1.0 INTRODUCTION

The HSE Management System of Dodsal is planned to be applied for the


construction of Dolphin Upstream & compression Facilities Project, MC2 Contract
in Ras Laffan Industrial City in Qatar. This plan will be in full compliance with
Client's Health, Safety and Environmental Specifications.

Dodsal's apply their Company specific HSE Systems, which are described
hereunder in the HSE Policy Statements and Guidelines. As a Sub-contractor on
this Project, Dodsal will manage a project-specific HSE System to the highest
standards in order to

 efficiently protect the health of our workforce and of all other personnel
 maintain and improve all safety measures
 protect our environment during all phases of the Project

1.1 Safety and Health Guidelines

Work safety and the improvement of health matters are important aspects of our
entrepreneurial business activities. Consequently the Board of Directors supports
all measures to upgrade work safety and the health of our workforce.

Accidents and health risks can be totally or at least partially avoided if risks and
dangers will be managed and countered by an organised implementation of safety
measures.

These include in particular:

 A clear assignment of authorities and responsibilities


 Full attention, without any compromise, to safety standards, the use of
technical safety installations and personal protective equipment by our
employees as well as
 Continuous on the job training of all personnel

To promote work safety and health improvement is one of the predominant tasks
of each Manager. The prevention of accidents on the basis of efficient safety
measures is an indispensable proof of qualification.

It is, however, only through planned co-operation of all group and company
employees that these guidelines can be successfully applied in practice, in order
to further enhance work safety and health issues of our workforce.

The Board of Directors of Dodsal considers safety and health a decisive


prerequisite and to also improve our company's financial performance which in
turn leads to job security.

1.2 Terms and Definitions

Client – JGC Middle East FZE.

RLC – Ras Laffan Industrial City.

Occupational health and Safety – Conditions and factors that affect the well
being of employees, temporary workers, visitors and any other person in the
workplace.

Accident – An unplanned, undesired event or condition, that results in harm to


people or damage to plant / equipment.

Audit – A systematic and independent examination of Health, Safety and


Environmental management systems and their outcomes to determine their
effectiveness.

TBT – Tool Box Talk.

Construction Environmental, Health & Safety Management System – The


systems, procedures and core processes to be used to manage the Health, Safety
and Environment.

Hazard – A danger in the form of an unsafe behavior or condition, which could


lead to an incident or accident.

Risk – Combination of likelihood and consequence(s) of a specific hazardous


event occurring.

Tolerable Risk – Risk that has been reduced to a level that can be endured by
the organization having regard to its legal obligations and its own OH&S policy.

Risk Assessment – Overall process of estimating the magnitude of risk and


deciding whether or not the risk is tolerable.

Health, Safety and Environmental Planned Inspection – Physical condition


inspection of the work place undertaken by local managers accompanied by HSE
managers as appropriate.

Health, Safety and Environmental Review – A formal recorded evaluation of the


effectiveness of the construction Health, Safety and Environmental management
system and identification of actions for continuous improvement.

Health, Safety and Environmental Tour – A highly visible Health, Safety and
Environmental site walkabout by senior management to gain feedback from the
workforce and demonstrate commitment to HSE.

Hazards / Risk Coordinator – Individual appointed by the Project Manager


responsible for identifying hazards and carrying out risk assessments in the work
area that they are allocated.

Incident – Any undesired event or condition that results or could have resulted in
harm to people, damage to property or the environment or loss.

Sub-Standard Practice or Condition – Deviation from an accepted best practice


or condition.

Zero Accident Performance – Accident in the term “Zero Accident Performance”


is defined as an occupational injury or illness in the inability of an individual to
work one or more days after the day of injury or onset of the illness.

2.0 HSE MANAGEMENT SYSTEM

2.1 Leadership & Commitment

The main objectives of the Dodsal's HSE activities with reference to the Dolphin
Upstream & compression Facilities Project are to eliminate negative impact on the
health and safety of all contract and third party personnel and to optimize
environmental protection measures in an organised HSE System by visible
management commitment.

Individual HSE commitment includes the following actions or activities:

Demonstrate that all hazards, which carry the potential to cause harm, including ill
health, injury, damage to property or the environment, or Dodsal's reputation, will
have been identified and that appropriate controls and recovery methods will be in
place so that the risks are eliminated or at least minimised.

Dodsal will demonstrate commitment towards HSE by setting personal examples


by adherence to HSE rules, attendance at the HSE meetings, Corporate HSE
audits and by complying with Client's HSE specifications and the country's
legislation.

Dodsal is committed to comply in full with the JGC HSE Plan UCB 15-01.

Dodsal and Client‟s HSE Policy Statement shall be communicated to all the
employees via suitable means. Project HSE plan shall be made available for all
Dodsal staff.

The commitment is, in the first place, to prevent all accidents and incidents from
happening during our operation. Line management shall be responsible for
maintaining safety standards, safety statistics, for assisting in arranging audits,
inspections, conducting toolbox meetings, investigations of accidents and
incidents, reporting and follow-up. Details are included in this Project HSE Plan.

The HSE objectives for this Project will be communicated to all Dodsal personnel
and actioned on site by line management, through co-operation with the Head
Office & HSE Department at Dubai, U.A.E.

2.3 Organisation, Responsibility, Resources, Standards and Documentation

2.3.1 Responsibility for HSE

2.3.1.1 HSE Responsibility of the Project Manager

The HSE responsibilities of the Project Manager shall be among


other duties, the implementation of the contract HSE Plan in
compliance with Client's HSE specifications.

His duties include, but are not limited to :

 Consult with the Project HSE Manager / Safety Officer /


Supervisor with regard to all aspects of HSE relating to all
project activities, plant or machinery.

 Attend HSE and construction kick-off meetings, HSE and


progress meetings.

 Attend the prestart audit if applicable.

 Investigate all incidents and near misses with the assistance


of the HSE Manager / Safety Officer / Supervisor to determine
their causes and to plan and implement measures in order to
prevent recurrence.

 Prepare and submit HSE reports to Vice President


(Operations) and copy to Corporate HSE Manager and
Client's Executives when required.

 Establish the site HSE rules with special attention to unusual


operations and HSE requirement of JGC.

 Establish disciplinary action necessary to develop a good,


functioning HSE program.

 Conduct monthly HSE Committee Meetings with Supervisors


/ Foremen / HSE Manager / Engineers assigned to this
Project.

 Carry out audits as leader or member of the audit team as


directed by the General Manager (Operations).

 Maintain liaison with the Contract Custodians of the


Company.

 Follow-up on HSE audits.

 Ensure that all members of his staff:

- Have attended / completed the mandatory project HSE


Induction training.

- Have been provided with and that they use the


necessary PPE.

 Ensure that the monthly emergency response exercises are


conducted on site.

 Attend Client's

a) Monthly Contractors' HSE meetings if applicable.

b) HSE and progress meetings as per schedule.

2.3.1.2 HSE Responsibilities of the HSE Manager

The HSE Manager will report to Corporate HSE Manager and will
advise the Project Manager and his staff. His responsibilities will
include the following:

 To lead the entire team, maintaining HSE awareness and to


create safe culture and working environment through
innovative ideas.

 Render assistance to the Project Manager in formulating and


adhering to the client's contract health, safety and
environmental plan, in line with the HSE policy of Dodsal and
the requirements of the activities of the contract.

 Assist risk assessment team to formulate Method Statement


and Risk Assessment for specific construction activities.

 Issue necessary directives to Safety Officer / Supervisor and


render advice to the Project manager, as required.

 Monitor and assess HSE measures on visits to sites.

 Attend / take part in HSE and construction kick-off meetings.

 Attend HSE audit.

 Planning and coordinating the safety trainings and program.

 Formulate new safety measures required for the Project.

 Prepare and submit reports on HSE issues to, Client's Safety


Executives and the Dodsal's Head Office whenever required.

 Render necessary advice to the Engineers / Supervisors /


Foremen during site visits.

 Monitor Toolbox talks conducted by the Site Foreman and


HSE meetings conducted by the Safety Officer.

 Take part in the HSE meetings at site.

 Carry out HSE inspections at construction sites with reference


to all HSE issues such as environmental protection, hygiene /
sanitation, fire prevention / protective measures, emergency
exercises etc. and render reports.

 Take part / attend in Client's HSE meetings.

 Take part in incident investigation and reporting of all


Incidents

 Place special emphasis on 'near misses' and encourage


reporting all of the near misses. Investigate all near misses
and accidents and utilize experience of HSE improvement
and progress.

 Liaise with Client's Safety Departments.

 Visit the construction site during ongoing activities.

2.3.1.3 Responsibilities of the Safety Officer

The site Safety Officer will report to Project HSE Manager and will
advise the Project Manager and his staff. His responsibilities will
include the following:
 Ensure that supervisors and workers have received the
necessary / required HSE Induction training.

 Conduct regular HSE inspection at sites.

 Conduct HSE meetings at area level. (Toolbox Talk reviews


and for other activities.)

 Take part in investigations of accidents (incidents including


near misses).

 Gathering and compiling of data.

 Interact with Construction Manager, Supervisors and


Workers.

 Interact with Client's HSE Department.

 Advise on the inspection of vehicles / equipment.

 Perform safety audits per specific instructions.

 Identify the areas of environmental protection requirements.

 Assisting Project HSE Manager in training requirements and


implement education of the workforce.

 Improve HSE consciousness amongst all Dodsal personnel


by using standard training aids such as videos, posters, films,
handouts, etc.

 Monitor the activities involving potential environmental


damage and advise on corrective actions.

 Discuss and assist Project HSE Manager the effectiveness of


the HSE monitoring program.

2.3.1.4 HSE Responsibilities of the Construction Manager

The Construction Manager will be fully responsible for all activities


on work site. His detailed responsibilities include the following :

 Implement Dodsal's and Client's HSE Policy and the Contract


HSE Plan.
 Ensuring the instigation of construction related risk
assessments to ensure all hazards are identified and
subsequent risk mitigated to tolerable risk, communicating the
same to appropriate personnel.

 Ensure the provision and use of suitable safety equipment


and protective clothing.

 Ensure that plant, vehicles and equipment are suitable,


regularly maintained, correctly operated and not misused.

 Ensure that all accidents and unsafe acts and conditions are
reported and investigated.

 Ascertain that all 'First Aid' facilities are kept available and
maintained.

 Ensure that provision of a tidy and well-organised working


environment.

 Ensure that the employees, under his control, are adequately


trained for the work in hand.

 Issue clear, specific and regular instructions on HSE


requirements, when directing operations.

 Consult the Project HSE Manager / Site Safety Officer /


Supervisor on safety aspects related to particular activities,
plant or machinery.

 Monitor monthly emergency response exercises.

 Conduct site HSE meetings.

2.3.1.5 HSE Responsibilities of Equipment Engineer

The Equipment Maintenance Engineer shall be conversant with


relevant rules and regulations of the country and Client's HSE policy.
His responsibilities will include:

 Responsibility that HSE Standards are being implemented by


workshop staff and visitors at all times.

 Ensure that employees are given adequate


information, instructions, supervision and training to enable
duties and activities to be carried out safely as per the Dodsal
and client HSE systems.

 Ensure that suitable personal safety equipment such as


protective clothing, safety glasses, safety boots, etc. is
provided and used.

 Ensure that vehicles, plant and equipments are


adequately maintained in an efficient, safe condition and
comply with the relevant laws of the country and licensing
requirements.

 Ensure that vehicles and plants provided to clients (if


required or necessary) or to be used during the
execution of the contract comply with stated rules, standards
and specifications.

 Ensure that pressure vessels have valid test


certificates, are maintained and retested at prescribed
intervals.

 Ensure that cranes, lifting machines and appliances have


valid test certificates and are retested at the appropriate
intervals, or whenever major repairs or modifications have
been carried out.

 Ensure that the equipment displays SWL and is properly


inspected (by a third party) every 12 months. Records of
these inspections must be kept.

