Parental Consent

participation of my son/daughter in the sport __________________ for the conduct School Sports Club
activities for the following dates of April 13, 20, 27, and May 4, 11, 18, and 25, 2024 at 8:00AM -
3:00PM to be held in the Covered Court of Doña Rosario High School.

I have considered the benefits that my son/daughter will derive from his/her physical participation
in the School Sports Club activities provided that due care and precaution will be observed to ensure the
comfort and safety of my son/daughter and that DepEd employees and personnel may not be held
responsible for any untoward incident that may happen beyond their control.
