TERMS AND CONDITIONS:
1. Payment of Job Order shall be paid every end of the month upon completion of the Job
Description;
2. Accomplishment Report and Daily Time Record (DTR) signed by both School Head and
worker shall be attached to the monthly claim for payment;
3. Job Order Contract Cost/Amount shall be Php 15,000.00 for the period: APRIL-JUNE,
2024 or Php 5,000.00 per month.
