COMPLAINT AFFIDAVIT

I, Juna Dela Cruz, 43 years old, (DOB: March 14, 1990) female, married,
teacher and residing at No. 1 Bagongbayan, Barangay Sto. Barbara, Quezon City wife
of (V1) after having been duly sworn to in accordance with law, do hereby depose and
states that:

1. I am the husband of MARK Dela Cruz, 45 years old, (DOB: June 13, 1990)
male, married, foreman and residing at No. 1 Bagongbayan, Barangay Sto.
Barbara, Quezon City;

2. On December 16 at 5:30 P.M, as I arrived home from the school, I received

call from PCpl Selos Santos, Investigator on Case informing that my husband
was lying on the pavement blooded in the site where he works.

3. Upon receiving the information, I immediately went to the area and saw my
husband was on pavement blooded sustained gunshot wound in the forehead.

4. Based on the statement of witnesses, my husband shot by suspect SAN

JOSE y Batang who caught their college and later in the custody of Arresting
Officer.

5. I am executing this affidavit to charge SAN JOSE y Batang for the crime
under Art. 232 of RPC committed against my husband, MARK Dela Cruz.