 Ensure that all "Lifting Tackle" has a valid test certificate,


means of identification and clearly displays the SWL. The
"Lifting Tackle" has to be maintained in good condition and
must undergo thorough inspection (by a third party) every 12
months with corresponding records kept.

 Ensure that only trained, competent, and experienced


licensed operators operate vehicles, plant and machines.

 Ensure that competent electrical staffs regularly check


fixtures, portable electrical installation and equipment.

 Ensure that fire extinguishers are provided and regularly


checked, and staff trained in evacuation procedures. Ensure
adequate housekeeping to reduce chances of fire and
ascertain safe working procedures. Maintain free passage
through escape routes at all times.

 Ensure the cleanliness and hygiene of stores area, workshop,


toilets, washrooms, tea-rooms and other facilities.

 Ensure that all accidents and dangerous occurrences are


reported and the more serious ones thoroughly
investigated.

 Ensure the safe storage, transport and use of flammable and


compressed gas cylinders.

2.3.1.6 HSE Responsibilities Of The Supervisors / Foremen

Since the Supervisor / Foremen are responsible for providing a safe


work environment to their workforce, the primary safety inspections
are their responsibility. As they spend most of their time in the
respective construction sections, it is their task to continuously
monitor working conditions. Their responsibilities include awareness
of accident potential, application of HSE measures in their areas,
ability to make intelligent decisions regarding corrective actions,
diplomacy in handling personnel and situations, a clear
understanding of our operations, and full knowledge of the rules and
regulations as per the Dodsal's Contract HSE Plan.

Their responsibilities include:

 To conduct daily TBT prior to the commencement of work,


with respective work crews / teams, and to provide additional
briefings in the event of any change in the work or associated
Method Statement, Risk Assessment, tools used, process and
/ or equipment.

 To be aware of the hazards of their activities and the


necessary controls to prevent incidents.

 To ensure that all personnel on site have undergone the


mandatory HSE Induction training courses prior to mobilising
to site.

 To ensure that all employees are provided with necessary


personal protective equipment.

 To implement and promote awareness of safe working


practices on job sites.

 To ensure that only trained and competent license holders


shall operate vehicles, plant and equipment.

2.3.1.7 HSE Responsibilities Of The Male Nurse

The Male Nurse will be based at or near to the work site. His
responsibilities include:

 To attend to all medical cases which come to the clinic. If the


medical case is beyond his capacity, to transfer the person to
the nearest Govt. Hospital for expert opinion and the
necessary follow up action. He shall follow all JGC site
medical procedures.

 He should only treat First-Aid cases, all other cases are to be


treated at the JGC Clinic. The JGC Doctor will advise if the
patient is to travel to local / international hospitals or facilities
for further treatment, dependent upon assessment and
condition of the patient.

 To keep a records of medical treatment on a day-to-day basis.


(Medical Treatment Register)

 To inspect the 'First Aid' boxes at site monthly and maintain a


record thereof.

 To explain to the Dodsal's workforce the importance of


preventive measures such as taking malaria tablets, saline
water etc, depending on the country's tropical climate and
location.

2.3.1.8 HSE Responsibilities Of Drivers And Operators

Only competent staff trained and in possession of the


appropriate license with Dodsal authorization and JGC consent, will
be permitted to drive vehicles and operate plant on or off the road
(e.g. light vehicles, H/D vehicles, cranes, plant, dumpers, scrapers,
graders, excavators, loaders, bulldozers, etc.)

Drivers and Operators shall observe all pertinent rules and


regulations and shall be trained and counselled in the
following:

 Check their vehicles and / or plant daily and complete the


relevant checklists.

 Not to allow anyone else to drive and / or operate there


allocated vehicle or plant.

 Headlights will be switched on while driving on graded roads.

 To abide by the site Speed Limit, and the wearing of seat


belts is compulsory.

 Ensure 8 hours of rest before driving, and a maximum of 12


hours of continuous duty in one shift, which is inclusive of
breaks if prescribed by client‟s rules and regulations.

General responsibilities for the artisans will be as follows:

 Observe the requirements of this HSE Plan and follow


instructions given by Foremen.

 Obey all hazard and safety signs.

 Use correct tools and equipment, report any damage or faults


found to the respective Foreman.

 Use correct personal protective equipment as directed and


report any defects to the Foreman.

 Keep and maintain tools and assigned equipment under his


control, in a safe and fully operational condition.

 Ensure that their work activities do not endanger themselves


or others.

 Help new employees to become aware of known


hazards and to maintain discipline (No 'Horse Play').

 Observe the laws & rules of the road and keep within the
permitted speed limit.

2.3.3.1 Training Policy

Dodsal's training policy is to provide all employees every possible


training and information available to enable them to carry out their
work in the safest possible way and in a risk free environment.

2.3.3.2 Objective

Prime objectives of HSE training are:

All Dodsal employees must undergo the basic HSE Induction course
conducted by JGC. All employees shall attain JGC conducted other
specific trainings as and when required through out the project life.

Tool Box Talks

TBT is one of the most effective and proactive ways of creating


safety awareness among the employees. Supervisor / foreman shall
conduct daily TBT prior to start the work.

Emergency response training for all employees shall cover:

c) Location of evacuation assembly points
d) Emergency phone numbers
e) Emergency response exercises
f) Identification of fire extinguishers type, location and method of
operation.

2.3.4 Standards and Documentation

The Project Manager / Construction Manager / Foremen have


attended the necessary safety courses such as Supervising Safety
and are aware of the safety standards. The responsible members of
the staff have read and are aware of the contents of this HSE plan.
The managing staff at the site are experienced and safety conscious
and they will ensure that safety hazards related to the specific
operations are constantly assessed and reviewed. The required staff
will also be nominated for the courses on educating them on safety
as and when required and cost reimbursed by the client.

A copy of the client‟s HSE Manual will be kept at each site office.

HSE training Matrix shall be maintained and updated by Project HSE


Manager. The details of training conducted at site shall be submitted
to Corporate HSE Manager as part of monthly HSE report.

2.4.1 Identification of Hazards and Effects

Dodsal will apply the Hazards and Effects Management Plan


(HEMP) to cope with any hazards, which are or may be associated
with this Project.

Key elements of the HEMP are:

Hazard Identification:

Hazard is something that has a potential to cause harm. Whilst


conducting risk assessment All Potential Hazards to be considered.
Below mentioned are few hazards with some example but not limited
to –
 Physical – Slips, Trip & Fall, Slippery floors, Confined space,
Working space constraint.
 Mechanical – Crushing, Caught in the machinery and
equipment.
 Chemical – Exposed to hazardous chemicals
 Electrical
 Environmental – Heat, Sand storm, High wind.

Hazards are being managed by ultimately applying the following


principles / methods:

1. Tolerating
2. Treating
3. Transferring
4. Terminating

Hazards, which may result in personal harm of whatever kind and


form, will never be tolerated under the HEMP system. This category
will only apply to isolated circumstances that may lead to slightly
negative consequences of negligible character such as extremely
remote cases of insignificant cost increase.

Treating is the next logical option after a hazard has been identified,
assessed and determined to be intolerable.

Controls and measures to treat this category of hazards are:

1. Engineering controls
2. Adequate work practices
3. Personnel Protective Equipment (PPE)

Engineering controls aim at eliminating the hazard(s) altogether by


carefully analysing the existing hazard and substituting it by a less
hazardous process, procedure, construction method, module, tool,
product or component.

Improvements of work practice include, but are not limited to,


enhanced education of the workforce, training of skills and
awareness, good housekeeping, proper / safe storage, etc,

The controlled uses of Personnel Protective Equipment (PPE) aims


at preventing certain hazards, which are inherently associated to
pipeline construction activities, do not reach our workforce.

2.4.2 Evaluation

The potential to cause harm or damage is defined as a hazard.

Harm could include ill health, minor or severe injuries and in the
worse of all cases, fatalities. Damage may be caused to plant and
equipment, property in general, vehicles, products, material or the
environment.
Since physical harm and damage to objects / property cause human
suffering, production losses, deviations of construction schedules
and increased liabilities with possibly far-reaching consequences, all
hazards must be analysed, assessed and controlled.

2.4.3 Recording of Hazards and Effects

The analysis of hazards, associated with pipeline construction work,


may be carried out by hazard categories, such as

The Construction Manager / Foreman will report all "Near Misses"


and incidents to the Project Manager and clients as required. These
reports will then be investigated and all recommended remedial
measures will promptly be put in place.

2.5 Planning & Procedures

The Dodsal's activities, within the scope of the operating country's laws and the
specifications, rules and regulations issued by clients are mainly based on "British
Standards", "Codes of Practice" and other procedures and Codes of Practice will
be drawn-up in-house and introduced, if required. These will then become an
integral part of the safety procedures.

These procedures (but not limited to) are included in the Dodsal's project specific
HSE Manual to be used by all Managers, Section Heads, Engineers and Site
Supervisors.

2.6 Implementation, Monitoring, Corrective Actions and Follow-up Actions

The HSE Plan for this Project shall be implemented and actioned on site by the
Project Manager / Construction Manager and through close liaison with the
Dodsal Head Office Manager and the HSE Division.

The implementation of the Contract HSE Plan will be audited quarterly by the
Corporate HSE Manager. The HSE audit will be conducted by the Project
Manager, Construction Manager, and Project HSE Manager. The targets
therefore will be monitored and results recorded to demonstrate HSE Compliance.
In actual fact the performance of activities critical to HSE shall be compared with
the Management controls established during application of the hazards and
effects management process. Instances of non-conformance shall be recorded
and follow-up and close-out actions documented.

The Vice President (Operations) of Dodsal will make inspections and reviews as
and when necessary.

2.7.1 Audit

The Corporate HSE Manager shall conduct an internal HSE audit on


a quarterly basis and shall be in line with JGC requirements as well
as Dodsal‟s own schedule.

The Project Manager, Construction Manager / Project HSE Manager


shall conduct a site HSE Audit monthly.

The Vice President (Operations) will participate in the Management


HSE Audit once during the execution period of this Contract.

2.7.2 Inspections

The following HSE audit / inspections shall be carried out by the


respective in-charges as per the schedule given below:

Monthly Senior Management HSE inspections shall be carried out


by the Line Managers.

2.8 Management Review, Corrective Action and Improvement

The HSE Manager / Project Manager / Construction Manager / Foremen shall be


involved in all audits and inspections that concern his respective field of
operations. Any item that is unsafe shall be taken out of service immediately.

Any procedure or practice which carries a potential risk must be discontinued and
shall not be resumed unless the potential for incidents is eliminated.

The HSE Department will maintain a list of all defects identified by them.

The Construction Manager will follow-up thoroughly any defects/discrepancies


observed by him or brought to his attention.

The HSE Department will check every month on the basis of an HSE punch list
that all reported defects are being actioned immediately.

2.8.1 HSE Promotion

To create Health and Safety awareness among the project


personnel, Dodsal shall conduct different promotional activities.
Such as –
- Safety Signs, Banners, Posters shall be displayed.
- HSE trainings, Daily Tool-box Talk shall be conducted.
- Every month Safety Warden, Safety Man of the Month shall
be selected based on those personnel‟s safety performance
and shall be suitably rewarded.

2.8.2 HSE Penalties

To ensure implementation of HSE procedure HSE penalty system


also shall be adopted. All project specific HSE rules shall be
communicated to all project personnel via different promotional
tools. In case of failure to comply with those HSE requirement
disciplinary action shall be taken on those individuals. The range of
penalties shall be from the written warning to that employee to
termination from the project. Site HSE Manager will assist Project
Director for deciding the penalties, specifically for project
termination.

3.0 HEALTH

3.1 Medical Emergency Response and Treatment Facilities

Health awareness amongst the workmen will be increased by way of films,


posters, etc. The employees will be introduced to a programme of health related
subjects to promote awareness, throughout our operations. The subjects will form
part of the monthly HSE meetings.

