PARENTAL WAIVER AND CONSENT FORM

I, the parent or legal guardian of the child named below, hereby grant my full consent for my
child to participate in the Youth Friendship Camp at Spur 1, Titulok, Bagumbayan, Sultan
Kudarat, from November 2-4, 2023.
I fully recognize that, just like in any other adventure, there may be some inherent risks in the
camp activities and travel. However, I wholeheartedly accept these potential risks on behalf of
my child. I am confident that the camp's rules and regulations are in place to ensure safety and
enjoyment. This camp is an ideal opportunity for my son/daughter to establish new friendships
and rekindle their spiritual connection.
I also hereby confirm that my child is not only physically capable but also mentally prepared to
fully participate in these activities. They are in good health, free from any physical or mental
restrictions. Furthermore, I understand and accept that the Camp Administrators cannot be
held responsible for any unforeseen incidents that may occur throughout the camp.
