CHAPTER I: Introduction

This chapter focuses on the discussion that identifies the need for the study and

relevance to the current situation. Specifically, the problem, purpose, related literatures,

hypotheses, definitions, theoretical and conceptual framework, and significance of the

study.

Rationale

Communication between nursing students and their patients plays a vital role in the

hospital field, for it serves as the foundation for giving the care that the patient needs.

Student nurse-patient communication promotes patient-centered care interactions that

honor the values and preferences of the patient and is done in the way that the practice is.

However, lengthened lockdown interferes with the development of communication skills

of nursing students because of the use of e-learning platforms instead of face-to-face

classes.

Any form of communication like speaking, reading, listening and writing is very

important especially in education sectors that needs a good base and serves as the

foundation of nursing students to have a good communication skills in the hospital setting

and acquire fluency. According to Centeno, A. (2021), during the pandemic, measuring the

level of communication skills in the hospital setting of the nursing students is quite difficult

because e-learning platforms are used since there is no face-to-face interactions.

The first one that a nursing student works on is establishing a patient’s trust and

rapport. Thus, good communication creates a close and harmonious relationship with

patients. It enables nursing students to interact with their patients and comprehend their

11

College of Nursing

feeling. Moreover, having good rapport has been demonstrated to enhance patient

happiness, clinical results, and treatment compliance.

Although communication itself seems simple, often when ones try to establish

communication with others, there is always a chance for lack of understanding that might

cause conflicts and frustrations in personal or professional life in relation with other people.

As stated by Jouany, V. & Martic K. (2022), successful communication helps ones better

understand people and situations. It helps in overcoming diversities, build trust and respect,

and create conditions for sharing creative ideas and solving problems.

In times of pandemic, nursing students do everything online, especially in

communicating with their patients. They often practiced nursing-patient interaction with

their relatives and teddy bears. With that, their level of communication skills in the hospital

might be decreased. According to Sook, S. & Soon, K. (2021), daily life has changed due

to COVID-19. This has affected nursing education and caused a shift to online learning.

Nursing students used e-learning platforms on their skills demonstration and nursing-

patient interaction causing their skills and way of communication less effective.

This research aims to evaluate the impact of post-lengthened lockdown due to

covid-19 on the communication skills in the hospital settings of the nursing students.

Improving the communication skills of their nursing students will be organized in line with

what the Joint Commission on Accreditation of Healthcare Organizations (JCAHO) has

established since 2016: Improving communication as a priority for improving patient

safety.

Review of Related Literature

Nursing is the largest health profession, with nearly 4 million providers practicing

across acute, primary, and public health care settings. In response to the pandemic, nursing

schools halted on-site course delivery and redesigned programs to attenuate risks to

students and faculty and one of the key challenges faces by nursing schools is the disruption

to clinical learning opportunities for the nursing student which can have impact on their

communication skills.

Foreign Literature

Changes in Nursing Education during Pandemic

COVID-19 pandemic affects the education system worldwide. The students,

specifically nursing students, felt that extended lockdown interfered with their educational

activities such as return demonstration and nursing-patient interaction Makhado,

Musekwa, & Luvhengo (2022). Confidence in communication skills was less during

quarantine than before Haggard (2022). Unsatisfactory communication skills acquisition

was one of the COVID-19 pandemic's side effects, which could lead to poorer clinical

nursing education quality. The transition to virtual learning presented challenges for

nursing students, as they had to adapt to communicating effectively through digital

platforms. This shift may have affected their ability to pick up non-verbal cues, build

rapport, and convey empathy, as these aspects are often better conveyed in person Kaveh,

Charati, & Kamali (2022).

