I, ____________________, have inspected for deficiencies and checked-out the
above piece of equipment to be rented and agree to return the equipment in the
same condition as it was loaned to me.
I understand that if the equipment is lost or damaged, the borrower is responsible
for the current replacement cost or repair cost.
The borrower shall shoulder the shipping fees.
If the equipment has not been returned on the specified check-in time and date,
100% of the agreed upon rate will be added to the total borrowing fee.
