Safe Lifting Operation Plan

OPERATION Director

PURPOSE AND SCOPE

To define a procedure for safe use of cranes & derricks to prevent incident so as to protect workmen
and neighbour from accident.

This procedure is applicable to all project sites.

Responsibility

The PM/PIC will be overall responsible for proper installation and maintenance of all lifting
equipment at site.

Lifting Supervisor is responsible for the suspension or lifting operation control of all lifting
equipment used at site.

The WSH COORDINATOR is responsible for regular monitoring of the legal requirements.

Rigger will check lifting gear, ensure the load is secured, balance and stable at all time, tie the tag
line, apply padding, check safety latch and etc

Signal man to conduct (if have walkie talkie, conduct radio check, ensure same frequency, maintain
constant communication with Operator), communication using hand signal and must be must be
clear and Operator and signal man must be able to see each other clearly, assist in barricading the
lifting zone and etc.

The crane operator shall conduct a routine check to ensure all the accessible points of the crane are
in good condition and the crane are functioning properly. The crane operator is responsible for
maintenance of the daily checklist.

Procedure

Mobile crane

The PM/SITE Supervisor shall order the mobile crane from the approved supplier list.

WSH COORDINATOR/ LS shall check the valid license number/ MOM registration number of the
company.

WSH COORDINATOR/ LS shall check the operator’s training certificate and valid registration number
with MOM.

The Engineer shall ensure that the construction of the crane access is according to Professional
Engineer (PE) design and drawing, if required

PE shall inspect and certify that the access for the crane is safe and adequate for use, where
necessary.

WSH COORDINATOR/LS shall inspect all the safety devices i.e. load bearing chart/ efficient brakes or
other locking device and shall ensure that outrigger is fully extended before starting hoisting
operation.

LS shall ensure only registered crane operators, appointed riggers and appointed signalman carry the
lifting works involving the use of mobile crane; the ground conditions are safe for any lifting
operation and brief all the crane operator, riggers and signalman on the safe lifting procedures. LS
shall brief all crane operators, riggers and signalman on the safe lifting procedures.

Signal man to ensure the ground is in good condition and surrounding lifting zone is to be fully
barricaded.

RECORD KEEPING

WSH COORDINATOR/LS/SUPERVISOR shall keep all the records as per Client’s/MOM requirements
such as:

Daily inspection checklist by the crane operator

Checklist for testing and inspection by Approved Person and Certificate of Approved Person at least
once a year.

Monthly checklist for maintenance by a Mechanic

Approval letter for use of Mobile care

Checklist of crane access by Engineer

LS checklist

PTW for lifting operation

The PM/PIC shall establish if the client requires more frequent inspection at least once a year

Non-Conformance

In case of any non-conformance such as expired certificates, damage/ non-availability of proper


safety devices etc. the WSH COORDATOR shall stop the operation immediately and inform the
PM/PIC.

The PM/PIC shall take actions against the responsible parties and shall ensure that the rectification
work has been done accordingly.

LS is responsible to take measures to rectify the unsafe condition reported by crane operator or
rigger.
