Sub: Request to Surrender My Old PAN Card

Respected Sir/Madam,

I, Dharmendra Sinh Rajput, am writing this letter to request you to cancel my old PAN Card with
number CLZPS6033B, which was issued few years ago which was lost. I had requested for the new
PAN Card and I have received my new pan card with the Pan number FUHPR8620H
Old PAN Card number- CLZPS6033B
New PAN Card number- FUHPR8620H
Since I have two PAN Cards now, I would like to request you to cancel my old PAN Card as I have
deregistered this card from all services. I shall remain grateful for this kind gesture of yours.