Reduced In-person Interaction, Challenges in communicating and Limited Access to

Clinical Settings

The teaching and learning methods of communication skills in the training of

medical and healthcare workers have been surprisingly influenced and constrained as a

result of spatial limitations ever since the COVID-19 pandemic struck in 2020. Some

nursing professors in United States acknowledges the shortcomings of nursing instruction

under COVID regulations which some of the students are not as able to get in to work with

patients because of poor communication skills Patorque & Jewell (2021). In the early

stages of the outbreak, for instance, pre-health care students had fewer opportunities to

interact with patients and were even asked to withdraw from the early clinical exposure

program, one of the most efficient methods for learning how to improve interpersonal and

communication skills. As a result, changing the techniques of teaching communication

skills in medical school has become crucial Liao, Murphy & Wu (2022).

Enhanced teamwork and collaboration

The pandemic has necessitated a multidisciplinary approach to patient care Clari,

Luciani & Conti (2021). Nursing students have had the opportunity to work closely with

other healthcare professionals, such as doctors, respiratory therapists, and pharmacists, in

managing COVID-19 cases. This collaboration has improved their teamwork and

communication skills, as they learn to effectively communicate with and understand the

perspectives of different healthcare team members that they can also apply to their patients

even if through online Sciannameo, Berchialla & Guilo (2021).

Local Literature

Shift to online learning

In the Philippines, both teachers and students faced challenges in adapting to online

classes and maintaining the minimal communication to support learning and development,

especially in nursing schools. The transition to the virtual environment brought about a

paradigm shift in nursing education because of the pandemic Oducado & Soriano (2021).

Migrating to blended learning within a short period was difficult, especially in the

Philippines. This shift to virtual platforms presented challenges for nursing students, as

they had to adapt to communicating and interacting with their peers and instructors

remotely. Online learning may have limited opportunities for face-to-face communication

practice and hindered the development of certain communication skills Estoque (2021).

This reduced exposure to real-life communication scenarios, such as bedside interactions,

which are essential for developing effective communication skills. It is possible that the

adjustment to the new mode of teaching and learning would cause those issues. This phase

may have unfavorable implications and may have a negative impact on nursing students,

especially on their communication skills, which are vital in their course Moralista &

Oducado (2020).

Development of patient education skills

During the pandemic, nursing students have played a crucial role in educating

patients and their families about COVID-19 prevention measures, self-care at home, and

the management of symptoms. This experience has enhanced their patient education skills,

including the ability to explain complex medical information in a clear and understandable

15

College of Nursing

manner. Lim & Rehencia (2022). Nursing students have learned to provide empathetic and

compassionate care, understanding the anxieties and fears faced by patients during this

time. They have honed their ability to offer emotional support, actively listen to patients'

concerns, and provide reassurance, thereby strengthening their overall communication

skills in building trust and rapport with patients Donato, Berdida & Elero (2023).

Synthesis

In other country, Covid-19 limits one’s daily living, either work or study. Focusing

on academics, students switch from face-to-face classes to online classes affecting their

ways of learning. In nursing education, it is essential to have in-person classes because of

laboratories and clinical exposures. However, due to covid-19, the opportunity to

experience legitimate skills improvement like the student nurse-patient interaction in the

hospital setting fades off because of restrictions. As a result, there are students whose

communication skills in hospital settings are affected because of a lack of interactions. On

the other hand, some students are not because of the collaboration a professor and students

do online.

Philippines faced challenges in dealing with the covid-19 due to the restrictions

especially when it comes to the education system; from face-to-face classes to online

learning. Nursing students adapted online laboratories and clinical activities. Thus, give

them difficulties because they are doing all online. Due to online learning, they have

limited interaction giving a negative impact in the way they communicate. However, there

are student nurses who are able to visualize online interaction into life giving their way of

communication a positive impact.

Theoretical Framework

This theoretical framework contains one theory from the Multidisciplinary Digital

Publishing Institute by Agea, J. & Costa, C. that serves as the basis for this research entitled

Impact of Post-Lengthened Lockdown Due to Covid-19 on the Communication Skills in

the Hospital Setting of Level III Nursing Students at Wesleyan University-Philippines.

Communication is one of the foundations in the nursing field, specifically in

interacting with patients, as proven in the research conducted by Agea, J. & Costa, C.

