will indicate guidelines for planning, installation
and maintenance.
 Sunshades
 Smoking areas

11.3.3. Noise Exposure


Contractor and subcontractors will take the necessary measures to protect workmen
against the harmful effects of long term exposure to excessive (loud) noise. Noise
exposure will be controlled by reducing sound levels or wearing the appropriate hearing
protection.

11.3.4. Illumination

Adequate lighting is essential to ensure safety in work areas, quality work and
productivity. Contractor’s site management and supervisors will make sure that all work
areas and places are properly illuminated.

During construction activities, airborne contaminants may be created which have harmful
effects on the respiratory and circulatory system. Workmen must be protected against
these harmful effects through effective controls, i.e. elimination, isolation or the use of
personal protective equipment (PPE).

Contractor’s site management will take the necessary action as practicable and possible to
control and minimize the harmful effects of airborne contaminants such as dust, fumes,

mists, vapors and gases.

11.3.6. Environmental Protection


There are generally two categories of pollution and environmental impact that a
construction enterprise needs to be concerned with:
 Those affecting the environment in the vicinity of the site
 Those affecting the working conditions within site boundaries and where control is
necessary, considering occupational safety and health

The areas of pollution can be classified into the following four categories:
 Solid Waste
 Noise, Vibration and Odor Nuisance
 Water Pollution
 Air Pollution

The following provides to prevent or minimize environmental impact. Contractor’s site


management and subcontractors shall take appropriate measures in consideration of site
conditions and surroundings.

(1) Solid Waste


During the construction, the following solid waste materials will be produced:
 Steel scrap, such as pieces of steel pipe, structure materials, etc.
 Concrete and asphalt materials
 Excess excavated soil
 Combustible materials, such as timber and veneers for forming work

 Solid toxic chemicals

These materials must be disposed of properly in such a manner as not to contaminate or


alter the construction site and surrounding environment. It is prohibited to dump any solid
waste in public places or non-designated areas.

It is most important that solid waste materials be segregated according to the nature of the

materials and the method of disposal. Mixing all solid materials will cause difficulties in
disposal and hamper the smooth operation.

Recyclable steel material for disposal shall be handed over to enterprises which can
change these materials into usable form if such enterprises are available in the country of
the construction site.

Material which do not cause any contamination to the surrounding soil through the
process of leaching shall be buried in an excavated pit. Such pits will be approved and
designated by Employer or local authorities.

If materials are combustible and do not produce an adverse influence on air pollution,
they shall be burnt in a furnace. If the materials are combustible but produce a harmful
emission of gases when burnt, it is prohibited to do so.

The disposal of solid chemicals shall be determined carefully by examining the nature and
characteristics of the chemicals.

Solid toxic chemicals shall never be disposed of in any unauthorized place. The method of
disposing of these materials shall be studied and determined at the beginning of the
project, if the production of such chemicals is expected.

(2) Noise, Vibration and Odor Nuisance

The best way to reduce these nuisances is to eliminate sources from the working site.

However, construction work often produces these nuisances to a certain extent.

If the degree of nuisance may exceed an acceptable level, the following measures shall be
considered in the work planning:

producing work shall wear appropriate noise protection equipment such as ear plugs and
ear mufflers. This will cover people working in the proximity as well. Duration of such
noise producing work shall also be considered. For example, short working hours are
preferable, so work shall be arranged not continuously but intermittently.
For odor nuisance, the work shall be limited to confined or enclosed buildings. The
worker shall wear an appropriate filter type face mask. Odoriferous materials shall be
stored in an enclosed container when not in use. The duration of exposure to the
atmosphere shall be reduced as much as possible.
(3) Water Pollution

The following are sources of water pollution in construction:


 Sewage water from office and camp area
 Spillage of oil and grease, and contaminated material from equipment maintenance
shop area
 Test and flushing water

Due to the concentration of a large workforce in a limited place, the site office, temporary
site facilities and camp are sources of sewage that must be properly managed.

(4) Air Pollution

In process construction, the biggest source of air pollution is the exhaust gas from
construction equipment.

Reduction of polluted air emissions from construction equipment can be attained by good
maintenance of internal combustion engines and by avoiding use of equipment that has
old, deteriorated engines.

Contractor’s construction management and supervision will check type and year of
manufacture of subcontractor's construction equipment during the construction planning.

If these are very old or seem to have deteriorated engines, they will instruct the
subcontractors to take appropriate measures such as overhauling engines and improving
the frequency of maintenance service.

11.4. Warning Signs, Barricades, and Signals

Contractor’s site supervisors will provide or instruct the subcontractors in its installation,
according to each worksite working conditions in various areas.

Site management and supervision will ensure that appropriate and adequate quantities of
signs, barricades and signaling supplies are available and are used, erected and maintained
as required to ensure the safety and health of site personnel.

12. ACCIDENT AND EMERGENCY PREPAREDNESS

12.1. Emergency Procedures

Contractor will ensure that all management staff, supervisors and workmen on the site fully
understand the procedures, and the actions to be taken during emergency situations.

