COMPANY HISTORY/DESCRIPTION
The MG Food and Event Services started year of 2015 by Ms. Maria Guia Faith
Fuellas. The letter M and G in the company logo comes from the owner name which is M stand
for MARIA and G stand for GUIA and also MG means MAKE IT GLAMOUR the company tag
line. The Business located at Brgy 9,Merchan St. Lucena City and believe.(we want to serve you
better).After long time in the field of quality and excellent services the business is known in
every corner in the city and also in some parts of Quezon Province.

Margarette, whose business address is Brgy Domoit,Lucena City and Anjo Constantino
collectively reffered to as the “parties”.
This contract regards the use of the property provided by venue, at the address above, on
03/02/2021 which is does include services or equipment to plan, coordinate, set up, deliver,
perform/conduct, or tear-down/clean-up after the event. In consideration of the mutual promises
and convenants contained herein, the Parties agree to follows:

EVENT CONTRACT
This contract is entered into between Reid rejan Villavicencio, with contact details
Queen Margarette Brgy Domoit Lucena City, Quezon/ 09069244349 and
Maria Guia Faith Fuellas, Event Director of MG Food and Event Services with
business address at Brgy 9, Merchan St, Lucena City, Lucena City 4301.
That:
MG Food and Event Services will provide full professional service including but not limited to
event managing, hosting, staging, directorship, and will provide the necessary supplies for the
personal upcoming event Mr. and Mrs. Villavicencio Wedding Reception which will be held
on March 02, 2021 Tuesday from the hours of 4 o’clock in the afternoon until 9 pm in the
evening.
TERMS AND CONDITIONS
1. Reid rejan Villavicecio will pay the total amount of (indicate the amount proposed and
agreed amount of full service), PHP 100,000 for the entire service provided. One half of the total
expenses PHP 50,000 is to be paid upon completion of this contract as a deposit to hold the date,
(note: deposit is not refundable) and the balance of the total amount to be paid two (3) days
before date of the event.
2. MG Food and Event Services furnish labor and equipment, materials, including set up and
breakdown.

PARTICIPANT AGREEMENT, RELEASE AND ASSUMPTION OF RISK
In consideration of the acceptance and participation in the services of MG Food and
Event Services, their planners, owners, officers, volunteers, employees, team members and all
other persons or entities acting in any capacity on their behalf (hereinafter collectively as Event
Team, I hereby release, indemnify, hold harmless, and discharge MG Food and Event Services,
from any and all liability for injury or harm that I or anyone might sustain and I hereby release,
hold harmless and discharge MG Food and Event Services, from any and all claims, demands,
or causes of action, which are in any way connected to my participation in this activity or use of
equipment to the extent allowable under Philippine laws.
1. I, Louise Marquez, manager of Queen Margarette Brgy. Domoit Lucena City,
acknowledges that my participation entails known and unanticipated risks that could result in
injury or damage to me, to property, or to third parties. I understand that such risks simply
cannot be eliminated without jeopardizing the essential qualities of the activity. THE RISKS
INCLUDE, AMONG OTHER THINGS: FALLING OFF THE STAGE RESULTING IN
MUSCULOSKELETAL INJURIES INCLUDING HEAD, NECK, BACK, HAND, AND
ARM INJURIES. Furthermore, MG Food and Event Services, employees have difficult jobs
to perform. They seek safety, but they are not infallible. They might be unaware of a
participant’s fitness or abilities. They may give incomplete warnings or instructions, and the
equipment being used might malfunction.
2. I expressly agree and promise to accept and assume all the risks existing in this
activity. My participation in this activity is purely voluntary, and I elect to participate in spite of
the risks.
3. I hereby voluntarily release, forever discharge, and agree to indemnify and hold
harmless MG Food and Event Services, from any and ALL claims, demands, or causes of action,
which are in any way connected with my participation in this activity or my use equipment or
facilities, including any such claims which allege negligent acts or emissions of.
4. Should MG Food and Event Services, or anyone acting on their behalf, be required
to incur attorney’s fees and costs to enforce this agreement, I agree to indemnify and hold them
harmless for all such fees and costs.
5. I certify that I have adequate insurance to cover any injury or damage I may cause or
suffer while participating, or else I agree to bear the costs of such injury or damage myself. I
further certify that I am willing to assume the risk of any medical or physical condition I may
have.
6. I agree that if any portion of this agreement is found to be void or unenforceable, the
remaining portions shall remain in full force and effect.
By signing this agreement, I acknowledge that if anyone is hurt or property is damaged
during my participation in this activity, I may be found by a court of law to have waived my right
to maintain a lawsuit against MG Food and Event Services, on the basis of any claim from
which I have released them herein.
