1. Reference: As per verbal instruction from SFO3 Shellborn A Lagar, Chief

Operation of this station regarding the above-mentioned subject.

2. In this regard, this station respectfully requests for issuance of Two (2) Units
Batteries 21 Plates 12 Volts for the replacement of the old battery of Isuzu Presha Fire
Truck with bearing plate number SJE 955.

3. We humbly request for consideration and approval.

4. Request acknowledge receipt.