Dodsal will ensure that each employee undergoes a medical examination.

3.1.1 Medical Emergency Procedures

Emergency procedures are clearly defined in Section 4.2 of this


document.

3.1.2 First Aid Boxes / Medical Care

First Aid boxes will be provided at the work area, the contents will be
checked monthly by the Company Male Nurse.

First Aid boxes with proper supplies shall be made available at the
work area.

3.1.3 Medical Facilities

The Dodsal's Male Nurse, based on site, shall be responsible for the
provision of medical facilities to the employees.

The Male Nurse shall ensure that First Aid boxes are stocked with
proper supplies / medicines and are available at strategic locations
at the work site.

The Dodsal's Male Nurse shall make monthly visits to all work sites
to inspect and ascertain the content of the First Aid boxes.

3.1.4 Hygiene

The Dodsal's Male Nurse shall make monthly inspections of the


work sites and Dodsal's premises along with the Project Manager
and Project HSE Manager and review the hygiene standards.

3.1.5 Occupational Health

Occupational health is part of the Dodsal's HSE Policy and strictly


adheres to the following objectives in order to safeguard the health
of the employees and to develop health awareness and promotion in
general.

The Male Nurse if stationed at site will treat the employees for minor
illness and forward the sick attendance reports to the Project
Manager on a weekly basis.

In case of major illness or emergency, the respective patient shall be


referred to Govt. Hospitals without delay for further treatment.

In case of an incident, the First Aider shall follow all JGC site
medical procedures (The JGC Doctor will advise as to whether a
casualty shall be taken to Hospital) accompany the casualty to the
nearest hospital after application of the required First Aid treatment.
The Client will be informed of the incident and the case will be
monitored which includes progress reports on the recovery of the
patient.

Dodsal's Male Nurse shall check the hearing capacity and vision of
all operators and drivers if required, prior to the commencement of
the Project.

Dodsal's Male Nurse shall guide and ensure that suitable


rehabilitation programmes are undertaken for rehabilitation of
employees involved in an accident.

The Dodsal's Male Nurse shall attend the monthly HSE meetings
with the Project HSE Manager and site supervisory staff for
implementing health promotion programmes. He will advise and
guide them on health matters.

Hearing protection shall be made available to all workers if exposed


to noise levels of 80 db (A) or above. The type of protection shall be
in accordance with the type of noise hazard and the work being
performed.

The biggest cause of Occupational Health accidents in industry /


work field is "Handling and Lifting". Materials and equipment are
often lifted or moved. This can be done in two different ways: By
manually handling or by mechanical handling. The workforce shall
be trained as to correct working practices by showing films and
practical methods to protect them from back injuries and other
"Handling and Llifting" related hazards.

3.2 Living Accommodation and Temporary Facilities

Camp shall be constructed as per RLC Camp Construction Guideline.


3.2.1 Living Accommodation

 Portacabins shall be provided with wall mounted air


conditioned units for accommodation. Accommodation shall
be such that it provides protection against severe rain and
high wind.

 Each sleeping place shall be 4m2 per person and minimum


distance of 1 m shall be maintained between beds.

 Each occupant shall be provided a cupboard (locker) for


keeping personal belongings.

 Beds shall be fitted with a mattress, a blanket, bed sheet and


a pillow.

 Adequate window space shall be provided.

 All porta cabins shall be fitted with heat/smoke detector.

 Accommodation shall be maintained clean and hygienic, a


cleaner shall be employed to maintain the accommodation.

 Electrical and power supply shall be provided 24 hours for


lighting, refrigeration and air-conditioning.

 Alternative and backup power supply shall be provided for


essential and critical load such as – access/egress lighting,
area lighting, security lighting, fire alarm system etc.

 A Supervisor shall be appointed to supervise camp hygiene,


and to carry out regular inspections of the camp.

 A fence shall be erected around the Camp.

3.2.2 Temporary Facilities During Construction

 Roads, Car Parks, Material storage areas shall be


constructed for the duration of the construction work at site.

 Temporary construction facilities such as offices, fabrication


shops, medical facilities shall be constructed at site and shall
be assessable via temporary roads.

 Toilets, washing facilities shall be provided at site.

 Suitable sewage and drainage system shall be provided.

 Adequate fencing shall be provided to protect the construction


site.

3.2.3 Drinking Water

Typhoid fever, dysentery and other diseases are often the result of
contaminated drinking water and lack of proper sanitation on the job.

Therefore supply of potable water is important, particularly in remote


desert / tropical areas.

 An adequate supply of drinking water shall be made


available, if the water is not bottled, it must be filtered.

 Portable containers used to dispense drinking water shall be


capable of being tightly closed and equipped with a tap.
Water shall not be dipped from the container.

 Any container used to distribute drinking water shall be clearly


marked DRINKING WATER and not used for any other
purpose.

 The common drinking cup is strictly prohibited.

 Where single service (paper or plastic) cups are supplied,


they are to be used once only, both a sanitary container for
the unused cups and a respectable for disposing of the used
cups shall be provided.

3.2.4 Pest & Vector Control

Regular pest control service shall be provided.

3.3 Chemical Agents

Where any corrosive, irritants, systemic poisons, carcinogens and other harmful
agents are present in the working environment as gases, vapours, fumes, dusts,
liquids as solids, it shall be ensured that the risks are eliminated or controlled in
accordance with the safe working procedure.

3.3.1 Agent and Hazard Identification

All information on health hazard shall be available and training shall


be given to all personnel involved.

3.3.2 Control Measures

Exposures by inhalation, absorption, ingestion or injection as far as


is reasonably practicable shall be controlled.

3.4 Physical Agents

If harmful physical agents like noise, vibration, ionizing radiation, non-ionizing


radiation are present in the work environment, all efforts shall be made to
minimise harm and exposure to workmen.

3.4.1 Noise

All areas where the noise level is more than 85 db (A) shall be
identified, workmen exposed to 85 db (A) or more shall be provided
with ear protection equipment.

3.4.2 Vibration

Where exposure to vibration results in whole body or localised ill


health effects, it shall be controlled or eliminated.

3.4.3 Ionizing Radiation

Exposure to ionizing radiation may result in mutagenic or health


related effects. Therefore as far as possible exposure shall be
eliminated or controlled.

Radiological workers shall undergo annual medical check-up. All


possible control measures shall be taken that the radiological
workers does not receive an effective dose of more than 0.75 mR
(mille rem). All statutory regulation shall be followed.

During operation precautionary measures shall be implemented


which shall include but not be limited to:

 Plan and work procedures


 Authorised personnel supervision
 Continuous personal radiation metering devices for exposed
personnel.
 Barrication of area at safe distances and restricted entry.
 Safe storage of radioactive substances on site.
 Emergency procedure to deal with source containers and
fires.

Exposure to thermal stress may result in ill health effects such as


heat stroke and other effects. Therefore precautionary measures
shall be implemented which shall include:

 Acclimatization
 Provision of sufficient drinking water
 Sufficient rest periods and sun shade
 Control measures, including engineering, procedural and
personal protective equipment as appropriate
 Sufficient air circulation for confined space.
 Train workforce about heat stress.

3.4.5 Non-Ionizing Radiation

 Visible light
 Sufficient lighting shall be available at the work place that
helps in identification of hazards and to reduce the likelihood
of fatigue.
 Ultra-voilet (UV) light

- Sunlight

All possible measures shall be taken to prevent workers being


extensively exposed to sunshine, e.g. shaded working area,
neck flaps, hard hats, long sleeves, etc.

- Arc Welding

Welders shall be provided with adequate protection for skin


and eyes.

3.5 Ergonomics (Human Factors)

The construction environment is different from the manufacturing environment,


where conditions change from day to day. A construction environment varies daily
because of the use of different crews that are present for completing a particular
portion of the construction. Not only the crews are changing, personnel within a
crew shift around on a daily basis. Hence co-ordination and communication
between and within the work crews is most important.

To cover these aspects an ergonomics focal point shall be established that will
develop and present training programmes for ergonomic awareness. For training

meetings or separate training sessions.

3.6 Health Promotion

3.6.1 Alcohol & Drugs Policy

The following policy is part of Dodsal's terms and conditions of


employment and will be strictly adhered to:

 Being at work whilst impaired by drugs, or alcohol is strictly


prohibited.

 The illicit use of legal drugs or the use, possession,


distribution or sale of illegal drugs are strictly prohibited.

 All Dodsal employees should be totally competent to carry out


their respective tasks and to exercise their responsibilities
during working hours and whilst on duty.

 Failure to comply with any of the above will lead to


disciplinary measures that will ultimately include dismissal.
Smoking is permitted in designated areas only.

 Staff who feel that they are exposed or vulnerable to alcohol


are encouraged to seek medical advice and to follow
appropriate treatment promptly.

3.6.2 Fitness Standards

All employees shall undergo pre-employment medical check-up and


annually thereafter.

According to the climate, habitat and health environment of the area


in which the construction activities are executed, specific health
instruction shall be given to the employees.

Special attention shall be given to personal hygiene, especially for


the handling and preparation of food, laundry and cleaning services.

4.0 SAFETY

4.1 Work Permit

Necessary work permits shall be obtained from JGC for all non-routine or
potentially dangerous work at grass root area. JGC‟s PTW procedure shall be
followed. Complementary certificates shall be obtained for the work, such as:

- Handling Toxic material


- Hot work
- Manual excavation more than 1m deep.
- Mechanical excavation more than 0.5m deep.
- Electrical work on live system
- Entry into confined spaces
- Scaffolding
- Ionizing Radiation
- Hydraulic / Pneumatic testing of systems
- Demolition work.
- System Isolation.

For common area consolidated work permit issued by JGC shall be used.

4.1.1 Management of Work Permit System

Project Manager shall be responsible for all permit to work system.


Engineers / supervisors, nominated by Project Manager shall under
go permit receiver training of JGC.

Permit receivers shall be responsible for the safety measures of that


activity mentioned in the permit.

All the permit recommendations shall be followed.

Workers shall be briefed about the permit requirements before


starting the work.

Upon completion of work permits shall be closed and returned to the


permit issuer.

HSE department will monitor PTW procedure implementation.

When reporting an emergency situation the caller must:

 Stay calm and speak clearly and concisely


 Give his name, Exact location and caller I.D (if available)
 Describe the emergency in question
 Advise whether an ambulance is required or not
 Answer all queries from operator

4.2.1 Procedures

In order to cope with any kind of emergency at the site or camp the
following measures shall be adhered to:

An Emergency Team will be formed.

The Emergency Director shall be the Project Manager or


Construction Manager in his absence.

Radio, Mobile, Verbal Mode of communication shall be used.

Trained personnel like First Aiders shall be responsible to carry out


first aid actions in case of injuries or illness. Their names and phone
numbers shall be displayed in camp at appropriate locations.

JGC‟s Onsite Emergency Procedure shall be followed incase of any


emergency happens at site.

Fire alarms for evacuation shall be placed at strategic


locations within camps. Assembly points shall be clearly marked and
communicated to all the occupants.

Head count system shall be in place for camp.

Required type of fire extinguishers shall be installed wherever


required.

All site related emergency shall be reported to Dodsal Emergency


Response Team (ERT) member. Dodsal ERT shall response in
coordination with JGC ERT.

Emergency contact numbers shall be displayed at the work site and


camp.

Emergency action plan shall be communicated to all Dodsal site


personnel via training.

The emergency response exercise shall be carried out at site once


a month.

In case of any injury or illness, the first eyewitness will inform the
nearest First Aider who will inspect the injured person and
simultaneously dispatch a message or send a messenger to the
Project Manager / Construction Manager / Project HSE Manager to
inform them about the incident and ask for appropriate assistance
assessing the seriousness of the injured person. In case of serious
accidents the injured person shall be taken to a Clinic / Govt.
Hospital without delay. The client‟s Site Representative shall be
informed about the emergency immediately. Dodsal‟s Head office in
Dubai shall be informed about the incident. In all instances where
treatment required exceeds the capability of a first-aider, the JGC
Medical Center shall be informed immediately. Dodsal shall not
transport any casualty to an outside facility without informing the
JGC Medical and HSE Departments.

