Good day!

As the highest authority in our company, I refer to your office regarding my status. I have been a regular
employee for almost 19 years and am currently assigned in HR. Last 2018, I have been admitted due to
kidney failure and was later advised by my attending physician to undergo dialysis thrice every week. I
still manage to report for work from Monday to Saturday and leave the office earlier only during my
dialysis schedule. I am so grateful and glad that our company has its medical benefits that help cover my
hospital fees. However, I still have excess bills that were charged as my personal cash advances in the
company.

With this, I am asking for your help to alleviate my current problem. It may seem bold, but I humbly ask
for a salary adjustment for a Level 4 Rank and File employee since I am currently in Level 3. This is to
patch up my cash advances through salary deduction and with extra something for my family.
