The applicant above named most respectfully submits as under :-

1. The applicant submits that, she is a permanent resident of

above mentioned address.

2. The applicant further submits that she is legally wedded

wife of Mr. Sultan Salim Shaikh and their marriage
solemnized on 08.08.2010 at Thakur pada, Mumbra.

3. The applicant submits that due to above said marriage a

issue is born namely “FALAK SULTAN SHAIKH” at the
address mentioned above on 31/07/2018, but due to lack of
knowledge and ignorance of the law, the applicant could not

informed about birth of her daughter to the officials of

Mumbra Prabhag Samiti in stipulated time.

4. The applicant submits that she requires the birth certificate

of her daughter namely “FALAK SULTAN SHAIKH” for
her school admission purpose.

5. The applicant submits that her daughter was born at

Mumbra, Thane, within the jurisdiction of Mumbra Prabhag
Samiti, Thane Municipal Corporation and hence this
Hon’ble Court has jurisdiction to try, entertain, and dispose
of the present application.

6. The applicant submits that that she has not filed any other
application in this court or in any other court for the same
subject matter and this is the first application.

It is therefore prayed that,

a. This Hon’ble Court be pleased to order and direct the
Registrar of Birth & Death Department, Mumbra Prabhag
Samiti, Thane Municipal Corporation to register the birth
date of the applicant’s daughter namely “FALAK SULTAN
SHAIKH” as 31/07/2018.
b. Any other just and equitable order may be passed in the
interest of justice.