The work at that location shall be stopped and unsafe conditions /


causes for the incident investigated and corrected.

A detailed investigation report will be submitted to the client through


appropriate project document. Remedial measures to prevent a
recurrence shall be forwarded to the client with the corresponding
investigation report. Only after the site inspection by a client‟s
representative (Project Manager and Safety Officer) and the site and
procedures considered safe, shall work be resumed.

Other than personal injury at site the affected personnel shall –


 Inform Dodsal ERT member.
 Stop the work
 Switch off all the electrical equipments
 Go to the nearest assembly point based on the wind direction.
 Wait for further instruction.

Any site related incident / injuries shall be reported to JGC via


appropriate project document (incident reporting format).

4.2.2 Training

Emergency response training for all employees shall cover:

Emergency phone numbers
Emergency response exercises
Identification of fire extinguishers type, location and method of
operation.
Location of fire extinguishers / equipment and use of the same
Isolation of electrical supplies and location of the main switches
Location of emergency telephones
Availability of emergency vehicles
Assistance to First-Aiders

All persons on site will be trained and competent. Records of


training are to be kept on site for individuals. All individuals are to
sign as proof of training given. Regular checks should be made to
ascertain that personnel have understood the training that has been
given, these checks should be recorded.

4.2.3 Frequency

Emergency response exercises shall be carried out monthly as per


schedule.

4.2.4 Scenario for Exercises

The scenario for the emergency response exercises should consider


any one or all of the following:

Heart Attack
Man Lost
Road Traffic Accident
Accident on Site (broken leg or arm)
Fire or Gas leak
Chemical spill

The scenario will be planned and prepared by the HSE Manager.

4.2.5 Recording of Exercises

A report of all emergency response exercises shall be made. They


will be reviewed for identification of the shortcomings in the system
and to determine learning points. The review shall be made by the
Senior Staff / Management at the site and at Dubai Head office and
will be communicated to the site staff for implementation in future
drills.

4.3 Safe Work Practice

4.3.1 General Conduct

The Dodsal's activities, within the scope of the operating country's


laws and the specifications, rules and regulations issued by Client
are based mainly on "British Standards", "Codes of Practice" and
other procedures and codes of practice will be drawn up in-house
and introduced, if required. These will then become integral parts of
the safety procedures.

These procedures (but not limited to) are included in the Dodsal's
project specific HSE Manual to be used by all Managers, Section
Heads, Engineers and Site Supervisors.

It is, however, only through planned co-operation of all group and


company employees that these guidelines can be successfully
applied in practice in order to further enhance the work safety and
health issues of our workforce.

The Board of Directors of Dodsal considers safety and health a


decisive pre-requisite to also improve our companies' financial
performance, that in turn leads to job security.

4.3.2 Abrasive Wheels

Before assembling the grinding disc, make sure that the number of
revolution per minute (rpm) indicated in the disc does not exceed the
rpm specified by the manufacturer of the unit. Check expiry date on
disc to be fitted.
To attach the grinding disc screw in the nut by hand lightly -
revolution of the wheel will tighten the nut to the correct tightness.
Tightening the nut forcefully may cause the wheel to break during
operation.

Always use safety glass and face shield to protect the eyes and face
from grinding sparks.

All grinders must be run idle for a few seconds before starting the
work. This will allow the nut to tighten itself.

Inspect the grinding machine daily before use. Ensure the proper
color-coding.

Do not operate the grinder without the safety cover (guard).

Take care not to hit the grinding disc against any obstacle while in
rotation.

Do not drop the grinder to the floor, ground or working bench.

Do not use grinding sparks to ignite oxy-acetylene welding


equipment.

Do not lift or move the grinder by the electric cable.

Warn all persons to stay away from the grinding operation because
of the danger of metal splinters.

Safety devices as per manufacturers instructions must be provided


for the grinders.

4.3.3 Blast Cleaning

4.3.3.1 Safety Precaution

Grit Blasting shall be carried out in an enclosed designated area.


The blasting media shall not contain more than 2% free silica.

The grit blasting area shall be indicated by prominent warning signs.


Caution boards shall be displayed like “DANGER”, “KEEP AWAY”,
“BLASTING IN PROGRESS”

If abrasive blasting is performed in other than a blasting enclosure,


measures shall be taken to ensure the fugitive dust shall not cause
nuisance to other employees.

4.3.3.2 Personal Protective Equipment

Personnel involved in abrasive blasting operations are required to


wear hearing protection.

Personnel involved in abrasive blasting operations shall always wear


an approved blasting helmet. It shall be supplied with fresh air
operating under a positive pressure system to prevent entry of dust
or abrasive.

The air supply to the blasting helmet shall be drawn from a clean air
area, and the exhaust from engines or other harmful gases or dust
from other sources must not affect this area.

In addition to the above, personnel engaged in grit blasting shall


wear suitable long arm apron and high boots for body protection.

All above personal protective equipment shall be inspected at the


beginning and end of every shift. Defective parts shall be replaced.
Items that require specific attention are the helmet, visor, the air
filtering system and hoses.

The abrasive blasting nozzle must be equipped with an automatic


safety shut-off valve, so that if the operator loses his grip on the tool,
the air and grit supply are immediately shut off.

Air shall be delivered to the blasting tool through an electrically


conductive hose, connected to earth so that static electricity build-up
is prevented, to avoid both shock hazards to the operator and spark
discharges that could ignite any flammable dust generated in the
blasting operation.

All couplings on the air-line shall be secured properly with hose


whips to prevent accidental disconnection while under pressure.

4.3.3.4 Personal Requirements

All personnel engaged in the abrasive blasting operation shall be


experienced and fully trained in the safe operation.

All personnel who are required to work in the vicinity of abrasive


blasting operation shall be informed on the hazards of the work and
procedure to minimise danger to themselves and their equipment.

At all times personnel engaged in abrasive blasting shall be


supervised by a competent person, who will ensure that the
breathing air supply is maintained at all times.

At no time shall more than one person work at one specific blasting
site. If more than one abrasive blaster is employed on a particular
operation, their work areas shall be physically separated to prevent
the blast of one operation reaching the other.

4.3.4 Chemicals Handling and Use

Many substances used within Dodsal‟s sphere of operations can


cause temporary or permanent damage to the human body if
allowed to enter it in one or more of the following ways:

- Ingestion (swallowing)

- Direct skin contact causing epidermal damage

- Absorption through the skin causing systematic poisoning.

- Inhalation of gas, vapour, mist or dust

All substances used within Dodsal‟s sphere of operations should be


viewed with suspicion until they are definitely known to be harmless,
and this includes certain petroleum products themselves.
Precautionary measures should be taken and protective clothing and
equipment supplied where persons could be affected. The
importance of personal cleanliness, washing and wearing of clean
clothing cannot be over-emphasized.

Known harmful substances include various inhibitors, acids and


alkalis, dusts and mists, and certain products of our operations.
Before opening up any equipment e.g. vessels, pumps, etc. a Work
Permit must be obtained which must show that they are drained of
chemicals and also indicate what protective clothing is required by
the workmen. This is particularly applicable to equipment removed
from site to workshops.

4.3.4.1 Standard

No hazardous material will be permitted on site without the prior


approval of JGC. Dodsal shall submit to JGC HSE Dept. a
description of the hazardous material, how it is to be used, stored
and the quantity, together with the MSDS and a Risk Assessment(s)
for the substance relating to it‟s use in advance. The storage of
hazardous substances on site must be kept to the lowest practicable
levels.

Check all suppliers‟ data on the materials used.

Control of substances hazardous to health (COSHH): A list must be


maintained and the prevention procedures must be placed in stores
and other important working locations and in different languages. All
supervisors must brief the employees before using the hazardous
chemicals, and HSE Dept. must be contacted before using the
materials.

4.3.4.2 Handling

All persons engaged in storing, transporting or dispensing harmful


substances and all persons engaged in operating, repairing and
adjusting, dismantling or cleaning equipment which contains or has

contained such harmful substances or the products which have been


treated by those substances must be fully instructed in the hazards
involved and be aware of the first aid procedure. Suitable safety
measures must be taken against accidental exposure of persons or
equipment to these substances.
4.3.4.3 Nature of Chemical Hazards

Before a chemical is used within Dodsal‟s operations, its properties


should be assessed by the HSE Manager to highlight any potential
danger to persons through accidental contact.

Details of physiological, materials and fire hazards should be made


available to all concerned with the storage and handling of harmful
chemicals with advice on appropriate protective equipment and
necessary first aid facilities.

Knowledge of the action and effect of the dangerous substances


used is necessary and this should be included in the training of
personnel handling or using such materials.

4.3.4.5 Storing Dangerous Chemicals

Chemicals shall be stored separately from other stores and access


to such chemicals should be limited to authorised persons.
Containers must be clearly labeled as to their contents and the
manufacturer‟s instructions should be carefully followed regarding
storage conditions.

Inter-reacting chemicals must not be stored near each other. Glass


shelves must not be used for storage unless fully supported along its
length, e.g. wood, metals.

Every precaution shall be taken to prevent the escape of dangerous


substances, and means shall be provided for rendering safe any
such escape. Areas in which any escape occurs shall be barriered
off and marked with notices until rendered safe.

4.3.4.6 Toxic, Dangerous Gases, Vapours, Fumes and Dust

Where toxic gases, vapours, dangerous fumes or dust are likely to


be found, adequate ventilation shall be provided, and where the
nature of the operation makes it practicable, exhaust appliances
shall be provided as near as possible to the point of origin.

The exhaust gases from any internal combustion engine shall not be
permitted to discharge in any building or confide space, but shall be
directed to open air.

Special attention is drawn to the hazard of Hydrogen Sulphide (H2S)


which is found in a number of operations.

Where hydrogen sulphide is to be expected, pipelines are to be


marked with yellow identification at flanges and banded at intervals
along the line vessels containing hydrogen sulphide will have a
yellow and around the vessel to enable easy recognition to be made.
Valve glands and drain points shall also be marked in yellow.

Any work involving the breaking of a line, draining or an operation


where toxic gas, vapours may be encountered shall not be
undertaken unless a fresh air type or self-contained type of
breathing apparatus is worn. This work will invariably be carried out
under a work permit.

4.3.4.7 Precautions when handling Inhibitors or Additives

These may be flammable in which case the usage precaution


against fire must be observed, or they may be corrosive or cause
skin irritation, suitable precautions covering the use of the specific
inhibitors or additives shall be implemented.

As far as possible the inhibitor or additive shall be handled in closed


systems, and the use of open containers should be avoided.

Suitable protective clothing e.g. Face Shield, PVC / Rubber gloves


and Apron shall be worn if there is a possibility of splashing, spillage,
or when working on or handling equipment coated with the material.

Prompt action should be taken if the inhibitor or additive is in contact


with eyes, skin or clothing.

- Eyes: Wash out at once with the solution provided and report
to the Hospital / First Aid attendant.

- Skin: Wash off thoroughly with soap and water and report to
the Hospital / First Aid attendant.

- Clothing: Remove any contaminated clothing and treat the


skin affected as above.

Clothing (including leather boots) which has been in contact with an


inhibitor or additive should be thoroughly cleaned with soap and
water before being worn.

4.3.4.8 Paints & Coating

An increasing number of products, many of them synthetic fall into


this category. While some are low risk, others are highly flammable,
toxic, or corrosive and can cause harmful effects to the skin. The
manufacturer‟s advice on precautions to be taken during use shall
be observed. Where practical, products that do not present high risk
or produce harmful effects should be chosen.