(2022), how the pandemic affects communication skills in clinical settings, stating that

communication has played a fundamental role in clinical settings, in managing and

communicating with the patient. This research aims to determine the impact of post-

lengthened lockdown due to covid-19 on the communication skills in the hospital settings

of level III nursing students at Wesleyan University-Philippines.

Agea, J. & Costa, C. (2022) stated that communication skills are key competency

for student nurses, as it is for establishing a relationship of support, safety, and quality of

care provided to the patient. The main objective of their research was to determine how the

pandemic affects communication skills in clinical settings by correlating to the

demographic profile of their respondents. With this as a basis, the researchers correlate the

respondent's demographic profile to the impact of lengthened post-lengthened lockdown

due to covid-19 on communication skills in the hospital settings.

As per result, the theory obtains that communication of nursing students was

slightly affected due to covid-19 which is based on their null hypothesis there is no

significant relationship between the independent and dependent variable. Herewith, the

18
WESLEYAN UNIVERSITY-PHILIPPINES

College of Nursing

researchers come up with the null hypothesis there is no significant relationship between

the profile of the respondents and the impact of lengthened lockdown on the

communication skills in the hospital setting.

This research that the researchers is working to has the same objectives with the

chosen theory; to identify the impact of pandemic to communication, especially in nursing.

Therefore, it is essential and has a huge help in preparing the student nurses to be able to

have a good communication with their patients in the future.

Statement of the Problem

This study aimed to determine the impact of post-lengthened lockdown due to

covid-19 on the communication skills in the hospital settings of level III nursing students

at Wesleyan University-Philippines. Specifically, it sought to answer the following:

1. What is the socio-demographic profile of the respondents in terms of:

1.1 age;

1.2 sex; and

1.3 civil status

2. How may the impact of post-lengthened lockdown due to Covid-19 on the

communication skills in the hospital setting of the respondents be described?

3. Is there a significant relationship between the respondents’ socio-demographic

profile and the impact of lengthened lockdown on the communication skills in the

hospital setting?

4. What recommendations can be made based on the findings of the study?

CHAPTER II

Research Methodology

In this chapter of the research, the technical components of the study and the

methodology will be highlighted. This includes the location, the individuals who will be

the respondents, and the procedures that will be used to conduct the study. Particularly, the

research design, research locale, respondents, sample processes, scope and delimitation,

instrument, data collecting, data processing tools, and ethical considerations.

Research Design

This study used the descriptive-correlational research design. Descriptive research,

according to Best K. (2016), uses quantitative methods to describe what is, describing,

recording, analyzing and interpreting conditions that exist. It involves some type of

comparison or contrast and attempts to discover relationships between existing non-

manipulated variables. This is an approach for testing objective theories by examining the

relationship among variables.

A descriptive-correlational was used to determine the impact of post-lengthened

lockdown due to COVID-19 on the communication skills in the hospital setting of third-

year nursing students.

Sampling Procedure

Total enumeration sampling is a type or purposive sampling technique that involves

examining the entire population that have a particular set of characteristics (Canonizado,

2021).

The researchers specifically used total enumeration sampling for the selection of

the respondents and that will be the whole population of third-year nursing students. The

sampling method was considered because the entire population is well-define, have an

impact and fraction of which may no measure what is required.

Scope and Limitation

This study used descriptive-correlational of research which focuses on the impact

of post-lengthened lockdown due to covid-19 on the communication skills in the hospital

settings of third-year nursing students. Therefore, the researchers gathered information and

data to figure out any changes in variables.

The target respondents were limited to 224 students which will be the entire

population of third year nursing students of Wesleyan University-Philippines Cabanatuan

City, Nueva Ecija during the academic year 2022-2023. The 224 students were chosen

because their courses involve effective communication skills in nurse-patient interaction

which is very important in nursing care. The socio-demographic profile of the respondents

that will be gathered will be limited only to the age, sex and civil status.

