Nadeeshika of “Wimala”, Porawagama, Elpitiya, been the Plaintiff of the
Divorce matter of District Court of Elpitiya has nominated, constituted, appointed
and do hereby nominate constitute and appoint Mr. Salakshana Kariyawasam
Attorney-at-Law, to be my instructing Attorney-at–Law and to appear for me and
in my name and behalf before the District Court of Elpitiya of the Democratic
Socialist Republic of Sri-Lanka, to appear and therein to file papers and to take all
necessary steps.

And to receive and to take all moneys that may be paid to him by me in the said
action, and to move for and obtain in my name any order or orders from the said
court for any payments of any sum or sums of money that may be deposited
therein in respect of me, and to give all necessary receipts, releases and discharges
therefore.
I do further authorize and empower my said Attorney-at –Law to take and use all
lawful ways and means and to do and perform all such acts, matters and things as
may be necessary in and about the premises which I being personally present
