questions is to ensure that any health needs or disability adjustments are met to enable you to
perform the tasks of your new role to your full potential.

1. If you have a health condition (long term or temporary) which may affect your ability to carry
out the tasks of the role, that you would like to discuss with Occupational Health, please send
an email to oax89488@gsk.com putting ‘New starter consultation requested’ in the subject
line. Please provide only the following information in the body of the email: Please do not
add any personal, health related information.

• Name
• DOB
• Contact phone number
• GSK location & job role
• GSK Manager’s name
• Start date

Any information provided to Occupational Health will be treated as confidential and used solely
for the purposes of assessing your capability to undertake the tasks of the specified role, with
or without appropriate and reasonable adjustments. The implications will only be discussed
with Line Management and, where appropriate, with Human Resources (or recruiting agency
for agency workers).

2. If you are aware of any workplace adjustments or accommodations, you need to carry out the
tasks of the role e.g. non-standard IT peripherals, assistive software & training, non-standard
furniture, or adjustments to the physical working environment, please send an email to
was.workplace-adjustment-service@gsk.com putting ‘New starter
adjustments/accommodations required’ in the subject line. Please provide only the following
information in the body of the email: Please do not add any personal, health related
information.

Workplace Adjustment Service

During your employment at GSK you may encounter circumstances leading to you
needing (temporary or longer term) adjustments to support effective working.
The Workplace Adjustments Service supports employees with injury, health condition, disability
or pregnancy, to enquire, request and secure specific physical adjustments items/services (e.g.
non-standard IT peripheral or furniture, assistive software or training) they need to be their best
selves at work.
The service is open to all UK and US employees - helping our employees 'do more, feel better,