Research Instrument

A questionnaire with two parts was utilized as the primary data collection

instrument in this study. A questionnaire is a sequence of questions with different choices

designed to gather information from respondents. It was carried out by third-year nursing

students at Wesleyan University-Philippines during the school year 2022-2023.

Part I contains the profile of the respondents. This includes the respondents’ age,

sex and civil status. Part II uses rating scales to identify the impact of post-lengthened

lockdown due to covid-19 on the communication skills in the hospital settings of the

respondents.

Data Gathering Procedures

The researchers submitted a request for approval letter to the Dean of the College

of Nursing. The researchers intended to start data gathering via online survey as soon as

they receive confirmation. The researchers sought a list of third-year nursing students

enrolled in the current academic year 2022-2023, as well as their social media accounts, in

order to distribute the questionnaires via Facebook messenger and Gmail. The online

survey questionnaire was made available on Google Forms in order to reach the intended

respondents and collect the appropriate number of responses. The researchers sent

messages to third-year nursing students that explained the study's objectives and requesting

their permission to participate in an online survey using questionnaires.

Data Analysis

The data that were obtained from the questionnaires which was distributed by the

researchers was tabulated, analyzed and interpreted with the use of the following sampling

techniques;

The researchers consulted their adviser for assistance in improving the

questionnaire in light of their research.

To evaluate the validity of it, the researchers ran a pilot study. The test-retest period

was one week, and the test was administered to 15 third-year nursing students who were

not among the actual respondents. To analyze the information gathered from the pilot test,

software like SPSS (Statistical Package for Social Sciences).

Ethical Considerations

The study was carried out at Wesleyan University- Philippines' College of Nursing,

with the permission of the faculty. The data received from the participants were handled

with care because it is vital to the research. In order for participants to be educated about

the study, the title of the study was provided, and the method of data collection was

thoroughly explained. Because of the covid-19 pandemic, safety protocols will be closely

adhered to during the research period.

Informed consent

The respondent's willingness to engage in the study is essential, which is the

autonomy. As a result, the researchers will provide informed consent detailing the

important information about the study in order to respect their freedom and independence

in deciding whether or not to participate without external influences. The researchers will

not persuade volunteers in the study.

A formal consent will be included in the survey questionnaire form. Some

information about informed consent will be provided in the data collecting process.

Respondents should be aware of how their answers will be used in the study.

Privacy and Confidentiality

According to Human Research Protection Office (2022), privacy is a subject’s

ability to control how other people see, touch, or obtain information about the subject. ‘

The researchers ensured that the respondents read and approved the consent in the

google online voluntariness of their participation. Thus, the researchers concealed the

respondents' identity by giving the results in aggregate rather than revealing specific

responses that could lead to their identification. Confidentiality’ define as how private

information provided by individuals will be protected by the researcher from release.

Protection from harm

The researchers ensured that the respondents will not experience any negative

consequences as a result of their participation. Researchers will also protect respondents

30

from exploitation because it is essential to keep any data provided by respondents in the

study. The results would be made public, and respondents would be informed that other

parties involved might be interested in reading them.

Risks and benefits

This research ensured that it is for the benefit of student nurses, the enhancement

of the current curriculum, and the nursing environment, including professors' interventions

for improving communication skills. Other researchers working on comparable topics will

find this study useful as a resource and reference.

Deception

Throughout the duration of this study, the researchers ensured that no misleading

information about the subjects is given and that no data will be manipulated.

Regarding the reliability of the findings, the researchers will put in a lot of work to

collect data to produce a dependable output. In order to obtain reliable information,

inferences will be taken from a variety of sources. The techniques and findings of the study

will then be confirmed by the research adviser.

CHAPTER III

Results and Discussion

This chapter presents the findings on the key problems of the research study which

were to identify the impact of post-lengthened lockdown on the communication skills in

the hospital settings of level III nursing students at Wesleyan University-Philippines as

well as the conclusion and recommendations.

Table 2 shows the percentage distribution of the respondents according to age. Most

of the respondents ages 21 which is 104 or 50 percent. On the other hand, the respondents

ages 23 and above which is 14 or 6.7 percent is the least. This implied that most of the

respondents were at the age of 21. Thus, third-year nursing students mostly aged 21.

