We are student - researchers of Gabi National High School - Senior High

School, Gabi, Compostela, Davao De Oro. As part of our two (2) weeks
School - Based Research Implementation, we are conducting an research
due to students computer - shortage experience. We are inviting you to
participate in our study. The aim of this research is to allow the students to
have a computer - like experience through our proposed innovation website.

This study used cluster sampling strategy to select respondent(s) for the
study. The main instrument to be used in the data collection is the adapt
survey questionnaire. The survey questionnaire will be used to collect
quantitative data on the factors associated experience and feedback of Gabi
National High School Students. This activity will take thirty to forty - five (30 -
45) minutes.

Your participation is completely voluntary. You may withdraw from this study
at any time without penalty.

All information obtained in this study will be kept strictly confidential. All
participants will be asked not to disclose anything said within the context of
the discussion. All identifying information will be removed from the collected
materials, and all materials will be stored securely.

With regard to being a respondent(s), please initial next to any of the

statements that you agree with.
