AFFIDAVIT OF DENIAL

I, SHERWIN BARTOLOME SORIA, of legal age,Filipino, and with residence and

postal address at 91 B5 L6 Servants of Charity, Pingkian Road, Quezon City, after
having been sworn to in accordance with law, do hereby depose and say:
1. THAT I have a Bank Account registered with the Landbank of the Philippines;
2. THAT morning of March 23, 2024, I made a balance inquiry via online banking,
specifically through LANDBANK iAccess;
3. THAT in so doing, an OTP was required to push through said transaction.
However, I’ve entered OTP for four times as the steps took quite a long time to
proceed. More so, I was able to check my account’s balance and to which
amounts to PHP 40,270.26;
4. THAT to my surprise, around 7 to 8 o’clock in the evening of the same day, I
failed to withdraw cash amounting to PHP15,000 at an ATM located at 7-11
Tandang Sora-Commonwealth due to insufficient balance. Said information
was shown on the screen of the same machine;
5. THAT I made a balance inquiry at an ATM located at 7-11 Philand-Pasong
Tamo, Quezon City, and it shows that I only have PHP155.26 left;
6. THAT to my honest belief, there was an unauthorized transfer of money
amounting to PHP40,100, which was not made by me and without my
knowledge and consent; and
7. THAT I am executing this affidavit based on personal knowledge, belief, and
authenticity of the record; to prove the veracity and truthfulness of the
foregoing facts and to inform the Landbank of the Philippines and the proper
authorities concerned of the matter and whatever legal purpose the same may
serve best.
