1. This refers to Bureau Order No. RET-2020-076 dated 27 October 2020

herein attached.

2. In this connection, the undersigned respectfully requests your good office

to be assisted with the facilitation of the necessary documentary requirements for
opening of an ATM pension account to Land Bank of the Philippines- Pagadian Main
Branch/ Ms. Gina P Baliton, Branch Manager within the region, who retired from the
service on 30 May 2021 and availed thru a Lump sum for three (3) consecutive years.
