Annexure 1: Specification of PPEs

1.0 Head Protection

Safety Helmet with Chin Strap


MOC -
Shell - HDPE/ABS,
Chin Strap-Nylon,
Harness with sweat band-LDPE
Approvals: confirming to IS-2925/EN: 397/ANSI Z 89
Marking: Certification, date of Manufacture
Shall be free from visual defects

2.0 Eye Protection

Protection Level: for protection against flying particles - dust /foreign particles.
MOC:
Lens - polycarbonate, UV protection, optical class 1 ZERO power
Frame -resistant to solvent
Approval: Conforming to IS-5983:1980/EN: 166 1F and EN: 170/ANSI Z87.1
Marking: as per applicable approval criteria
Shall be free from visual defects - frame /apparent scratches - lens.
Safety Goggles - non ventilated or with direct/indirect vents

Protection Level: fits the face, surrounding the eyes in order to shield them from impact, splash and vapor
hazards.
MOC:
Lens: Optional - Polycarbonate or Acetate, UV protection, optical class 1 ZERO power and resistant to
impact.
Frame: vinyl is preferable - resistant to solvents/hydro carbon vapour with strap preferably Neoprene.
Special Feature:
Distortion-free peripheral vision can be worn over prescription eyewear and with half-mask respirator.
Approval: Conforming to IS-5983:1980/EN: 166 1F and EN: 170/ANSI Z87.1
Marking: as per applicable approval criteria
Shall be free from visual defects - frame /apparent scratches - lens.
Welder's Goggle (soft frames with single / two eye-pieces lens) / Welding Shield with adjustable headgear
(robust fiber body)

Protection Level: protection from harmful light rays, flying sparks, metal spatters and slag chips.
Lens: designed to filter, absorb or reflect harmful radiations emitted during arc welding, cutting operation,
recommended shade lens # 10 for electric arc welding and # 5 for gas welding/cutting operation.
Special Features: full face protection (for welding shield)
Approval: Conforming to IS-5983:1980/ EN: 169/EN: 175/ANSI Z87.1

3.0 Hand Protection

Canvas/Knitted Cotton Hand Gloves


Protection Level: general purpose
MOC - 100 % cotton material Features Stitching: thread can be inseam or out seam, 6 to 8 stitches per 1
inch.
Design: comfort fit.
Length: recommended min 12 inch for Canvas HG and 10 inch for Knitted cotton hand gloves. Shall
be free from visual defects

Leather Hand Gloves


Protection Level: Protection against abrasion and cut injury
MOC - chrome tanned split/grain leather soft, pliable and free from fibres
Feature -
Thickness: 1.0 to 2.0 mm
Stitching - thread can be inseam or out seam, 6 to 8 stitches per 1 inch. Cuff shall be stitched by a double
row of sewing. Design: comfort fit
Length: min 11.5 inch Canvas HG and 10 inch Knitted cotton HG Approval: EN
388 / IS 6994 (Part I) shall be free from visual defects

4.0 Safety Shoes - low shoe / ankle boot

MOC - made from Leather 1.0 to 2.0 mm thick

Steel Toe Cap for complete toe protection
Sole - Antistatic as well as oil, water, acid and abrasion resistant - Polyurethane
Lace - Cotton
Design: comfort fit
Approval: confirming to IS 15298:2002 (Part I and II) /EN 345 - steel toe Marking:
Manufacturer's identification mark
Date of manufacture - at least quarter and year
Shall be free from visual defects

5.0 Earplug - disposable / permanent


Protection Level: Attenuation level - min. NRR min 25 dB(A)
Features -
Soft and tapered shape, easy to insert into (permanent) / expand to exact shape of (disposable type) ear canal
and give comfortable feeling.
Hypo allergic
Approval: confirming to IS9167/EN 352
Shall be free from visual defects

