PAKYAW CONTRACT AGREEMENT

This agreement made and entered , by and between EDNA T. TUYOR, married,of legal
age and School Head of Patong Elementary School herein after referred to as the FIRST PARTY:

AND
JONATHAN N. MERZA of legal age, married,a PTA member with residence and postal address at
Mahayag,San Miguel,SDS herein referred to as the SECOND PARTY.

WITNESSETH:

THAT THE FIRST PARTY:

1. Will provide labor necessary for the proper and faithful performance of the MASONRY
WORKS of Patong Elementary School at the contract amount P 1,700.00 pesos.

2. Will responsible precaution to prevent disorderly conduct by any members. The School may
require the Pakyaw Contractor to remove from work and of the member who in the opinion of
the school misconduct himself and or incompetence

3. Will complete the work in accordance with the corresponding plans and specifications under the
supervision of the Physical Facilities Coordinator

THE SECOND PARTY:

1. Will provide the labor necessary to carry out the implementation of the job.
2. Will finish the project within calendar days upon approval if this contract.
3. Will collect full payment for labor per item number 3 above.
