JOINT AFFIDAVIT OF TWO (2) DISINTERESTED PERSONS

We, ALEXANDER S. ARMEA and ELMER C. TULAYLAY, both of legal ages,

Filipino, and are residents of BF Condominium, Intramuros, Manila, respectively, after
having been duly sworn to in accordance with law, do hereby depose and state that:

1. We personally know JOSE JOY MIRABEL DE LOS SANTOS who is our long
time friend and the fact of his birth was registered at the Local Civil Registrar of
Florida Blanca, Pampanga;
2. We know for a fact that the person by the name “JOSE JOY MIRABEL DELOS
SANTOS” as appearing on his information in Pag-ibig Fund, and the name
“JOSE JOY MIRABEL DE LOS SANTOS”, refer to one and the same person
and that his true and correct name is “JOSE JOY MIRABEL DE LOS SANTOS”;
3. We are not in any way related to any of the foregoing person within the fourth
civil degree either by consanguinity or affinity, nor do we have any pecuniary
interest in this matter whatsoever;
4. To remove confusion as to the names, we hereby declare that the names
“JOSE JOY MIRABEL DE LOS SANTOS” and “JOSE JOY MIRABEL DELOS
SANTOS”, refer to one and the same person whose registered name is “JOSE
JOY MIRABEL DE LOS SANTOS”; and
5. We are executing this affidavit to attest to the truthfulness of the foregoing
statements and to establish the real identity of “JOSE JOY MIRABEL DE LOS
SANTOS” and “JOSE JOY MIRABEL DELOS SANTOS,” and for all legal
intents and purposes it may serve.

fact of his birth was registered at the Local Civil Registrar of San Jose Siaton,
Negros Oriental;
7. We know for a fact that the person by the name “CAROL G.QUIO” as appearing
on his information in SSS member NARIONECIAS G.QUIO as one (1) of the
beneficiary, and the name “CAROLINA G. QUIO”, refer to one and the same
person and that his true and correct name is “CAROLINA G. QUIO”;
8. We are not in any way related to any of the foregoing person within the fourth
civil degree either by consanguinity or affinity, nor do we have any pecuniary
interest in this matter whatsoever;
9. To remove confusion as to the names, we hereby declare that the names
“CAROLINA G. QUIO” and “CAROL G. QUIO”, refer to one and the same
person whose registered name is “CAROLINA G. QUIO”; and

accordance with law, do hereby depose and say:

1. THAT my name is BRIGIDA G. QUIO as stated in my Certificate of Live Birth.

Place of Birth is at San Jose Siaton, Negros Oriental;
2. THAT in my Marriage Certificate, my Date of Birth is on October 8, 1978 and my
father’s name Arsenio C. Quio not Anselmo C. Quio ;
3. THAT since childhood, my friends and acquaintances knows my Date of Birth is
on October 8, 1970 not October 8, 1978,; and thus I have been using the Date of
Birth in my personal records and ID’s, transactions and communications; and
4. THAT I am executing this Affidavit to attest to the truth of the foregoing facts and
to use the same for whatever legal purpose it may serve.