6.0 Dust Mask - disposable

Protection Level: Filter Class P1/ FFP 1/FFP 1S level


Features -
Shape: moulded cup shaped design
Twin head bands having very good elastic retention properties ensure good face seal
Nose Clip - soft and pliable aluminium ensures comfort and secure face seal and reduce eyewear fogging.
Approval: IS15322/EN 149.
Shall be free from visual defects.
7.0 Full body Safety Harness 21

Full Body Harness with Lanyard - double Y shaped with energy/shock absorber
MOC -
Webbing and Rope - Polypropylene / Polyamide Thread
- Nylon/Polyester/Polyamide
Metal Components - Alloy Steel - corrosion resistant
Approvals: CE certified, EN approved or IS certified
Marking: CE marked / ISI marked as per IS 3521
Special Features:
Harness
One dorsal attachment D ring for fall arrest at back.
One/Two dorsal attachment D ring or textile loops for fall arrest at front.
Adjustable shoulder and thigh straps.
Has tell-tale indicator to illustrate if the harness has already had a fall.
The Anchor points shall be marked/indicated on the webbing.
Confirming to EN 361 and/or IS 3521.
Lanyard - 1.8 Mtr (6-feet) length
Double Y shaped Polyamide Lanyard equipped with a strand suggesting degradation of material.
Rope ends splices and covered with PP protective sleeve.
One side of the rope (attachment with dorsal D ring at back) is equipped with energy absorber while the
other free ends equipped with scaffold hook/anchorage hook as per utility requirement. Confirming to EN
355 and/or IS 3521.

8.0 INHERENTLY FLAME RESISTANT WORK APPAREL (IFR)

Provided with 2 chest pockets with zipper and protective fly over the zipper, two pockets on lower front and
back side, a utility / tool pocket on the right leg and a pen pocket on the left arm sleeve
Provided with full length heavy duty, two way brass zipper and a full length protective fly over the zipper.
Buttons (push type) shall be provided in the fly to keep it close
The threads used to stitch the clothing shall be of FR (Kevlar or equivalent) material.
The zipper used should also be made out of FR material with metallic teeth
Reflective tape must be affixed to coveralls to enhance visibility in low-light and man overboard
circumstances. The reflective tape shall be, 2” wide and affixed as follows:
Reflective tapes and button used shall heat resistance in their original form and shall not melt, separate or
ignite.
A separate strip of white color made of same fabric shall be provided over the right breast pocket to write
the name of the person.
Company logo and name shall be stitched in the left pocket. The logo shall be of 1.5 inch size and the letter
size is 1.5 cms,
Company logo shall be stitched in the back side of the coverall. The logo shall be of 4.5 inch size
Hooking provision shall be provided inside the collar to hang the coverall
Support letter from vendor to be asked for certifying on the genuinity of the fabric and accessories used
along with safety features.

Annexure 2: Penalties to be levied on the contractor for violation

(A) PPE violation (Not using proper PPE or damaged PPE while at work):
Penalty on Contractor per incident
1. Helmet : Rs. 100
2. Safety Shoes : Rs. 100
3. Safety Goggles : Rs. 100
4. Full Body Harness : Rs. 10,000
5. Hand Gloves : Rs. 100
6. Other PPE : Rs. 100

(B) Tools & Tackles violation : Penalty on Contractor per incident


1. Uncertified : Rs. 500
2. P&M without having Inspection : Rs. 1,000

(C) Safety Meeting violation : Penalty on Contractor per incident Not


attending the monthly safety meeting : Rs. 100

(D) Safety Audit violation : Penalty on Contractor per incident Non-


compliance of records/documents : Rs. 200

E) Housekeeping violation: Penalty on Contractor per incident


Housekeeping not in order around the job: Rs.100

1. Plant Mangers / Maintenance Manager / EICs and Apex shall be authorized to impose / authorize the
penalty.
2. Contractor may appeal for waiver of the penalty on case to case basis. Decision on waiver shall be
taken by Site head / E&M head / Operation head only.
