We are Grade 12 students of Los Banos Senior High School taking up

Humanities and Social Sciences and currently conducting a research entitled
“COMMUNICATION ANXIETY ON PUBLIC SPEAKING AND ACADEMIC
PERFORMANCE OF SELECTED GRADE 11 STUDENTS OF LOS BANOS SENIOR
HIGH SCHOOL”.

In connection with this, we humbly request your good office to give us permission to
administer and give a set of questionnaires to selected grade 11 students both STEM
and HUMSS on the 20th of March. The gathering of data needed for this study will be
done through face to face.
