Ang feedbacking ng post qualification na resulta ay pinadaloy ni BAC Head Risy Liza
Taladtad at sa gabay ni Engr. Cyril Joseph Gado na isang TWG mula sa programa ng
KC-NCDDP-Additional Financing.

Ito ang naging daloy ng gawain;

1.Pagpresinta ng Eligibility Requirement ng Lowest Calculated na Kotasyon na supplier.
 Iprinisenta ni BAC Head Risy Liza Taladtad ang mga business permits ng
RMD Marketing na ibinigay noong ginanap ang post-qualification activity.

2. Pagpresinta ng isinigawang pag-bisita sa lugar ng Negosyo ng supplier at mga

materyalis na kailangan.
 Sunod ay ikinuwento din ni PT Head Victoria Gabat ang nangyari noong
ginanap ang post-qualification activity.ayon sa kanya, una sa lahat ay
humingi muna sila ng gabay mula sa KALAHI staff na sina Engr.Cyril Joseph
Gado-TF at Esmeraldo Gusi Jr. MCT-CEF. Pagkatapos ay nagtungo na sila
 sa RMD Marketing Warehouse kung saan ay nandoon ang mga materiales at
ang mga iba pang stocks. Una nilang sinuri ang Washed Sand at kumuha ng
sample, sunod ay ang Washed Gravel, G1 at ang huli ay ang Portland
cement. Tiningnan din nila ang mga iba pang stocks ng materyales na
mayroon ang nasabing Warehouse.

3. Kasunduan para sa pag-deliver ng materyalis na naayon sa specifications na nasa

Purchased order.
 Sa Kabuuhan ay maayos naman na ang materyales na nasuri nila ayon kay
PT Head Victoria Gabat maliban sa washed sand.ayon kay BDC-TWG
Chairperson Edelmer Garachico,sa pagserve ng Purchase order ay dapat
masiguro na walang nakahalong malalaking gravel sa washed sand,at ito ay
dapat masiguro din sa pamamagitan ng bago tanggapin ang delivery ng
materyales ay kailangan na ito ay masuring mabuting tatangap na Bolunter
na ito ang tama at dapat idiliver materyales.

4. Pag-apruba ng mga sumusunod na kasunduan;

 Tamang pag-deliver ng materyalis- na ayon sa Terms and Conditions na nasa
PO. Ang mga sumusunod ay ang Terms and Conditions na nasa Purchase
Order:
 All prices quoted herein are valid, binding, and effective at least within
forty-five (45) calendar days (the number of days may vary depending on
the period of delivery) from the date of quotation.

 AWARDEE shall be responsible for the source (s) of his

supplies/materials/equipment and shall make deliveries in accordance
with schedule, quality, and specifications required in the Purchase Order.
Failure by the AWARDEE to comply with the same shall be ground for
cancellation of the award and purchase order and for re-awarding the PO
to the ALTERNATE AWARDEE.

 Subject to the provisions of the preceding paragraph, where AWARDEE

has accepted a Purchase Order but fails to deliver the required product(s)
within the time called for in the same Order, shall be extended a
maximum of fifteen (15) calendar days under liquidated damages to make
good his/her delivery. Thereafter, if AWARDEE has not completed
delivery within the extended period, the subject Purchase Order shall be
cancelled. The DEFAULTING AWARDEE is subjected to disqualification
in any KC-NCDDP AF procurement, without prejudice to the imposition of
other sanctions.

 All deliveries by the supplier/contractor shall be subject to inspection and

acceptance by the BDC-TWG through Glory Bell G. Amar-MIT Head. All
necessary laboratory tests undertaken by the BDC-TWG through Perline
S. Fernando-PIT Head on the item(s) shall be for the account of the
supplier/contractor.
  Rejected deliveries shall be construed as non-delivery of
product(s)/item(s) ordered and shall be subject to liquidated damages,
subject to the terms and conditions prescribed under paragraph 4 hereof.

 Supplier shall guarantee his deliveries to be free from defects. Any

defective item(s)/product(s), therefore, that may be discovered by the
BDC-TWG through Edelmer M. Garachico-BDC Chair within three (3)
months after acceptance of the same, shall be replaced by the supplier
within five (5) calendar days upon receipt of a written notice to that effect.

 In case of delay in delivery, Liquidated Damages (LD) will be deducted to

the final payment. The basis of computation of LD is one percent (%) of
the cost of all the goods undelivered, after the deadline until completion of
the deliverables. Number of days shall be based from the agreed
completion date (or deadline of delivery) until the actual completion date
(or full delivery) regardless of progress (or delivery made) in between the
agreed completion vs actual completion date (weekends, and holidays
included); otherwise, 1/10 of 1% for contracts higher than 10M.

 In no case shall the sum of liquidated damages exceed ten percent (10%)
of the contract amount. If it does, the contract is deemed automatically
terminated by the C/BSPMC, without prejudice to other courses of action
and remedies available to it.

 All duties, excise and other taxes and revenue charges, if any, shall be for
the supplier’s/contractor’s account.

 Awarding of contract to LCRQ and passed the post qualifications conducted.

 Iminungkahi ni BAC Head Risy Liza Taladtad kay BDC-TWG Chairperson
Edelmer Garachico na ang RMD Marketing ay pumasa batay sa Lowest
Calculated and Responsive Quotation (LCRQ) at sa ginawang post-
qualification activity. Tinanggap naman ito ni BDC-TWG Chairperson
Edelmer Garachico.