Use and Operation

Where operations involving paint / solvents producing flammable


vapours are carried out:

- Great care shall be taken when mixing, decanting and


transferring paint / solvents.

- Naked flames and lighted cigarettes shall not be permitted.

- Adequate ventilation should be provided.

Empty or redundant paint and solvent containers should be disposed


of in such a manner that they cannot give rise to risk of fire or hazard
to the environment.

All paint / solvent spillage should be cleaned up without delay.

Any rags or other combustible material contaminated with paint /


solvent should be disposed of in a metal container with metal lid.
The container should be emptied regularly.

Were paint spraying operations are in progress, in addition to the


above requirements the following should apply:

- The correct type of spraying equipment shall be used.

- Spraying should be carried out in a booth with suitable


exhaust ventilation.

- Warning notices and barriers should be provided as


appropriate.

- Fire fighting equipment should be readily at hand.

Special precautions, examples of which are given below, shall be


taken when working with paint that may contain lead to prevent
absorption through skin, swallowing or inhalation of the fumes:

- Old paint layers should not be removed by dry rubbing or


blowlamp as poisonous dust / fumes may result. The surface
should be prepared using wet and dry abrasive and water.

- Where removal cannot be achieved by non-dust / fume


producing methods, special precautions shall be taken to
protect personnel from intake of lead dust / fumes.

- Lead paint should be removed by use of water rinse, paint


remover or solvent.

Particular care is required when working in confined spaces, where


ventilation is restricted, or spraying where levels of toxic or
flammable vapors may become high. Adequate arrangements for
respiratory protection, and the provision of breathing apparatus shall
be made. Smoking and other sources of ignition shall be strictly
controlled.

Adequate washing facilities, including the provision of proprietary


cleansers shall be made available. Workers shall be instructed to
use these facilities before any meal break and after work.

4.3.5 Compressed Gas Cylinders

4.3.5.1 Safety Requirement

All cylinders shall be identified by colour coding and maintained in a


clear and legible condition.

The name of the gas contained within the cylinder shall be clearly
marked on the cylinder body.

All cylinders shall be maintained in good condition and free from


corrosion.

The date of manufacture and / or last hydrostatic test shall be


clearly and legibly marked on the cylinder body.

Cylinders shall have protective guards or caps fitted to protect the


valve assemblies.

All Couplings associated with cylinders containing flammable gases


shall be left hand threaded. For other gases right hand threads shall
be used.

Cylinder valves shall be of the correct type for the gas content and
pressure rating of the cylinder and manufactured to international
standards.

4.3.5.2 Transport of Compressed Gas Cylinders

All cylinders when being transported shall have the protective


guards or caps in place. Cylinder shall be transported in the upright
position and placed in such a manner that they cannot strike against
other cylinders.

Oxygen cylinders shall under no circumstances be transported with


acetylene or any other flammable substances.

Cylinders shall not be dropped from a vehicle, but shall be lowered


to the ground under controlled conditions.

Cylinders shall not be lifted using chains or metal slings. Lifting shall
be carried out using flat band slings.

4.3.5.3 Storage of Cylinders

Cylinder storage areas shall be clearly identified, with the names of


gases stored.

Cylinders shall be stored in an upright position within the store and


secured with chains.

Full and empty cylinders shall be stored separately.

Cylinders containing combustible gas shall be stored separately


from oxygen.

Storage rooms shall be well ventilated and designed so that the


cylinders are shaded from the direct rays of the Sun.

Smoking, naked lights or sources of ignition shall not be allowed


within or near to the vicinity of the cylinder storage.

Fire extinguishers shall be located outside the storage area.

4.3.5.4 Use of Cylinders

Compressed gases shall be handled by only experienced persons

Before using gas cylinders, ensure that they are properly supported
and secured to prevent being knocked over.

Flash back arrestors and check valves shall be used on both hoses
at all times.

Hoses shall be properly colour coded to the Internationally


recognized standards.

Cylinders shall be kept in chained trolleys or tied off when on site.

Connections to piping, regulators and equipment shall be always


kept tight to prevent leakage.

4.3.6 Demolition Work

Due to inherent hazards in demolition work, special care shall be


devoted to safe procedures and communications to safe work
performance.

It is not appropriate to make assumptions in any stage of work and


precautions shall always be taken. From start to finish, each person
connected with the work shall adopt an attitude of "Prepare for Any
Possibility". Responsibilities shall be delegated only to those whose
professions qualify them to make the judgements and decision that
are required to assure the safest possible performance of the work.

Special care shall be exercised to locate and make underground or


otherwise hidden lines, cables and sewers using the best available
information.

4.3.7 Electricity

4.3.7.1 Electrical Installations

Only trained, experienced and authorised electricians shall be


deployed to carry out repair / maintenance work.

All permanent and temporary installed electrical equipment shall be


protected by an Earth Leakage Circuit Breaker (ELCB) or Ground
Fault Circuit Interrupter (GFCI) against over load and ground fault
conditions.

Switchgear shall be mounted properly on rigid metallic frame


suitably earthed.

All control switches shall be properly secured.

Care shall be taken that all electrical equipment is connected to the


correct voltage for which it is designed.

All electrical equipment, with the exception of double-insulated hand


tools, shall be fitted with a ground conductor.

Plug adapters shall not be used, all equipment shall be fitted with
the correct plug to fit in the corresponding socket.

Multiple appliances or power tools shall not be plugged into a single


socket via a multi-outlet adapter.

All permanently installed electrical equipment shall be fitted with


isolation switches. Such isolation switches, if remote from the
equipment, shall be fitted with effective lock-out devices so that they
may be made safe while undergoing maintenance or repair work.

4.3.7.2 Portable Electrical Powered Tools

All portable electrically powered tools shall be inspected, tested,


serviced and used in accordance with the manufacturer‟s
instructions. Only personnel who have been trained and authorised
shall be allowed to use them.

All portable electrically powered tools shall have the following


construction features:

Fitted with adequate fuse protection, either in the tool body or its
electric plug, and be either:
Class 2, double insulated or
Class 3, a tool designed for operation at extra low voltage (max. 42
Volts AC)
Be provided with an automatic shut-off device that renders it
inoperative if the operator releases his hold.

When electric power extension cords are used, they shall have
adequate current rating for the equipment to be powered and
contain an independent earthing conductor. They shall be fitted with
a male plug on one end and female socket on the other. The socket
housings shall be of sound shockproof construction.

Portable electrically powered tools shall not be used in potentially


hazardous or explosive environments.

Power extension leads shall be routed in such a way so as not to

hot surfaces, sharp or abrasive surfaces or in contact with corrosive


chemicals.

Periodic maintenance of electrical tools and ELCB‟s shall be carried


out and documented.

Where applicable, protective guards shall be securely fitted, and


correctly adjusted.

Tools found defective shall be labeled and removed from service for
repairs.

Electrically powered portable saws shall be fitted with both a fixed


guard over the upper half, and a movable guard over the lower part.
Both guards shall be operative.

When cutting materials that may generate dust, proper respiratory


protection shall be worn.

4.3.8 Entry Into Confined Spaces

4.3.8.1 Preparation

A Toolbox meeting shall be held by the Supervisor / Foreman with


all the crew members, to provide the following explanation to them
clearly:
 The scope of work
 The hazards involved
 Control measures provided
 The emergency procedure.

Ensure that one person in-charge of the crew is holding the


responsibility of the job to be carried out, he may be the Permit
Holder.

Ensure that a “Permit to Work” is obtained and specific work activity


is clearly written e.g. confined space entry.

Utilize those persons who have experience in confined space work.

4.3.8.2 Procedure

Ensure that the tank / vessel is drained, flushed, washed, emptied


and vented.

Confirm by using the appropriate measuring equipment that the


oxygen concentration in within acceptable limits, and no toxic and/or
explosive gas vapour exists in the air. Continuous monitoring of the
air in the confined space shall be undertaken during the work
period.

Supplied airline respirators should be used where required.

Maintain a watch person outside the tank / vessel, who will keep a
continuous watch on the personnel working inside.

Provide a proper ladder access inside the tank / vessel for


personnel.

No gas cylinders (oxygen-acetylene-propane etc.) should ever be


allowed into confined spaces, and check that hoses are clamped
firmly to avoid accidental gas leakage.

4.3.9 Excavations

4.3.9.1 Preparation

A Toolbox meeting shall be held by Supervisor / Foreman with all


the crew members providing a clear explanation on:

 The scope of work


 The hazards involved
 Control measures provided
 The emergency procedure

Ensure that one person in-charge of the crew is holding the


responsibility of the job to be carried out, he may be the Permit
Holder.

Ensure that a “Permit to Work” is obtained from the controlling


authority and the specific work activity is clearly written e.g.
excavation, and shown clearly on the drawing.

4.3.9.2 Precautions

Before starting excavations, the actual work area shall be checked


with cable detector for the presence of any underground electrical
cable, or “pilot” trenches should be made by hand digging.

Excavated material shall be kept 1 meter away from the trench


edge, to prevent it falling back into the excavation.

All excavations shall be barricaded with caution tape and warning


signs like “DANGER”, “DEEP EXCAVATION”, “STOP”, “KEEP
AWAY” shall be provided.

If the excavation is crossing a road:

 A Diversion road shall be provided.


 Caution boards shall be kept at appropriate distance to warn
and reduce vehicle speed.
 Person with flags shall be provided to control traffic.
 Flashing lights shall be provided during the hours of
darkness.

When hand excavation work is undertaken, or if personnel are


required to enter the excavation, strict adherence of the following is
required:

In order to prevent a cave-in, adequate shoring or sheathing shall


be provided for:

 Excavations deeper than 2 meters.


 Excavations deeper than any adjacent underground buildings
or structures.
 Suspect or unstable ground conditions.

If shoring and sheathing is not provided, the excavated side


surfaces shall have suitable slope to prevent a cave-in.

4.3.10 Lifting / Crane Operations

4.3.10.1 Crane Operator - Personal Requirements

The operator shall be in possession of a current crane operator‟s


license applicable to the country in which the project is located.

Dodsal through their rigging superintendents, etc. shall ensure that a


competence test is performed prior to engaging a particular
operator, and his competence must be beyond doubt.

Dodsal shall issue a certificate indicating the type of cranes the


operator is allowed to use and other cranes shall not be operated
unless the operator is properly trained to use another type of crane
under the supervision of a qualified person.

Crane operators shall be more than 20 years of age, and have


satisfied the company concerning physical health and medical
condition. All the crane operators must be in possession of valid
medical fitness certificate, and a medical examination must be
carried out with an authorized medical practitioner once in a year.

4.3.10.2 Crane Operators - Special Precautions

The following information shall be known to all supervisors and


crane operators: a separate hand book should be issued and
signed to establish that the information has been given and is
understood.

When the crane operator is physically or mentally unfit, he shall not


engage in crane operation.

The operator shall not engage in any practice that will divert his
attention while operating the crane.

When handling loads, the operator shall not start crane movement
until the banksman or complete load is within his range of vision.
The banksman shall be identified by an orange vest, and he shall
assist the crane operator to make a safe lift.

The crane operator shall respond to operating signals only from the
trained appointed banksman, but he must obey an emergency stop
signal from anyone at any time.

The crane operator is in command of all crane-handling operations


and is primarily responsible that the crane operation is carried out in
safe manner. Whenever there is any doubt as to safety, the crane

operator shall have the authority to stop and refuse to handle loads
until safe conditions have been re-established.

Each day before the crane is put into use the operator shall perform
all movements and maintenance necessary to ensure that the
machinery is functioning satisfactorily. He shall ensure that all
safety devices are set and operating correctly and that the wire
ropes are in approved working condition.

Cranes shall not be used if any limit switches or other safety


equipment is out of order.

No equipment shall be used for loads exceeding its safe working


load (SWL). When multiple wire rope or chain slings are used the
angle of the legs shall be considered.

