1. That my maiden name is Sanjeev Kumar.

2. That at present my name as per all records, including birth certificates,
educational certificates and identity documents is Sanjeev Kumar.

correct name is Sanjiv Kumar.
4. Henceforth, I shall be known by the name of Sanjiv Kumar for all purposes,
