I then undertook an Interventional Fellowship at the Toyohashi Heart Center in
Toyohashi, Japan, from August 2008 to July 2009, under the supervision of Dr
Takahiko Suzuki, Dr Osamu Katoh and Dr Etsuo Tuschikane. This gave me the
opportunity to acquire a comprehensive experience in interventional techniques to
treat coronary chronic total occlusion (antegrade and retrograde), Primary PCI,
rotational atherectomy, left main stem PCI, IVUS, Optical Coherence Tomography
and peripheral intervention. This time also allowed me to understand Japanese
approach, techniques and perform research in above mentioned areas. This post was
approved by JCHMT/SAC for training in cardiology.
While in Japan, I continued and refined my interventional training and also
participated in several “live case demonstrations”. I returned to LHCH as the senior
interventional trainee to consolidate my interventional experience. I was appointed as
Locum Interventional Cardiologist at St George’s Hospital, London (May 2010 to Feb
2011) and later worked as Consultant Cardiologist at Fortis Escorts Hospital in India
(Feb 2010 to Jan 2011). Currently, I am working as Consultant Interventional
Cardiologist at Frimley Park Hospital and St Georges Hospital.

Consultant Experience
I have gained experience working as Consultant Cardiologist in current post, Locum
Consultant Interventional Cardiologist at St George’s Hospital, London and Fortis
Escorts Heart Centre, India for last 5 years. During this period I have undertaken
following projects:
1. Established a Radial lounge for coronary angioplasty as day case at St George’s
hospital and Frimley Park Hospital.
2. Established CTO angioplasty service, workshop and referral from South West
London Region.

3. Audit of primary PCI data and presented to Pan-London Group.
4. Established Complex Coronary Angioplasty service at Frimley Park Hospital, UK.
5. Established CT Coronary angiogram service at Frimley Park Hospital and
completed 2 years and around 600 cases.
During my training I have found that, in order to provide the best quality clinical care,
the processes and protocols which underpin the delivery of care are as important as
the delivery itself.
I have completed the mandatory management residential training course. This
improved my management skills and multi disciplinary team interaction in day to day
clinical care.
Throughout my post-registration hospital career I have tried to be involved in the
management and organisation of medical services and training. In North Wales (1997-
99), I was the SHO level RCP representative and via this role and through directorate
meetings, played a major part in developing training and service compliant shift-rotas
for medicine and cardiology. As an SHO, I coordinated the SHO daily and on-call
rota and managed the annual leave
During my specialist registrar training I have been regularly involved in developing
local protocols, auditing performance and reviewing clinical incidents. I was
responsible for the listing of elective and emergency patients attending for procedure.
This involved close liaison with senior medical staff, nursing colleagues and bed co-
ordinators. In collaboration with the bed manager, I co-ordinated in-patient hospital
transfers and then listing them for the catheter laboratory procedures.
This gave me a unique perspective to aspects of hospital management and practices
crucial to improving patient care that will influence my practice as a consultant.
I have updated the protocol and guidelines for the use of haemostatic compression
devices (TR Band and Radistop) following transradial coronary procedures within the
LHCH and had them ratified.
I have been instrumental in starting up radial lounge facility for coronary angioplasty
and complex angioplasty workshop (including Chronic Total Occlusions) at St
Georges Hospital in London and Frimley Park Hospital.

Research
MD Thesis
I have completed a research project “Comparison of Introducer Sheaths and
Compression Devices in Transradial Coronary Procedures and the impact on vascular
function- A Randomised Clinical Trial” under the supervision of Dr J.L Morris and
Prof George Hart at University of Liverpool, UK. I have spent 12 months (Aug 06-
Aug 07) in recruiting and collecting data for this trial. I have submitted my Thesis to
Liverpool University and viva was conducted on 10th May 2011 and passed the
examination with minor revision. I have been awarded MD (Research) in 2012 from
Liverpool University, Liverpool, UK. My work has described clinical outcomes after
transradial coronary procedures in contemporary practice and also provided insight
into its impact on endothelial function. I have published several research papers and
presented my work at national and international meetings.
Clinical Research
Co- Investigator of CARE-HF study at Blackpool Victoria Hospital, Blackpool, and
Involved from screening, consenting, randomisation, data collection, pacemaker
implantation, atrioventricular optimisation, and follow up etc. I have recruited 10
patients for this trial.
Played a key role in the development of “NWQIP Risk stratification model” for
predicting outcomes after coronary angioplasty. This was performed as North-west
Quality Improvement Program initiative across tertiary care centres in Northwest of
UK. This model is used widely in contemporary practice.

