PARENT CONSENT AND WAIVER FORM

As a parent/guardian of the student-participant, I hereby give my consent, and

acknowledge by my signature that my son/daughter __________________ of Year Level and
Section____________ will join the Sports for Educ Fest 2024 to be held at Bago City College on April 1-5,
2024.

I am aware of the usual risk and dangers inherent in the said contest thus, accidents can
occur with or without any fault on either the part of the student, or the teachers where the contest is
taking place. By allowing my child to participate in this activity, I accept the risk or any untoward
incident, and conform to the school rules and regulations that this activity is suitable for my
son/daughter in order to enhance his/her skills as a student.
