QUIT CLAIM AND RELEASE

KNOW ALL MEN BY THESE PRESENTS:

I, BIAY, EMMERSON MALANUM of legal age, Filipino, and with residence and postal address at
PANGPANG SAN CARLOS CITY do hereby acknowledge receipt of the sum of PHP 98,836.81 as
equivalent to the amount of US$ 1,784.38 based on the current official foreign rate, which constitutes
as payment for the Provident Fund by AMOSUP - PTGWO ITF and JSU - based on the length of
service of BIAY, EMMERSON MALANUM with the company per the service record hereto attached
as ANNEX "A".

Now, THEREFORE, the foregoing premises considered, I have hereunto remised, released, and
forever discharged, and do for myself, my heirs, executor and administrators, forever discharge the
AMOSUP - PTGWO-ITF and JSU, their successors, assigns, director, officers and members from any
and all manner of action or actions, cause/s of action, sum/s, of money, accounts damages or
members, I ever had or may have, or which our heirs, executors, administrators can or shall or may
have, upon or by reason or upon any matter, cause or thing in connection with, arising our of, and by
reason for the case aforementioned.
