MAHESHBHAI PATEL.
3. That by mistake my name has been mentioned as ASHISHKUMAR
MAHESHBHAI PATEL instead of ASHISH MAHESHKUMAR PATEL
in the said documents.
4. This is also to categorically state that my correct name is ASHISH
MAHESHKUMAR PATEL.
5. That I therefore request the concerned authorities to may please update and
rectify my name as ASHISH MAHESHKUMAR PATEL.
6. That I am enclosing a self-attested copy of AADHAR CARD AND PAN CARD
as a proof of my correct name.
