This refers to the above captioned matter particularly relevant to the request to
tie up all property records in relation to assessment and/or tax mapping records of
Transfer Certificate of Title No. 99512, which certified true copy, property plan and tax
clearance are hereto attached for easy reference. For this purpose it is humbly
requested that aforesaid parcel of land covered with Transfer Certificate of Title No.
99512 be linked and entered/included in the Tax Declaration and/or property
assessment and tax records of your Honorable Office.

It is likewise requested that certified copies of assessment records of the said

real property and all other records relative to its assessment be issued for all legal
intents and purposes.