The load shall be attached to the hook by means of slings or other


devices, suitable protection (softening) shall be provided between
the sling and sharp unyielding surfaces of the load to be lifted.

Cranes shall not be used for transport of personnel, except as


prescribed in the hoisting of personnel by a personnel basket.

Cranes must only be used for vertical lowering and lifting of loads.

Limit switches during normal operation of the crane must not be


used as stopping devices.

The operator of the crane must not leave the operating seat when a
load is suspended on the hook.

If there is a temporary warning sign on the switch or engine staring


controls, the operator shall not close the switch or start the engine
until the warning sign has been removed by the responsible person
placing it there.

A suitable container or basket shall be used for mass transportation


of loose material, so that no material is likely to fall during operation.
Test certificates must be obtained before using the basket or
container from an authorised agency.

Main hoist arrangement of cranes used for loading / unloading of


supply vessels, should only be used when the hoisting and lowering
speed exceeds the moving velocity of the supply vessel.

4.3.10.3 Riggers

The rigger is responsible for properly attaching the load to the crane.

He must be properly trained in slinging / rigging, the standard lifting


hand signals and the general capabilities of the crane with which he
is working.

As far as it is possible, he should remain with the same crane


operator so that they can work as a team.

He is authorized to give signals to the crane operator for load


attachment only. In certain complicated lifts, the banksman may
require the assistance of a rigger, but the riggers signals must be to
the banksman and not to the operator, who should be warned that
such an operation is in effect.

4.3.10.4 Serviceability, Maintenance and Inspection

No crane shall be operated unless it is released to do so by the


concerned section. This will be issued after the crane has been
found to be serviceable by a Maintenance Engineer.

All repairs and alterations shall be carried out under the direction of
the Maintenance Department. If repairs to the main structure are
necessary, a proof test shall be carried out prior to going into
operation.

Where welded repairs are carried out to steel members, e.g. boom,
„A‟ - frame or masts of cranes, the repair shall be inspected by a
qualified welding inspector and a proof load test carried out.

The Maintenance Engineer may at any time withdraw a crane from


service if he considers it is not in good operating condition.

Cranes and lifting equipment shall be in good mechanical condition


and assembled in accordance with the manufacturer‟s instructions,
deviations from these instructions shall not be permitted.

At least once in every year each crane shall be subjected to an


independent survey. Color code will be followed for all lifting
machines.

This survey shall consist of:

- A thorough examination

- A load test.

A copy of a test certificate shall be requested and kept on file by the


Rigging Superintendent and the Maintenance Engineer.

4.3.10.5 Restricted Areas

A crane shall not enter a restricted area, or operate within one and a
quarter boom lengths of an oil line, gas line, or overhead power
cable unless a work permit has been issued. The minimum distance
from an overhead electrical cable = boom length (fully extended) + 6
meters.

A permit will only be issued if the crane is fitted with a spark arrestor.
The HSE Dept. shall be contacted for further advice on this matter.

A distance of 20 feet shall be maintained between any part of an


operating crane, or its load or attachments and any overhead
powerline. Should it be necessary to approach any closer a work
permit will be required.

4.3.10.6 Side Booms and A-Frames

Sidebooms and A-Frames are mainly used for the movement of


pipes and other associated material. The load of a side boom or
A-frame should be raised a minimum distance above the ground
during transit. This will ensure that, where there is soft sand, etc,
the load will ground before the equipment becomes unstable.

4.3.10.7 Riggers, Slings and Lifting Equipment

- Always wear the provided personal protective equipment


(PPE), Safety Helmet, Gloves and Safety Footwear.
Signalman shall wear separate vest.

- Check the efficiency of the ropes, chains and slings.

- Use the correct equipment for the job, installing the slings in
the correct method.

- Check the maximum safe working load (SWL) of the slings on


the chart.

- Protect the safety of others by keeping them away from or


under suspended loads, and preventing them from crossing
the area where work is in progress.

- Clearly indicate the maneuvers to be carried out to the crane


operator.

- Every worker must pay attention to the stages of the


operation performed so that the job has a correct,
chronological and constant sequence.

- A nut or a bolt, even though quite small and light, becomes a


dangerous missile when dropped from a height.

Preliminary Requirements

- Carefully assess the weight of the load.

- Check the strength of the sling attachments.

- Work out the correct suspension points in order for the load to
be correctly balanced once lifted.
- Check the safe working load (SWL) of the lifting and slinging
equipment to be used.
- Method statement shall be prepared and get approved form
JGC for lifting and transporting awkward load or the load
more than 50T.

4.3.11 Locking out / Isolation of Equipment, Pipes and Pipelines

4.3.11.1 Electrically Powered Equipment

The purpose of this procedure is to protect any one who performs


electrical work on live electric circuits or electrically powered
equipment, or who performs mechanical work on live electrically
powered equipment.

When a specific electric circuit is to be deactivated, safety tags and


locks shall be installed after the electric power supply is
disconnected.

A work permit shall be obtained from JGC before work starts. The
person performing the work shall check that de-energizing actually
took place by operating the actuation switch for electrical driven
equipment, and by checking the physical disconnection in the MCC
or substation, in the case of electrical equipment.

A precautionary note in Red Colour "DANGER - DO NOT


OPERATE" shall be placed with the tag at the de-energizing point.

4.3.11.2 Isolation of Equipment, Pipes and Pipelines

Isolation of equipment and pipelines shall be done in accordance


with the Permit to Work procedures. The necessary isolation shall
be done as per isolation drawing and tags / locks installed at the
isolation points. A precautionary notice "DANGER - DO NOT
OPERATE" shall be placed in red colour.

4.3.12 Scaffolding

4.3.12.1 Scaffolding Hazards

The severe accidents to workers that are due to falls from a height
or to scaffold collapses clearly show the importance of properly
constructed and maintained ladders and scaffolds. Apart from
collapses the principal hazards are:
a) Unsecured ladders slipping
b) Use of unsuitable or faulty materials
c) Inadequately supported scaffold boards
d) Inadequate or irregular platform widths
e) Omission of mid and hand rails or toe boards

4.3.12.2 Instruction for Scaffolding, Staging and Ladders

All personnel, including supervisors, involved in the erection and


dismantling of scaffold and staging shall be experienced in the work.

No person, other than scaffolders and supervisors involved, shall be


permitted to be upon any part of incomplete scaffold or staging.

All erection and dismantling activities shall be carried out in such a


way that non-involved personnel, in the vicinity of the work, are not
at risk.

Personnel, other than the scaffolders and supervisors involved, shall


only be permitted upon scaffolding staging that has been inspected
and clearly labeled “ready for use”. The Scaffold-tag system will be
complied with.

The mixing of scaffold material types shall not be permitted.

4.3.12.3 Inspections

The scaffolding inspection logbook shall be maintained on site to


record all inspections.

Prior to erection, the supervisor in charge shall inspect the ground


area upon which the scaffolding is to be positioned.

A scaffold “ready for use” (Green Tag) or “not to be used” (Red Tag)
tag shall be placed at the access point.

Where any scaffold or staging is altered it must be inspected prior to


use.

Scaffold shall be inspected by scaffold competent person.

4.3.12.4 Scaffolding Terms

4.3.12.4.1 Standard

A tube used as a column or vertical in the construction of a scaffold,


and transmitting the load to the ground via a base plate.

4.3.12.4.2 Ledger

A tube spanning horizontally and tying a scaffold longitudinally, it


may also act as a support for put logs, transoms or board bearers.

4.3.12.4.3 Transom

A tube spanning across ledgers to tie a scaffold transversely, which


may also support a working platform.

4.3.12.4.4 Ledger Bracing

A tube incorporated diagonally between alternate pairs of standards.


This shall start at base level and continue through the full height of
the scaffold.

4.3.12.4.5 Facade Bracing

A tube provided to preserve longitudinal stability. Facade bracing


shall start at base plate level and continue to the top lift, it may
zig-zag from bottom to top, or can be a continuous sloping tube.

4.3.12.4.6 Tie

A tube used to connect a scaffold to rigid structures.

4.3.12.4.7 Guard Rails

A tube connected at the edge of platform to prevent persons falling


from platform, guardrails shall be a minimum of 1 meter above the
working platform.

4.3.12.4.8 Toe-Boards

A Board positioned at the edge of a platform so as to prevent


persons, tools and materials falling from the platform.

4.3.12.4.9 Boards and Working Platform

The dimensions of work platforms may vary; dependent upon their


intended function, the platform shall be fully boarded between the
handrails.

4.3.12..5 Means of Access

The means of access to and means of egress from, all scaffolding


shall be via ladder(s) or staircases.

Ladders shall be of proprietary manufacture and shall be


constructed from timber or aluminum; a timber ladder must not be
painted.

The correct angle for ladders shall be a ratio of 1 to 4 (75 degrees),


with a minimum protrusion above the landing of 1 meter, or 4 to 5
rungs.

Tools and materials should not be carried by persons ascending or


descending ladders.

A person working on or from a ladder must always have both feet on


the rungs and a secure handhold. If work to be done necessitates
the use of both hands, a safety harness must be used.

Only one person should be on a ladder at a time.

4.3.12.6 Dismantling

No persons other than experienced scaffolders shall be involved in


the dismantling of scaffolding.

No items shall be thrown to the ground; it is preferable to lower all


items by bag with line or lowering by hand.

During dismantling, no component that endangers the stability of the


remaining structure shall be removed.

The procedure of dismantling shall be orderly and planned and


should proceed generally from the top in horizontal sections.

4.3.12.7 Personal Protective Equipment (PPE)

All personnel shall be provided with, and shall use, appropriate


personal protective equipment (PPE).

4.3.12.8 General

No part of any operational line or pressurised piping shall be used as


a support, or tie point for scaffold.

Where any hot work is to be carried out, scaffolding boards shall be


protected with a non-combustible covering, which shall be laid and
tied in such a way that it shall not constitute a tripping hazard.

During the possibility of a thunderstorm, no work shall be executed


at a height where a person can be exposed to lightning.

Electrical, pneumatic and hydraulic tools shall not be hauled up or


lowered by their power lines.

Where practicable, personnel shall not be assigned to work directly


below any overhead work positions. Barricade shall be maintained.

Tools and equipment shall not be kept in any location where they
may present a tripping hazard, or may fall from the structure.

4.3.13 Welding & Cutting

4.3.13.1 Required Protective Clothing

4.3.13.2 Electric Welding

4.3.13.2.1 General Regulations

All electric welding equipment (Distribution panels, switches,


sockets, plugs, cables, electrode holders etc.) must be kept in
perfect working order and protected from any risk of damage, and
distribution panels will be provided with inbuilt ELCB‟s.
Splicing in welding cable is not allowed.
Operating equipment shall not be used for grounding.
Re-bar shall not be used for grounding.
Additionally, all electrical equipment shall be inspected in
accordance with the project inspection criteria.

4.3.13.2.2 Distribution Panels and Switches

The electric current distribution panels must also be located where


they are protected from atmospheric conditions; their metal parts

should be connected to the earthing system; they should have a


main switch complete with fuses and ELCB‟s; there should be a
clearly visible warning sign and notices giving details of the voltage
used and a number of sockets with safety covers corresponding to
the number of machines to be supplied. Distribution panels must be
provided with an inbuilt ELCB system.

4.3.13.2.3 Cables and connections

The cables feeding current to the electric welder must be of a gauge


suitable for the strength of current they will carry. They must be kept
in a location where they will not be crushed, safe from contact with
sharp objects and insulated from humidity. Any connections must
be made only by means of the appropriate insulated fittings (plugs
and sockets).

The plugs and terminals used must be suited for the current and the
type of use.

The electrode-holding clamp must have a well-insulated handle that


should not be broken or chipped, also check for cracks or chips in
the insulation cover.

4.3.13.2.4 Safety Precautions:

Electric Welding: Take extreme care with the ultra violet rays; always
use an effective mask with idiactinic glass, and warn those people
around you when you are about to start work. If possible use
shielding.