According to the study conducted by Whannell R., Allen B. & Lynch K. (2020), 18

to 22 years old students are in tertiary level. About 73% ages 19 years old when freshman

year, 20 years old when sophomore year, 21 when junior year, and 22 when senior year.

Moreover, 27% started tertiary level at the age of 18.

Table 3 shows the percentage distribution of the respondents according to sex.

There are 152 which 73.1 percent of the respondents who are female and 56 which is 26.9

percent are male. This implied that there were more female than male among the

respondents. This is expected because nursing profession mostly dominated by women.

According to Nurse Statistics (2022), 86% of nurses as well as nursing students are

women and 14% are men. As stated by Dill B. (2023), many female flocks to nursing

profession because of their inherent capacity to care for another human being.

Table 4 shows the percentage distribution of the respondents according to civil

status. Out of 208 respondents, 203 which 97.6 percent are single and 5 or 2.4 percent are

married. This implied that most of the respondents are single since they are still in their

junior year.

A study conducted by Kramer, M.S. (2019) stated that college students have wide

range of maturity. They are more ready for serious and long-term relationships than

entering married life. However, others may be more interested in exploring their options.

Table 9 presents the distribution of the impact of post-lengthened lockdown due to

covid-19 on the communication skills in the hospital settings of the respondents. The table

consists of 30 questions in which the item “I am not confident doing my rounds because

I’m afraid to interact with my patients” got the highest weighted mean of 3.14. It means

that the respondent’s communication skills are moderately affected by lengthened

lockdown due to covid-19. Furthermore, the item “I'd rather do paper works than rounds I

don’t like interacting with my patient” got the lowest weighted mean of 1.68. It means that

the respondent’s communication skills are highly not affected by lengthened lockdown due

to covid-19.

This implied that most of the items concerning the impact of post-lengthened

lockdown of the communication skills in the hospital setting were rated “disagree” with

the weighted mean of 2.31. It means that the respondent’s communication skills are

moderately not affected by lengthened lockdown due to covid-19.

According to the National Library of Medicine (2021), the covid-19 pandemic

brought a disruption to nurse education for both faculty and students as all non-clinical

nurse education courses worldwide moved to distance or online learning. However, the

sudden shift to online education does not hinder the students to learn communication skills

in interacting with their clinical instructors, co-nursing students, and patients.

The correlation coefficient between post-lengthened lockdown and age was found

to be -.058, between post-lengthened lockdown and gender was .059, and between post-

lengthened lockdown and civil status was -.109. None of these relationships were

statistically significant (p > .01). Therefore, there is no significant relationship between

lengthened lockdown and the profile variables (age, gender, and civil status) in relation to

the impact on communication skills in the hospital setting. Suggesting that these profile

variables may not play a substantial role in determining the impact on communication skills

in the hospital setting during extended lockdown periods. Thus, accept the null hypothesis.

Summary

This research aimed to determine the impact of post-lengthened lockdown on the

communication skills in the hospital settings of level III nursing students at Wesleyan

University-Philippines. The descriptive-correlational was used in this study. Survey

instruments in the form of questionnaire in google form were distributed in level III nursing

students to know their communication skills in the hospital settings. This research was

conducted at Wesleyan University-Philippines at the department of College of Nursing

(CON) located at Mabini Extension, Cabanatuan City, Nueva Ecija. 3100, Philippines.

Wesleyan University-Philippines.

The respondents were 208 out of 223 from level III nursing students at Wesleyan

University-Philippines S.Y. 2022-2023.

The data were analyzed using the Statistical Package for Social Sciences (SPSS)

which interpreted the frequency count, percentage, weighted mean, and the Pearson

Correlation (r) that were included in the statistical treatment of the data.

