the remedial reading class in reading or basic literacy skills and arithmetic for
SY 2023- 2024 (Project ABYAN : Kaakbay Pabasa, Yuhum sa Anak ,
Nagahatag Kasadya ) every
a f t e r n o o n from 1:00pm-3:00pm (Grade 6) Starting
April 17 , 20024 until _________________, conducted by Special Program in
Science , Technology and Engineering students as part of the requirements in
their Araling Panlipunan subject and in collaboration of our school,
Assemblyman Segundo Mososo Memorail School under the supervision of
his/her subject teacher, Mr./Ms.

I have considered the benefits that my son/daughter will get from this intervention
program, and I fully entrust the welfare of my child provided that due care and
precaution will be observed to ensure his/her safety. Rest assured that I won’t let
his/her teacher and other DepEd personnel be held liable for any untoward
incident that may happen beyond their control during the span of the intervention.