Wait a few minutes before checking or removing the „slag‟ from the
welding operation so that the weld has time to cool down, and use
clear glass goggles to carry out this operation.

When the welding work is done the cables must be put back in their
place, the electric welder must be switched off and the switch on the
distribution panel should be disconnected to eliminate any chance of
an accident. All the distribution panels must be inbuilt with an ELCB.

Precaution shall be taken to contain welding and gas cutting sparks


from falling on the ground while working on the higher elevation.

4.3.14 Working At Heights

Where it is required to work at height, appropriate scaffold shall be


erected, or working platform (Man basket, Arial platform) shall be
provided.

Makeshift scaffold, such as boxes, crates and drums are prohibited.

Scaffolding other than metal tube shall not be permitted, except


when a written approval is provided for a special case.

Wooden ladders shall not be allowed except for electrical work.

Ladders and scaffolding shall be regularly inspected and records


shall be maintained.

Proper escape and access routes shall be provided.

A Full Body Harness with double lanyard and shock absorber shall
be used at all the time, and in conjunction with the site requirement,
Dodsal shall operate the 100% Tie-Off policy when employees are
exposed to a fall of 1.8 meters, or more. Other fall protection
arrangement i.e. lifeline, rope grab, stop chute etc. shall be used as
and when required.

4.4 Equipment Standards

4.4.1 Hoisting Equipment - Ropes, Chains and Cables

It shall be ensured that ropes, chains and cables are inspected and
registered on arrival at the site and at regular intervals of six (6)
months thereafter. All records of inspections shall be maintained.

Each item shall be marked with an identification number, and the


safe working load (SWL). Monthly inspection and color-coding shall
be done.

4.4.2 Ladders

Ladders shall be of proprietary manufacture and shall be


constructed from timber or aluminum.

“Home Made” ladders constructed on site shall not be permitted.

Ladders must be in good condition and free from defects i.e. broken
rungs, split strings.

Ladders:

- Must be secured at the top


- Must be properly positioned at the base
- Must extend atleast 1 meter above the upper step-off point.

All ladders shall be positioned at a ratio of 1 to 4 (75 degrees), and


have a minimum protrusion above the landing level of 1 meter or 4
to 5 rungs.

All ladders shall be inspected at monthly intervals.

Aluminum ladders shall not be used in zone 0 or zone 1 hazardous


areas.

Tools and materials should not be carried by persons ascending or


descending ladders.

A person working on or from a ladder must always have both feet on


the rungs and a secure handhold (maintain three (3) points of
contact). If work to be done necessitates the use of both hands, a
safety harness must be used.

Only one person should be on a ladder at a time.

4.4.3 Quality of Tools and Usage

Always use the correct equipment for the work, and avoid resorting
to makeshift solutions.

An electrician must check all electrical hand tools before distributing


to the site. Monthly inspection and color-coding shall be maintained.

A maintenance register for all electrical hand tools must be


maintained by the Electrician / Site Store Keeper.

Equipment should be kept clean and in perfect working order,


unserviceable equipment must be tagged and returned to stores.

4.4.3.1 Hand Tools

4.4.3.1.1 Screwdrivers

Screwdrivers should be of the correct size and tip to fit the head of
the screw. Screwdrivers shall not be hammered or twisted, and they
must never be carried in the pocket.

4.4.3.1.2 Hammer

The right type of hammer must be selected for the job.


Hammerheads should be secured to handles with proper wedges.
Brass / Nylon headed hammers must be used in hazardous areas to
avoid creating sparks.

4.4.3.1.3 Chisels

Cutting edges shall be kept sharp with the original shape and angle
maintained, when a sledgehammer is used for striking a chisel, the
chisel should be held by a second person using a pair of tongs.

4.4.3.1.4 Pickaxes and Shovels

Pickaxes and Shovels shall be maintained in a serviceable


condition. Shovel blades shall not be allowed to become blunt,
turned, split or jagged.

4.4.3.1.5 Spanners and Wrenches

Only the right type and size should be used. Extension leverage for
spanners and wrenches shall not be used unless the wrenches are
designed for such use. A wrench shall not be used as a hammer
and should not be hammered to tighten a bolt unless otherwise
designed for such use. Use of adjustable wrench will be
discouraged.

4.4.3.1.6 Pliers

Pliers are not to be used as a wrench. They are meant for gripping
round objects and for cutting soft metals and wires. When cutting
metal / wire with pliers, it shall be ensured that the scrap does not fly
off and cause an injury. Only long handled pliers shall be used to
cut wire under tension.

4.4.3.1.7 Jacks

Hydraulic / Screw Jack capacities must be adequate and strong


enough to raise and maintain the intended load, and the load must
be positioned on the centerline. No one shall get under the load
being raised. The stability of the equipment and the Jacks must be
ensured before raising the equipment.

4.4.3.1.8 Hacksaw

The correct type of blade shall be selected to suit the material to be


cut. The blade should be set with the teeth pointing in the forward
direction and sufficient tension shall be applied to ensure the blade
is maintained rigid within the hacksaw frame.

4.5 Personal Safety

Dodsal is responsible to ensure that all employees wear the appropriate personal
protective equipment during all operations where there is an exposure to a
hazardous condition. All PPE purchased for use on this project shall comply with
an appropriate standard (CE or equivalent) and be suitably marked.

4.5.1 Clothing

All construction employees shall wear appropriate clothing suitable


for the job. Any loose clothing, such as Shes / Mandrels / Gutturals
shall not be allowed. Coveralls (applicable only during SIMOP‟s &
operation phase, and these will be of the fire retardant type for
employees working directly on facility plant).

4.5.2 Head Protection

Employees working in areas where there is a possible danger of


head injury from impact or from falling or flying objects shall be
protected by safety helmets. Hardhat shall be fitted with sweat band
and chin strap.

4.5.3 Foot Protection

Steel toe lather safety shoe shall be worn to provide foot protection.

Whenever it is not feasible or possible to reduce noise levels or


duration of exposures, ear protection devices shall be provided.

Ear protective devices inserted in the ear (ear plugs) must be fitted
correctly to provide adequate protection. Plain cotton is not an
acceptable protective device.

4.5.5 Eyes and Face Protection

Employee shall be provided with eye and face protection equipment.


Safety glass is mandatory requirement. Safety glass shall be clear
unless specified. Those employees using prescription glass shall use
OTG (Over The Glass) safety glass with the prescription glass.
For grinding, chipping, gas cutting or other such activities need face
shield. Face shield shall be fitted with hardhat. Welding hood also
shall be fitted with hardhat. Chemical goggles shall be provided for
handling hazardous chemicals.

4.5.6 Respiratory Protection

In emergencies or when controls are required to prevent harmful


exposure to employees, appropriate respiratory protective equipment
shall be provided by Dodsal.

The chemical and physical properties of the contaminant as well as


the toxicity and concentration of the hazardous material shall be
considered in selecting the correct type of respirators.

The nature and extent of the hazard, work requirements and


conditions as well as the limitations and characteristics of the
respirator shall also be factors considered when making the
selection.

Employees required to use respiratory protective equipment shall be


instructed in the use and limitations of such equipment.

Respiratory protective equipment shall be inspected regularly and


maintained in good condition. Gas mask canisters and chemical
cartridges shall be replaced as necessary so as to provide complete
protection. Mechanical filters shall be cleaned or replaced as
necessary in order to avoid undue resistance to breathing.

Respiratory protective equipment that has been previously used shall


be cleaned and disinfected before it is issued back to the employee
or to another employee. Emergency rescue equipment shall be
cleaned and disinfected immediately after each use.

4.6 Road Safety Awareness

Vehicles allocated to this Project shall be suitable for the work that they are
deployed for and shall comply with manufacturer‟s specifications. All vehicles to be
used for this contract using the client‟s public network roads shall be approved and
will comply with pertinent local traffic laws. A back up alarm will be fitted to all
vehicles, and all vehicles must abide by the site traffic rules, and be provided with
a dry chemical fire extinguisher.

Toolbox talks and other safety meetings are an important means of


communicating safety messages to the personnel in charge of vehicles. Videos,
safety posters and friendly counseling at unscheduled meetings are major
communication steps, whenever and wherever possible.

4.6.1 Vehicle Safety Regulations

All small passenger vehicles shall be fitted with seat belt on all the
seat. Driver shall not start the journey without ensuring use of seat
belt by all the passenger.

Riding on the material carrier will not be allowed. Only vehicle


intended for carrying for passenger shall be used.

Site specified speed limit shall be followed.

4.6.1.1 Vehicle Specification

Vehicles shall be adequate and suitable for the work that they are
allocated and shall comply with the client‟s and manufacturers
specifications.

All vehicles will be Dodsal or client approved and will comply with
local laws of the country where they will be operating.

All lifting equipment installed on vehicles, will be certified by a third


party inspection.

4.6.1.2 Vehicle Maintenance

Vehicle maintenance shall be carried out strictly as per the schedule


as laid down in the attachment of this HSE plan. Vehicles shall be
kept road worthy, and only qualified mechanics shall make repairs.
Daily vehicle checks shall be made by the driver before the start of
his journey and / or duties.

4.6.1.3 Journey Management

Journey Management shall be in accordance with current Journey


Management Plan specific for the project, the procedure must
comply with the all Dodsal‟s requirements.

All long distance journeys' undertaken will be recorded.

In addition to the Dodsal's Journey Management Plan, a board will


also be maintained at the site office by a nominated person who
would be the focal point for monitoring the journeys. The Journey
Plan shall be transmitted by fax to the destination and will also be
advised over the phone.

When the nominated person monitoring the arrivals, notices that a


vehicle is overdue by 2 hours, he will initiate the appropriate action
and initiate the search for the overdue vehicle.

4.6.2 Refueling of Vehicles / Construction Equipment and Running


Maintenance

Refueling of vehicles shall be done in a designated area only

- Smoking shall not be permitted in the area.

- Welding or any spark or flame producing operation shall be


stopped within a radius of 10 meters during re-fuelling
operations.

- All engines must be stopped when refueling.

- Plastic containers used for transporting or transferring light


hydrocarbons, such as petrol or kerosene are prohibited.

- Regular maintenance must be conducted (e.g. brakes and the


steering mechanism).

- Fuel cans shall be of metal type with a spring closing lid and
spout cover that will safety relieve pressure if the can is
exposed to high temperatures.

- Empty fuel oil and lubricating oil drums / containers shall


immediately be removed from the Work Site.

4.7 Fire Safety

4.7.1 Fire Prevention

Adequate fire fighting equipment will be available at various


locations (stores, workshops, fuel compounds, camp, vehicles)
consistent with the relevant risk. The fire extinguishers will be
monthly inspected by the HSE department and records kept and
maintained as required. Staff will receive training in fire prevention
and the use of extinguishers. Regular exercises will be conducted for
fire extinguisher practical training.

Electrical wiring shall be installed in compliance with the


requirements of the applicable country codes wherever applicable.

Smoking shall be prohibited at or near the vicinity of operations that


constitute a fire hazard and such areas shall be conspicuously
posted: "No Smoking or Open Flames".

4.7.1.1 Open Yard Storage

Driveways between and around combustible storage piles shall be


sufficiently wide enough and maintained free from rubbish,
equipment or other materials. Aisle ways shall be spaced to afford
access for vehicles / fire trucks.

The entire storage site shall be kept free from accumulation of


unnecessary combustible materials. Weeds and grass shall be kept
down and a regular procedure provided for the periodic cleanup of
the entire area.

Portable fire extinguishing equipment, suitable for the fire hazard


involved, shall be provided at convenient, conspicuously accessible
locations within the yard area. Portable fire extinguishers shall be

placed so that minimum travel distance to the nearest unit shall not
exceed 30 meters (100 feet).

