WAIVER FORM

I/We, the undersigned, hereby willingly and voluntarily give full consent for my/our
son/daughter, _____________________________, to participate in the 2024
Regional Schools Press Conference in Ilocos Sur from April 9-13, 2024 (excluding
travel dates).
I/We understand that our son/daughter will not be staying in the Billeting school
(Bantay East Central School) assigned by the SDO Ilocos Sur during the duration of
the contest. Instead, He/she will be staying with the school paper advisers in
________________ (name of hotel) which 10-minute walk from the contest venue.
We, the parents/guardian, hereby fully release and discharge SDO Ilocos Sur,
Bantay East Central School, SDO Dagupan City, ______________ (school) and all
its School Paper Advisers from any and all claims for personal injury, property
damage, or other losses resulting from our son/daughter’s decision to stay in a
transient house instead of the assigned billeting school.
We hereby assume all risk of personal injury, property damage, and other losses
which may result from our decision to let our son/daughter stay outside the billeting
school during the duration of the contest.
We further agree to indemnify and hold harmless SDO Ilocos Sur, Bantay East
Central School, SDO Dagupan City, ______________ (name of school) and all its
School Paper Advisers from all claims, suits actions, injuries, damages, and losses
sustained/arising out of, connected with, or in any way associated with our decision
to not let our son/daughter stay in the billeting school during the duration of the
contest
