JUDICIAL AFFIDAVIT OF KORINA CANCHEZ

I, KORINA CANCHEZ, of legal age, married to MAR CANCHEZ, and living at No. FD
247 Pines Park, KM4, Balili, La Trinidad, Benguet, Philippines, witness of the plaintiff in this
instant case, state under oath as follows:

PRELIMINARY STATEMENT
The person examining me is Atty. Elza Jayne Calanno with address at 2nd floor, Felicidad
Building, Otek Street, Baguio City. The examination is being held at the same address. I am
answering her questions fully conscious that I do so under oath and may face criminal liability
for false testimony and perjury.

This affidavit/testimony of Korina Canchez is being offered to prove that I have seen the
vehicle parked in the house of Juan Dela Cruz;

Q1. Do you swear to tell the truth and nothing but the truth?
A1. Yes Ma’am.

Q2. Are you aware that you may face criminal liability for false testimony or perjury if you
will not tell the truth?
A2. I am, Ma’am

Q4. Do you know the defendant in this instant case, Ms. Korina?
A4. Yes, Ma’am. I am the neighbor of Mr. Juan Dela Cruz.

Q5. How many years you and Mr. Dela Cruz are neighbors?
 A5. Almost 2 years Ma’am.

Q6. Can you narrate to me how did you see the vehicle?
A6. On January 12, 2024, my family and I came home from vacation and I was appalled
that Mr. Juan Dela Cruz

Q7. Did you ask the defendant about the vehicle?

A7. Yes Ma’am.

Q8. What did he answer?

A.8 That he bought the van to a friend of his.

Q.9 Can you state the physical feature and plate number of the vehicle if you can remember?
A.9 Yes Ma’am. It is a Super Grandia with a plate number GAA 2424.