4.7.1.2 Indoor Storage

Storage shall not obstruct or adversely affect the means of exit.

All materials shall be stored and handled with due regard to their fire
characteristics.

Non-compatible materials that may create a fire hazard shall be


segregated by a barrier having a fire resistance of at least 1 hour.

Materials shall be stored to minimize the spread of fire internally and


to permit convenient access for fire fighting. Stable piling shall be
maintained at all times. Aisle space shall be maintained to
accommodate the widest vehicle that may be used for fire fighting
purposes.

Adequate clearance shall be maintained around lights and heating


units to prevent ignition of combustible materials.

A clearance of 0.6 meter (24 inches) shall be maintained around the


path of travel of fire doors unless a barricade is provided, in which
case no clearance is needed. Material shall not be stored within 1
meter (36 inches) of a fire door opening.

4.7.1.3 Storage of Flammable and Combustible Liquids

Only approved containers and portable tanks shall be used for


storage and handling of flammable and combustible liquids. Metal
safety cans shall be used for the handling and use of flammable
liquids in quantities greater than one gallon, except that this shall not
apply to those flammable liquid materials that are highly viscid
(extremely hard to pour), which may be used and handled in original
shipping containers. For quantities of one gallon or less, only the
original container or metal safety cans shall be used for storage, use
and handling of flammable liquids.

Storage areas shall be kept free of weeds, debris, and other


combustible materials.

Flammable liquid store shall be made outside of main store.

Fire Extinguishers will be provided where combustible material and


flammable liquids are stored.

At least one portable fire extinguisher shall be provided on all tank


trucks or other vehicles used for transporting and / or dispensing
flammable or combustible liquids. All fuel trucks shall have two dry
chemical fire extinguishers (i.e. one 6 kg for the tank section, and
one 2 kg for the cab area).

There shall be no smoking or open flames in the areas used for


fuelling, servicing fuel systems for internal combustion engines, and
receiving or dispensing flammable or combustible liquids.

Conspicuous and legible signs prohibiting smoking shall be posted


wherever required.

The engines of all equipment being fuelled shall be shut off during
the fuelling operation.

4.7.1.4 Signs, Signals and Barricades

Signs and symbols required for accident prevention shall be


displayed at all times when work is being performed, and shall be
removed or covered promptly when the hazards no longer exists

“Defective Tags” shall be used as a temporary means of warning


employees of an existing hazard, such as defective tools, equipment,
etc.

Construction areas shall be posted with legible traffic signs at the


points of hazard, as situation demands.

When operations are such that signs, signals, and barricades do not
provide the necessary protection on or adjacent to a highway or
street, flagman or other appropriate traffic controls shall be provided.

Hand signaling by flagmen shall be by use of red flags at least 0.5


meter square (18 inches square) or sign paddles, and in periods of
darkness by red warning lights.

Flagmen shall be provided with and shall wear a red or


orange reflective warning garment while flagging. Warning garments
worn at night shall be of reflective type material.

4.7.2 Protection

Dodsal shall be responsible for the provision and maintenance of an


effective fire protection and prevention program at the job site
throughout all phases of the project work and shall ensure the
availability of the fire extinguishers as required.

Access to all available fire fighting equipment shall be


maintained at all times.

All fire fighting equipment, provided by Dodsal shall be


conspicuously located. Fire extinguishers shall be provided for all
welding machines and office locations.

All fire fighting equipment shall be periodically inspected and


maintained in operating condition. Defective equipment shall be
immediately replaced.

Fire extinguishers and water buckets shall be protected from the


direct sunlight.

A fire extinguisher shall be provided within 15 meters (50 feet) or


wherever more than 22 liters (5 gallons) of flammable or combustible
liquids or 2 kilograms (5 pounds) of flammable gas are being used
on the jobsite. This requirement does not apply to the integral fuel
tanks of motor vehicles.

The use of Carbon tetrachloride and other toxic vaporising liquid fire
extinguishers is prohibited.

4.7.3 Fire Fighting

Adequate numbers of personnel will be trained on the use of fire


extinguishers, including practical training.

Fire plan shall be prepared in accordance to JGC fire plan.

5.0 ENVIRONMENT

The Management is committed to comply with the guidelines and specifications of


the client in all matters of environmental protection related to the execution of this
construction project, by exercising efficient environmental project management.
All measures to be taken will be in line with Project's Environmental Policies.

It is the overriding objective of Dodsal‟s environmental measures to keep


environmental awareness within the workforce as a number one priority at all
times, to eliminate or minimise the impact of the Dodsal's logistic and technical
activities during the execution of the contract.

In detail, the Management, supported by the Project Manager and shall ascertain
that the workforce is made aware of all environmental requirements and,
whenever necessary, trained to protect the environment.

Dodsal will identify potential environmental problems such as accidental spills,


handling of combustibles, chemical substances etc. in order to avoid incidents.

Waste management throughout this Project is of prime importance. It is the


objective of Dodsal to:

 Reduce the generation of waste


 Handle and discard unavoidable waste in accordance with the site rules
and regulations
 Minimize the overall impact on the environment.

SPILL RESPONSE PROCEDURE

Any accidental construction related spillage shall be contained and cleaned as


soon as possible. Where necessary, the area of spillage shall be cordoned off
and hazard signs shall be posted. Spillage of hazardous liquids must be reported
to the client.

Continued emphasis shall be given to good housekeeping standards. The Safety


Officer will carry out periodical inspections. It is essential that contingency plans
are in place for responding to emergencies involving accidental spills of
chemicals, and that the effectiveness of such plans shall be tested regularly
through emergency drills and exercises.

All spills and leaks of any chemical shall be reported immediately by Dodsal to the
JGC Environmental representative.

All personnel involved in the transportation and handling of chemicals must have
sufficient understanding of the HSE aspects of the chemicals to enable them to
carry out their work safely, in an environmentally acceptable manner, and not
endanger their own health.

Copy of MSDS of all chemicals shall be kept in the clinic and Dodsal HSE
department. A copy will be submitted to the client also.

5.1 Human Resources

Because of the significant importance to use natural resources in an


environmentally sound manner while protecting the health and safety of our
employees and the public, we herewith repeat Dodsal's guiding environmental
principle, i.e. our dedication and continuous efforts to improve the compatibility of
our operations with the environment while economically supplying high quality
products and services to our clients.

Since environmental considerations continue to be a priority and key element in


our planning of construction operations, the environmental protection measures
will be in full compliance with the client‟s project environmental specifications and
requirements as well as with the applicable laws, rules and regulations.

Compliance with the client's environmental specifications is ascertained by


Dodsal‟s planning, implementation and maintenance of environmental protection
plans and systems throughout the execution of the project.

5.2 Emissions to Air

5.2.1 CFC's & Halons

CFC's or Halon type fire extinguishers shall not be used at the


construction site and camp.

5.2.2 Exhausts & Fumes

All the generators shall be regularly serviced and maintained so that


they do not generate smoke.

All vehicles shall be serviced and maintained regularly.

5.3 Aqueous Effluents

5.3.1 Domestic Sewage

All types of domestic sewage shall be disposed through wastewater


treatment facilities as per RLC guideline.

5.3.2 Other Aqueous Effluents

All aqueous effluents shall comply with State of Qatar Environmental


Protection laws and regulations. Care shall be taken to prevent soil
contamination by equipment lubricants, fuel and paint. Secondary
containment and collection of spills shall be made and disposed of in
the area specified by Client.

5.4 Waste

5.4.1 Principles for Waste Handling and Disposal

Waste disposal forms part of the Dodsal's activities. Waste


generated during field operations / industrial activities shall be
disposed of through the client‟s waste facilities as provided to
Dodsal. The respective environmental representative will be
responsible for coordinating the disposal.

Chemicals, hazardous liquids etc. shall be stored on site on hard


standing and protected from radiation due to the sun, if necessary.

HAZMET cards for all chemicals shall be made available, and


appropriate hazard signs shall be provided. Personnel using such
chemicals shall be familiar with the appropriate handling procedures
and use of P.P.E.

Any accidental spillage shall be contained and cleared as soon as


possible. Where necessary, the area of spillage shall be cordoned off
and hazard signs be posted. Spillage of hazardous liquids will be
reported to the client.

Hazardous waste shall not be stored more than 90 days.

Continued emphasis shall be given to good housekeeping and clean


up. The Safety Officer will carry out periodical inspections. Monthly
environmental audit will be done.

5.4.2 Working on Site

If client does not provide appropriate facilities for the disposal of


environmental waste. Dodsal are required to put a system in place
with local approved vendors (The client can furnish details of these
approved vendors for reference).

Any waste from vehicle maintenance such as filters, belts, tyres,


glass, canisters and PVC waste, shall only be disposed of through
use of the appropriate facilities provided by the client.

Lubricants / chemicals shall be disposed of at the chemical disposal


point, at the client‟s designated disposal area.

Molten welding slag waste, electrode debris and grinding discs, etc,
shall be collected daily in metal containers and disposed of through
the provided facilities.

Continued emphasis shall be given to good housekeeping and clean


up. The Safety Officer will carry out periodical inspections.

All paints that have exceeded their shelf life and are beyond their
date of expiration, shall be returned to the manufacturer for disposal
or further processing.

All sludge arising out of tank cleaning and other operations shall be
disposed of at the Sludge Farming Site, or as advised by the client
and / or local municipal regulations.

In connection with the application and removal of plastic coating, any


excess or unusable material shall be collected separately in special
bins, wrapped in polythene waste bags and sealed. These bins are
to be labeled "Man-Made Mineral Fibre Waste - Do Not Open" and
disposed of in the dedicated landfill locations as provided to Dodsal
by the Client.

5.4.3 Promotion

Dodsal shall promote and communicate to its employees the need


for environmental conservation within the application and
implementation of the HSE Plan.

Critical examination of the processes generating significant amounts


of waste, alternate materials should be analysed and applied to
reduce the volume of waste.

Reviewing the efficiency of existing processes and analyse any


deviations from the site proven practices.

Checking if regular inspection and maintenance of plant and


equipment is carried out to limit mechanical failures, which may be
the source of incidental generation of waste.

Checking if good housekeeping and segregation of waste systems


are being practised during operations / processes, as this will lead to
waste reduction.

Ensuring that old stocks have been utilised and depleted when
changing over to other raw materials, products or chemicals.

5.4.4 Practice

Dodsal's practical measures with regard to the above items are as


follows:

 All environmental actions will be taken by the site manager


and personnel in compliance with this HSE Plan. Individual
practical measures will aim at preventive actions rather than
reacting to incidents. In detail, the workforce will apply the
following practical steps:

 All detergents shall be biodegradable.

 Disposal of oil in the workshop will be to client‟s designated


area.

 Stocks of hazardous chemicals such as paints, thinner and


cleaners will be held to a minimum.

 Accident and emergency response will be actioned and


reported in the same manner as emergency procedures in
this document.

 Litter and waste will be handled and disposed of in


compliance with approved procedures, spills will be coped
with in line with the site proven and approved practices.

 It is the policy of Dodsal to take responsibility for any spill that


is directly attributable to our actions and / or negligence.

5.5 Appearance of the Worksite

Good housekeeping is one of the first laws of accident prevention and shall be of
primary concern to all line Managers and foremen. Good housekeeping shall be
planned at the beginning of the job by supervisory personnel and carefully
supervised by them to the final cleanup of the job. Confusion will be reduced and
operations will be made more effective where good housekeeping planning has
been carefully made.

During the course of the work, areas will be free of scrap and debris of all kind.

Rubbish, debris, and waste and materials constitute fire and accident hazards
and shall be removed from work area as they accumulate. This applies in
particular to field warehouse and field shop areas where operations may remain at
one place only over a temporary period of time. Machinery should be checked to
be sure all oil has been wiped from areas where employees may step so as to
prevent them from slipping on such surfaces.

Any oil spills should be immediately cleared up so as to avoid a safety or fire


hazard.
