AFFIDAVIT OF RELATIONSHIP

That I, GEMMA FUENTES BUELVA, of legal age, Filipino, with residence and postal
address at Ph10BPKG.6 Blk 94 Lot Excess, Accohomes, Bagong Silang, Caloocan City , after
having been duly sworn to in accordance with law do hereby depose and state to wit;

1. That I personally know LHEAN MEGS P. FUENTES, who died on November 09, 2023
at Rosario Maclang Bautista General Hospital. And I, GEMMA FUENTES
BUELVA, is her AUNTY on her father side.

2. That I am the only authorize person who can claim the body of LHEAN MEGS P.
FUENTES at Rosario Maclang Bautista General Hospital.

3. That her father, DANILO V. FUENTES, who is residing at Zone 7, San Vicente,
Tinambak, Camarines Sur, is not available of the moment and can’t claim the body
personally of her daughter because of weak signal on their area resulting to a loss of
communication and also because of it is difficult for him to travel far because of his
condition and because of the difficulty of transportation because he will still recover from
the mountain of Bicol.

That we are executing this affidavit to attest to the truth of all the foregoing facts mentioned
above and for whatever legal intents and purposes this may serve best.
