NOT TO PARCELIZE THE LANDHOLDING

ages, Filipinos, and co-owners of an agricultural land embraced by the Transfer Certificate
Title No. C-________ with an area of _______________________ (_____) SQUARE
METERS, MORE OR LESS, situated at Barangay ______________, Municipality of
_______________, Davao del Norte, do hereby depose and state:

1. We are executing this collective affidavit in connection with the implementation of the
Support to Parcelization of Land for Individual Titling (SPLIT) Project, wherein the Field
Validation Team (FVT) conducted consultations to gather essential information regarding the
actual area of tillage and the present land use of the farmer-beneficiaries' allotted area;

2. During the validation process, we made a conscious and voluntary decision not to
parcelize the aforementioned landholding because we have already waived portions of the said
property and lawfully disposed of it to another party;

3. We solemnly declare that this decision is made of our own free will, without any
coercion or undue influence upon us;

4. With this collective affidavit, we hereby confirm that we have consciously OPTED
NOT TO PARCELIZE the said landholding, as we have already relinquished a portion of our
rights, interests, and claims to the aforementioned property;

5. That, we fully understand the consequences of our decision to voluntarily opted NOT
to parcelize the subject property under the SPLIT Project;

6. We solemnly declare that the information provided herein is true and correct to the best
of our knowledge and belief.
