Please be advised of the following endorsements:

 Members whose names appear in the master list will avail of their Annual Physical Exam, only
once, at their convenience from April 1 to 30, 2024.
 Annual Physical Exam results must have original signatures of physicians and medical
technologists.
 The Annual Physical Exam results must be released within 2 weeks after the said examination
of each member has been completed. Please email the soft copy of results to authorized
companyrecipients: ph.ols.bpp@biposervice.com, teresia.liang@biposervice.com,
markanthony.perena@biposervice.com & markerone.cruz@biposervice.com.
 Forward the billing together with the master list provided by the undersigned, affixed with the
individual signature of members to Cocolife Healthcare-Claims Department.

The following are the examinations included in the package:

a) General Physical Examination & Medical History Taking
b) Routine Urinalysis
c) Routine Fecalysis
d) Complete Blood Count
e) Chest X-ray
f) Electrocardiogram (ECG) Covered for 35 years old and up
g) Pap’s Smear Covered for 35 years old and up
Examinations not listed above must be charged directly to the patient.