The following will be considered to establish emergency procedures:

 Coordinate with the Employer’s representative in establishing emergency


procedures.
 Make a list of available emergency services and confirm their communication
channels.
 Ensure that subcontractors clearly understand their responsibility and role in the site

emergency procedures.
 Designate first aid/medical supplies, and other material and equipment that may be
required for emergency use.
 Set up emergency response teams.
 Determine emergency evacuation routes for work areas and the construction site.
 Establish a personnel staging area or assembly points for evacuees.
 Make sure that all personnel clearly understand emergency warning siren signals or
other warning signals used by the Employer, Contractor and/or other authorities.
 Provide summarized communication diagrams and necessary maps, and display them
at proper places in the site.
 Designate a supervisor to conduct a head count and give instructions to evacuees as
necessary.
 Post emergency information for each work area on the site.
 Conduct emergency procedure drill periodically.

12.2. First Aid and Medical Provision

Contractor and subcontractors will provide or make arrangements to provide adequate first
aid facilities, equipment and qualified attendants for work site.

12.2.1. Considerations on First Aid

The following are key considerations in the preparation of first aid facilities at site:

 Experienced and knowledgeable personnel who can lead the operation and give

necessary training to others shall be assigned. A list of qualified personnel such as


first aiders or nurses shall be provided and published through all organizations at site.
 Adequate and properly stocked first aid kits, complying with local labor laws, shall
be kept at locations in accordance with the same laws.
 An ambulance or other suitable vehicle will be provided to transport the injured and
sick to the nearest medical facility.

12.2.2. First Aid Training

Contractor intends to provide training courses so as to increase the number of


knowledgeable personnel and enhance the total site performance in emergency/first aid
cases.

In order to realize this intention, Contractor and subcontractors supervisors will be trained
in basic first aid and CPR (Cardio Pulmonary Resuscitation).
12.2.3. First Aid Treatment

First aid facilities shall be readily available to all personnel and at all times at site.

Records of all types and causation of minor injuries and sickness will be maintained and
analyzed by Contractor.

13. SITE INSPECTIONS, AUDITS AND PERFORMANCE MONITORING

13.1. HSE Inspections

(1) Contractor and Employer teams for periodic inspections will be formed consisting of
the following members:
 Contractor’s Site Manager
 Contractor’s Site HSE Manager
 Employer’s Representative
 Employer’s HSE Manager

(2) A standard HSE inspection checklist will be used to conduct each inspection.

(3) The Site HSE Manager will prepare a written report on the HSE problems/deficiencies
identified during inspection of each facility and work area.

(4) When potential deficiencies are found, immediate corrective actions will be taken for

incident/accident prevention.

(5) Reports on the HSE deficiencies will be kept in site office for statistical purposes and
analysis.

(6) Contractor will instruct each subcontractor to submit a weekly report of site inspection
using a standard form.
(7) Crane operator is required to check his crane every day using the prescribed form.

13.2. Accident and Near Miss Investigations

13.2.1. Investigations

All accidents, incidents and near miss occurrences will be investigated. The cause or the
chain of events that led to the accidents/incidents and near-miss must be determined.

When the cause is known, appropriate actions will be taken to prevent recurrence.

Accident investigations will be conducted without prejudice. The investigations will focus
on finding the facts, not fault or placing blame.

13.2.2. Reporting and Records Keeping

In the case of a fatality or an accident causing disability, a report (normally by telephone)


shall be issued immediately to the appropriate local authorities or police.

Any accidents will be reported to the Employer and the Managing Director in head office
by facsimile, telephone or e-mail.
Using information from the reports and records, management can determine weak areas in
the HSE plan. These weak areas can then be targeted for corrective action. Analysis of the
data will show the type of corrective action required. This may be training and education,
motivational and awareness schemes, new equipment and tools or new working methods.

13.3. Hazard Analysis, Unsafe Acts and Corrective Actions

 Site management and supervisors will perform hazard analysis as required to identify
and eliminate the dangers of high risk and activities.
 Monitor the site, work areas and jobs being performed on a routine and timely basis.
 Site management and supervisors will take immediate action to ensure hazards are
made safe and unsafe acts corrected.
 Supervisors will give clear instructions for the work to be performed. Workmen will
be made aware of the hazards of their work.
 Adequate and appropriate personal protective equipment will be provided for the
work to be performed.
 As required, workmen will be given training to ensure they have the knowledge to
perform their job safely.
 Communications will be maintained between supervisors and workmen to ensure
effective hazard control and the elimination of unsafe acts.
 Supervisors will respect and take into consideration the suggestions and
recommendations made by workmen concerning the work performed. Immediate
action will be taken to eliminate reported hazards and unsafe acts.

13.4. Performance Measurement


Among a number of diversified measurements available in different countries and different
industries, Contractor uses the following two measurement indices for assessing safety
performance:

14. SUBSTANCE ABUSE POLICY

The consumption of any intoxicating substances during working hours or within such time
previous that a person would still be considered intoxicated is prohibited.

Any such substance such as alcoholic beverages, illegal drugs, inhalants and prescription
drugs are not to be consumed during working hours or within such time as the person would
still be mentally or physically impaired.

The use of prescription drugs during working hours is prohibited unless verification from the
prescription physician states that the medication would not impair the person’s ability to
perform their specific duties. This verification must be in writing from the physician and on
the office letterhead.

Contractor will implement education of the workforce at site safety orientation courses and
safety awareness campaigns of the dangers and consequences of substance abuse during
employment on the site to ensure compliance with the substance abuse policy.

As standard policy on all security access points and worksite locations, Contractor’s security
staffs are given the power to stop and search person on site, drivers of motor vehicles are
observed carefully.