Specifically, this research undertaken answered the following questions:

1. What is the socio-demographic profile of the respondents in terms of:

1.1 age;

1.2 sex; and

1.3 civil status

2. How may the impact of lengthened lockdown due to Covid-19 on the

communication skills in the hospital setting of the respondents be described?

3. Is there a significant relationship between the respondents’ socio-demographic

profile and the impact of lengthened lockdown on the communication skills in the

hospital setting?

4. What recommendations can be made based on the findings of the study?

Summary of Findings

1. Profile of the Respondents

Age

Most of the respondents ages 21 which is 104 or 50 percent. On the other hand, the

respondents ages 23 and above which is 14 or 6.7 percent is the least. This implied that

most of the respondents were at the age of 21.

is 26.9 percent are male.

Civil Status

Out of 208 respondents, 203 which 97.6 percent are single and 5 or 2.4 percent are

married.

2. Impact of Post-Lengthened Lockdown on the Communication Skills in the

Hospital Settings

Consists of 30 questions in which the item “I am not confident doing my rounds

because I’m afraid to interact with my patients” got the highest weighted mean of 3.14. It

means that the respondent’s communication skills are moderately affected by lengthened

lockdown due to covid-19. Furthermore, the item “I'd rather do paper works than rounds I

don’t like interacting with my patient” got the lowest weighted mean of 1.68. It means that

the respondent’s communication skills are highly not affected by lengthened lockdown due

to covid-19.

3. Relationship between the Respondents’ Profile and the Impact of Post-

Lengthened Lockdown on the Communication Skills in the Hospital Settings

The correlation coefficient between lengthened lockdown and age was found to be

significant (p > .01). Therefore, there is no significant relationship between lengthened

lockdown and the profile variables (age, gender, and civil status) in relation to the impact

on communication skills in the hospital setting.

Conclusions

Based on the summary of the findings of the study, the following conclusions are

drawn:

1. A typical level III nursing students was female with an age of 21 years old and was

single. Therefore, most of the respondents were female, 21 years old and single in

status.

2. The impact of post-lengthened lockdown due to covid-19 in the communication

skills in the hospital setting of the respondents was moderately not affected.

Therefore, most of the items concerning the impact of post-lengthened lockdown

of the communication skills in the hospital setting were rated “disagree”. It means

that the respondent’s communication skills are moderately not affected by

lengthened lockdown due to covid-19.

3. Age, sex, and civil status does not influence the communication skills in the hospital

setting of the respondents. Therefore, there is no significant relationship between

the profile of the respondents and the impact of lengthened lockdown on the

communication skills in the hospital setting.

4. The respondents’ profile variables did not play a substantial role in determining the

impact on communication skills in the hospital setting during extended lockdown

periods. Thus, accept the null hypothesis.

5. In the review of related literature there were studies which contradicted the findings

of the study. On the other hand, there were studies which supported the findings of

the study. The result varies among the respondents.

Recommendations

Based on the summary of the conclusions, the researchers offered the following

recommendations:

1. Qualitative exploration: Employ qualitative research methods such as interviews,

focus groups, or observational studies to explore the lived experiences and

perceptions of nursing students regarding the impact of the lengthened post-

lockdown period on their communication skills. Gather rich narratives to gain

insights into challenges faced, coping mechanisms employed, and adaptations

made during this period.

2. Suggest to the clinical instructors a continuing education programs or resources

related to effective communication for nursing students to reinforce and expand

more their communication skills.

3. The present study should be re-applied to a large sample by comparing the impact

of post-lengthened lockdown due to covid-19 on the communication skills in the

hospital settings of every nursing student and use more influencing factors such as

level of self-esteem, way of speaking and peers.

47
WESLEYAN UNIVERSITY-PHILIPPINES

College of Nursing

4. To the future researchers, re-asses and expand review of related literature and

framework to have emergence of new evidences or other recent phenomenon of

your research problem and to test the validity and reliability of the research they

will be conducted.

5. For the betterment of future researches, it is suggested to consult a person with

expertise such as clinical instructor for the study to be thorough and credible in

measuring the communication skills in clinical settings.
