Are controls in place to prevent personnel entering the drop zone? Yes No N/A

Where there is a risk of dropped objects, an exclusion zone been established? E.g. for
every 4 metres in height barricade 1 metre out from the vertical to allow for deflection?

Exclusion zone shall be danger taped’ with information tags attached at each level where
there is a dropped object risk?

Are controls in place to prevent falling objects? Yes No N/A

Scaffolds/Platforms/Structure which captures material build-up has been washed down


before work commences?

Are rated lanyards used and attached to tools or equipment that could fall from height?

Rubber stoppers or safety collars are fitted to Crow / Podgy / Pry Bars?

Drop mats are in place at the work area to cover any gaps where objects may fall from one
level to another?

Loose items have been secured in containers, tool bags or tool boxes and not left near open
edges?

Equipment which can move during high wind is secure?

Scaffolds/Platforms/Structure is clear of unnecessary tooling and debris? Kick rails in place


and free of gaps and at appropriate height?

Handrails have infill protection fitted? I.e. Shade cloth, plastic mesh?

If Equipment/Structure has been removed creating open penetrations, hard barricading is in


place?

Work shall STOP by everyone if there is a requirement for entry into the drop zone. Work
will not continue until the area is clear?