Investigated Retrograde approaches in Chronic Total Occlusion of Coronary arteries
and plaque morphology by IVUS and Optical Coherence Tomography at Toyohashi
Heart Centre, Japan (Aug 208-Aug 09). I have presented the data in international
meetings and published several articles on CTO techniques including IVUS guided
and retrograde techniques. I have paper looking at distribution of TCFA and plaque
rupture in coronary artery disease patients. I have recently published paper looking at
coronary artery remodelling and plaque characteristics as assessed by OCT and grey
scale IVUS.
I have been involved in several studies evaluating the outcomes after coronary
angioplasty in different patient sub-sets and population.
Research Career
I enjoyed an extremely rewarding and productive one year as a research registrar in
Liverpool (2006-07). Under the supervision of Dr John Morris, I completed
recruitment of patients for my MD. I have gained considerable experience in writing
research grants and ethics committee proposals, as well as becoming proficient in
organising a large clinical trial, randomisation, day-to-day running and statistical
techniques. During my research period I worked in close collaboration with scientists
at John Moore’s University and this gave me good understanding of the physiological
basis of the clinical problem in question. I have been instrumental in formulating
close collaboration between LHCH and the department of sports medicine at John
Moore’s University and am involved in further studies looking at the ways to optimise
endothelial function following transradial procedures. I feel more adept at assessing
other research projects and far more critical in my analysis of published data –
essential skills in the current climate of evidence based medicine.
I have been able to renew my research interests whilst working as an interventional
fellow in Japan. In collaboration with Dr Suzuki and colleagues, I have been involved
in several bench studies and clinical studies and have published several manuscripts in
peer-reviewed journals. My stay in Japan gave me the opportunity to experience a
different perspective in the management of coronary artery disease.
During the last year I have been writing up my MD and also written up several papers
related to my study which have been accepted in peer-reviewed journals.

I very much hope that I will have the opportunity to continue and develop my
research interests throughout the rest of my clinical career.
Teaching
I have been involved in weekly ward-based and lecture-based teaching of medical
students and the SHO’s (2003, 2004-2005)
As a registrar at Leighton Hospital, Crewe, I was the Associate Tutor of RCP, London
and was responsible for teaching program for postgraduate trainee.
I am involved in teaching medical students and nursing staff and have organised
formal PACES teaching for MRCP candidates.
I have performed educational supervisor role for the senior house officers and
registrars during my post as consultant cardiologist at SGH, London and India.
I have attended Teach the teacher course over two day period and this has further
improved my teaching skills.

Career aspirations
I have been very fortunate over the last 15 years to have worked in several centres of
excellence and received an abundance of high quality training in medicine and
cardiology. I have received first-class training in general cardiology and PCI from
John Morris, Rod Stables, Joe Mills, Ralph Perry and David Ramsdale, and have
continued to develop my skills during a period of out-of-programme-activity in Japan.
Under Dr Suzuki & Dr Katoh’s supervision at Toyohashi Heart Centre in Toyohashi, I
have become experienced in chronic total occlusion PCI, rotational atherectomy,
acute PCI in STEMI/shocked/cardiac arrest patients, and had considerable exposure to
the use of IVUS, Cardiac multi-slice CT and Optical Coherence Tomography.
Overall, I believe that my inter-personal and communication skills along with
my teaching and research interests and high-quality PCI training make me ideally
suited to the Teaching hospital Interventional Cardiology Consultant post. I would
relish the opportunity of being part of a dynamic, forward thinking department and
bring enthusiasm, dedication and hard work to all aspects of service provision and
development. I hope that my experiences in Liverpool (day-case radial PCI, twilight
ACS lists, 24/7 primary PCI service), Japan (chronic total occlusion PCI, 24/7
primary PCI service and intravascular imaging) and Frimley (Experience delivering
PPCI, Day case Radial PCI, establishing CTO and complex PCI service and CTCA
service) will allow me to be an integral part of service improvement and innovation.
Leisure activities
My time away from work is spent enjoying with my son Ananmay and daughter
Maanya (10, twins) and with my wife Vandana who is a post CCT O& G Senior
Clinical Fellow. I love travelling, listening music and enjoying worldwide cuisine.
